Purpose: A previous integrated patient-level analysis demonstrated a significant benefit of implementing once-monthly injectable paliperidone palmitate (PP1M) earlier in the treatment course for schizophrenia. Earlier therapeutic interventions during the first 3-5 years after disease onset can positively impact long-term outcomes in schizophrenia. This present analysis evaluated the risk of relapse both overall and by different durations of illness (0-3 years, >3-5 years, and >5 years from diagnosis) in adult patients with schizophrenia who received PP1M or oral antipsychotics (OAPs).

The emerging field of implementation science (IS) facilitates the sustainment of evidence-based practice in clinical care. This article, the second in a series on applying IS, describes how a nurse-led IS team at a multisite health system implemented the Brøset Violence Checklist-a validated, evidence-based tool to predict a patient's potential to become violent-in the system's adult EDs, with the aim of decreasing the rate of violence against staff. The authors discuss how they leveraged IS concepts, methods, and tools to achieve this goal.

Article Synopsis
  • - The study aimed to assess the effectiveness of a computational phenotype (CP ABD) in identifying acute brain dysfunction (ABD) in pediatric sepsis patients, which occurs in about 20% of cases but is difficult to detect.
  • - Researchers tested CP ABD against traditional methods (Glasgow Coma Scale and delirium scores) using a dataset of 527 children, finding that CP ABD had the highest diagnostic performance, sensitivity (83%), and specificity (93%).
  • - The findings suggest that CP ABD is a superior tool for identifying confirmed ABD in pediatric sepsis, providing a reliable method for future research using large electronic health records.

The purpose of this study was to examine the association of physical activity with socioeconomic conditions, demographic factors, depression, and loneliness among older adults with diabetes in the United States. Using data from Wave 3 of the National Social Life, Health, and Aging Project (NSHAP), we found that male respondents, those with incomes greater than $100,000, and those with less depression were more likely to be physically active among older adults with diabetes. Education level and loneliness were not significant factors influencing physical activity among older adults with diabetes.

June 2024

From the University of Pennsylvania School of Nursing, Philadelphia, PA (SA); University of Pennsylvania Perelman School of Medicine, Philadelphia, PA (RF, AS, ES, NO, JP, ML); University of Pennsylvania Center for Emergency Care Policy and Research, Philadelphia, PA (ES); University of Pennsylvania Urban Health Lab, Philadelphia, PA (SA, ES); and University of Pennsylvania Center for Addiction Medicine and Policy, Philadelphia, PA (SA, RF, NO, JP, ML).

Objectives: Buprenorphine is not reliably stocked in many pharmacies, and pharmacy-level barriers may deter patients from opioid use disorder care. We surveyed all outpatient pharmacies in Philadelphia to describe variation in buprenorphine access and developed a map application to aid in identifying pharmacies that stock the medication.

Methods: Using a dataset from the Bureau of Professional and Occupational Affairs, we conducted a telephone survey of operating outpatient pharmacies (N = 422) about their buprenorphine stocking and dispensing practices.
