Background: In an aging surgical patient population, preventing complications such as oversedation has taken increasing priority in perioperative care. Intraoperative use of virtual reality (VR) may decrease sedative requirements. We hypothesize that the use of immersive VR during total knee arthroplasty (TKA) will lead to decreased propofol requirements, improved patient-reported satisfaction, and reduced postoperative opioid requirements compared to active and usual care controls.

Using Compressed Virtual Reality Exposure Therapy for an Adult Critical Illness Survivor: A Case Report.

Crit Care Nurse

December 2024

Caitlin L. Cox is a clinical research coordinator at the Orlando Health Heart & Vascular Institute, Orlando, and a PhD student in the University of Central Florida College of Nursing.

Article Synopsis
  • Nearly 25% of adult survivors of critical illness experience PTSD symptoms after ICU discharge, often triggered by sensory stimuli, with delirium being a significant risk factor for PTSD.
  • A case study involved a survivor who faced PTSD symptoms and fears of illness that led her to self-isolate, which prompted her to participate in a clinical trial for Compressed Virtual Reality Exposure Therapy (C-VRET).
  • Post-therapy results showed significant reductions in PTSD and depression scores, increases in physical activity, and improvements in resiliency, indicating C-VRET's potential effectiveness for PTSD in critical illness survivors.

Article Synopsis
  • - Wong-type dermatomyositis (WTDM) is a rare variant of dermatomyositis first identified in 1969, combining features of classic dermatomyositis and pityriasis rubra pilaris, but has limited documentation due to its rarity and possible clinical oversight.
  • - A review of existing literature suggests that WTDM behaves more like a subtype of classic dermatomyositis rather than just a combination of symptoms from both conditions, indicating a better treatment response with traditional DM therapies.
  • - Clinicians are advised to conduct thorough evaluations for WTDM patients, including detailed medical histories, exams, and lab tests, and to monitor for systemic issues or cancer, while further research is needed to explore its underlying mechanisms and long

Background: Muscle-invasive urothelial carcinoma is an aggressive disease with high rates of relapse. Whether pembrolizumab as adjuvant therapy would be effective in patients with high-risk muscle-invasive urothelial carcinoma after radical surgery is unknown.

Methods: In this phase 3 trial, we randomly assigned patients, in a 1:1 ratio, to receive pembrolizumab at a dose of 200 mg every 3 weeks for 1 year or to undergo observation.

Introduction: The renin-angiotensin system (RAS) has been demonstrated to modulate cell proliferation, desmoplasia, angiogenesis and immunosuppression. We examined the association of RAS inhibitors (RASi)-namely angiotensin converting enzyme inhibitors (ACEi) and angiotensin receptor blockers (ARB)-with neoadjuvant chemotherapy (NAC) for muscle-invasive bladder cancer (MIBC) preceding radical cystectomy (RC).

Patients And Methods: We retrospectively investigated concurrent RASi use with NAC prior to RC in 302 patients with MIBC from 3 academic institutions.

Mantle cell lymphoma (MCL) is a mature B-cell lymphoma associated with cyclin D family rearrangements and typically expresses CD5 and cyclin D1. Epstein-Barr virus- (EBV-) positive MCL is rare, and the role of EBV infection and its transformation in MCL remains unclear. We present a case of CD5-negative classic MCL that progressed to an EBV + pleomorphic MCL six years after the initial diagnosis.

Stable laser resonators support three fundamental families of transverse modes: the Hermite, Laguerre, and Ince Gaussian modes. These modes are crucial for understanding complex resonators, beam propagation, and structured light. We experimentally observe a new family of fundamental laser modes in stable resonators: Boyer-Wolf Gaussian modes.
