Article Synopsis
  • - This study investigates Petersen's hernia, which can occur after specific types of stomach surgeries, and compares two methods for preventing bowel obstruction caused by this hernia: the conventional defect closure method and the newer mesenteric fixation method.
  • - Conducted as a randomized controlled trial in Korea, the research includes patients with diagnosed gastric cancer who have undergone certain types of surgical anastomosis and aims to determine the effectiveness of each method in preventing bowel obstruction over a three-year period.
  • - Results from this trial are expected to provide valuable insights on the efficacy and safety of the mesenteric fixation method, potentially establishing it as a superior technique for closing Petersen's defects.

Article Synopsis
  • Coronary bifurcation angles and vessel tortuosity can affect plaque formation and blood flow, potentially leading to significant issues like myocardial ischemia.
  • A study analyzed 133 patients with angina to measure the impact of these anatomical factors on the severity of left anterior descending coronary artery disease.
  • Findings indicated that the combination of vessel curvature and angle (measured as d20*cosθ) serves as a strong predictor for significant LAD stenosis, emphasizing increased risks with greater tortuosity and altered angles.

Malnutrition and Associated Factors in Acute and Subacute Stroke Patients with Dysphagia.

Nutrients

August 2023

Department of Rehabilitation Medicine, Gyeongsang National University College of Medicine and Gyeongsang National University Hospital, Institute of Medical Science, Gyeongsang National University, Jinju 52727, Republic of Korea.

Most patients with stroke suffer from complications and these include dysphagia. Dysphagia can cause malnutrition, and malnutrition affects prognosis and recovery. However, there is a lack of accurate studies on the nutritional status of stroke patients with dysphagia and its associated factors in different phases of stroke.

A 63-year-old man developed chest pain and dyspnea. Venoarterial-venous extracorporeal membrane oxygenation (ECMO) was applied to the patient due to failing heart after percutaneous coronary intervention. We used an additional ECMO pump without an oxygenator for transseptal left atrial (LA) decompression and performed a heart transplant.

Purpose: By analyzing sentinel basin dissection (SBD) data from the SEntinel Node ORIented Tailored Approach (SENORITA) trial, we sought to determine the precise extent of the sentinel basin (SB) without a tracer.

Materials And Methods: This study investigated SB length in patients (n = 25) who underwent laparoscopic SBD for early gastric cancer (EGC) in the SENORITA trial. SB length along the greater curvature (GC) and lesser curvature (LC) was measured intraoperatively before performing SBD.

Osteochondritis dissecans of the metacarpal head in a soldier treated with osteochondral autograft transplantation surgery: A case report.

Medicine (Baltimore)

January 2023

Department of Orthopedic Surgery, Yeouido St. Mary's Hospital, College of Medicine, The Catholic University of Korea, Seoul, Republic of Korea.

Rationale: Osteochondral autograft transplantation surgery (OATS) was introduced as a treatment for hand osteochondritis dissecans (OCD), and successful results were reported. However, the cartilage height of the donor plug has not yet been established compared with that of other joints. We present a case using OATS for OCD of the 3rd metacarpal (MCP) head that occurred in a soldier and the findings of magnetic resonance imaging (MRI) images taken at 12 weeks after surgery.

Background/aims: Anorectal functions are influenced by gender and age. This study sought to define the normal anorectal pressure values measured with conventional anorectal manometry (ARM) and to evaluate the effects of age and gender on anorectal function in asymptomatic subjects.

Methods: Conventional ARM was used to measure the anorectal pressures of 164 asymptomatic healthy subjects, including 86 males and 76 females.

Objective: This present study was conducted to determine the structural-mechanical stability of various fixation constructs through finite element (FE) analysis following simulation of a basicervical fracture and to introduce the clinical implications.

Materials And Methods: We simulated fracture models by using a right synthetic femur (SAWBONES®). We imported the implant models into ANSYS® for placement in an optimal position.

Background: To compare the incidence and severity of ipsilateral shoulder dysfunction and lymphedema of 2 groups of patients needing to undergo unilateral breast cancer surgery, one of which had only received printed education materials and the other group which had received educational materials plus preoperative education.

Methods: We selected 61 patients who had been diagnosed with unilateral breast cancer and planned to undergo surgery. Before surgery, patients were randomly assigned, either to a control group that only received printed education materials about exercise for shoulder pain relief and lymphatic edema prevention following breast cancer surgery, or to an experimental group that received the printed education material with personal education.

Article Synopsis
  • This study examines the differences in spinal stress during various movements (flexion-extension, lateral bending, and axial rotation) between osteoporotic and normal spines using a 3D finite element model.
  • Findings show that osteoporotic spines experience higher von Mises stress in the nucleus pulposus and certain annulus fibrosus areas during all movements compared to normal spines.
  • The results suggest that increased stress in the lumbar region, particularly in osteoporotic vertebrae, may help explain disc-related pain and degeneration.

  • - Nilotinib, a tyrosine kinase inhibitor, is approved for treating Philadelphia chromosome-positive chronic myeloid leukemia (Ph+ CML) and this study aimed to evaluate its safety and effectiveness in real-world settings in South Korea.
  • - The 12-week study involved 669 adult patients with Ph+ CML, monitoring adverse events (AEs) and treatment responses, finding that 61.3% experienced AEs and 89.5% achieved a complete hematological response (CHR).
  • - The results indicated that nilotinib was generally well tolerated, with consistent safety and efficacy compared to earlier research, affirming its use in routine clinical practice for these patients.

Patients with weak or no symptoms accelerate the spread of COVID-19 through various mutations and require more aggressive and active means of validating the COVID-19 infection. More than 30% of patients are reported as asymptomatic infection after the delta mutation spread in Korea. It means that there is a need for a means to more actively and accurately validate the infection of the epidemic via pre-symptomatic detection, besides confirming the infection via the symptoms.

Purpose: To determine the performance of quantification of liver extracellular volume fraction (fECV) using dual-energy CT (DECT) compared with CT imaging for ruling out high-riskesophageal varices(HRV) in cirrhotic patients.

Methods: We retrospectively analyzed 229 cirrhotic patients (training [n = 159] and internal validation cohorts [n = 70]) who underwent dual-source DECT, serum marker assessment, and esophagogastroduodenoscopy (EGD) from 2017 to 2020. The fECV score was measured using iodine maps from 3-minute delayed, equilibrium-phase images at 100/140 Sn kVp.

Article Synopsis
  • The study examines how the volume of gastric remnants (GRVs) after gastrectomy affects patients' quality of life (QOL), specifically comparing those with small vs. large remnant volumes.
  • Researchers collected data from 78 gastric cancer patients who underwent distal gastrectomy, using questionnaires and CT scans to assess their QOL over time.
  • While dysphagia (difficulty swallowing) showed significant differences between the two groups after 6 months, overall QOL scores did not vary significantly at other time points, suggesting GRV volume has minimal impact on long-term functional outcomes.
