University of Rennes, CHU Rennes, Inserm, COSS1242, CIC 1414, Anesthesia and Intensive Care Department, Rennes, France. Electronic address:

Background: The way that pain is assessed in the PACU could impact on postoperative pain and analgesic consumption. However, there is currently no evidence to support this speculation. The authors hypothesised that using a comfort scale reduces postoperative opioid consumption when compared with a standard numerical rating scale (NRS) to evaluate pain in the PACU.

Background: Out-of-hospital cardiac arrest (OHCA) represents a major burden for society and health care, with an average incidence in adults of 67 to 170 cases per 100,000 person-years in Europe and in-hospital survival rates of less than 10%. Patients and practitioners would benefit from a prognostication tool for long-term good neurological outcomes.

Objective: We aim to develop a machine learning (ML) pipeline on a local database to classify patients according to their neurological outcomes and identify prognostic features.

Article Synopsis
  • Cerebral venous sinus thrombosis due to vaccine-induced immune thrombotic thrombocytopenia (CVST-VITT) can lead to severe bleeding and high mortality, particularly among patients requiring decompressive surgery post-COVID vaccination.
  • A study reviewed 128 CVST-VITT patients, finding that 27% underwent surgery, with a 65% in-hospital mortality rate in this group compared to 29% in non-surgical patients, highlighting factors like preoperative coma and pupillary reflex absence as key mortality predictors.
  • Despite the high mortality, around 80% of the surgical survivors were functionally independent at a 6-month follow-up, indicating some potential for recovery even after critical interventions.

Article Synopsis
  • Fiberoptic intubation training for difficult airways is complex and usually requires substantial experience, with previous simulations focusing mainly on normal airways.
  • A study involving 86 anesthesia trainees compared training on a normal airway versus training on a normal airway followed by difficult scenarios, finding that the normal airway training group performed better on familiar scenarios.
  • Ultimately, training on difficult airways did not improve skills on unseen difficult cases, suggesting that multiple exposures to a specific airway type are necessary for proficiency.

Purpose: To evaluate and compare which factors are relevant to the diagnostic decision-making and imaging workup of intracerebral hemorrhages in large, specialized European centers.

Methods: Expert neuroradiologists from ten large, specialized centers (where endovascular stroke treatment is routinely performed) in nine European countries were selected in cooperation with the European Society of Neuroradiology (ESNR). The experts were asked to describe how and when they would investigate specific causes in a patient who presented with an acute, atraumatic, intracerebral hemorrhage for two given locations: (1) basal ganglia, thalamus, pons or cerebellum; (2) lobar hemorrhage.

Background: Hypotension and blood pressure (BP) variability during endovascular therapy (EVT) for acute ischemic stroke (AIS) due to an anterior large vessel occlusion (LVO) is associated with worse outcomes. However, the optimal BP threshold during EVT is still unknown given the lack of randomized controlled evidence. We designed the DETERMINE trial to assess whether an individualized BP management during EVT could achieve better functional outcomes compared to a standard BP management.

Like modern metagenomics, ancient metagenomics is a highly data-rich discipline, with the added challenge that the DNA of interest is degraded and, depending on the sample type, in low abundance. This requires the application of specialized measures during molecular experiments and computational analyses. Furthermore, researchers often work with finite sample sizes, which impedes optimal experimental design and control of confounding factors, and with ethically sensitive samples necessitating the consideration of additional guidelines.

Crosstalk between brain, lung and heart in critical care.

Anaesth Crit Care Pain Med

August 2020

Department of anaesthesia and critical care, university hospital of Toulouse, university Toulouse 3 Paul Sabatier, Toulouse, France.

Extracerebral complications, especially pulmonary and cardiovascular, are frequent in brain-injured patients and are major outcome determinants. Two major pathways have been described: brain-lung and brain-heart interactions. Lung injuries after acute brain damages include ventilator-associated pneumonia (VAP), acute respiratory distress syndrome (ARDS) and neurogenic pulmonary œdema (NPE), whereas heart injuries can range from cardiac enzymes release, ECG abnormalities to left ventricle dysfunction or cardiogenic shock.

Article Synopsis
  • - The study investigates the effectiveness of a revised computed tomography angiography (CTA) score that includes superior petrosal veins (SPVs) for confirming brain death (BD), comparing it to the established 4-point score.
  • - The research analyzed 69 adult patients who met clinical brain death criteria, finding that the new "Toulouse score" showed a positive predictive value of 100% and the lowest misclassification rate compared to the 4-point score.
  • - The results suggest that incorporating SPVs and middle cerebral arteries into the score aids in accurately confirming brain death, maintaining similar sensitivity to the traditional 4-point score, especially in cases where craniectomy has occurred.
