Ketoconazole revisited: a preoperative or postoperative treatment in Cushing's disease.

Eur J Endocrinol

January 2008

Department of Endocrinology, Diabetes, Metabolic Diseases and Nutrition, Hôpital de la Timone, Centre Hospitalier Universitaire de Marseille and Faculté de Médecine, Université de la Méditerranée, 264 rue St Pierre, Cedex 5, 13385 Marseille, France.

Context: Although transsphenoidal surgery remains the first-line treatment in Cushing's disease (CD), recurrence is observed in about 20% of cases. Adjunctive treatments each have specific drawbacks. Despite its inhibitory effects on steroidogenesis, the antifungal drug ketoconazole was only evaluated in series with few patients and/or short-term follow-up.

Unlabelled: Corticotropin-releasing hormone (CRH)-stimulated petrosal sinus sampling is currently the gold standard method for the differential diagnosis between pituitary and ectopic ACTH-dependent Cushing's syndrome. Our objective was to determine sensitivity and specificity of desmopressin test during petrosal sinus sampling.

Patients And Methods: Forty-three patients had petrosal sinus sampling because of the lack of visible adenoma on magnetic resonance imaging (MRI) and/or because of discordant cortisol response to high-dose dexamethasone suppression test.

Gamma knife radiosurgery is a successful adjunctive treatment in Cushing's disease.

Eur J Endocrinol

January 2007

Federation of Endocrinology, Diabetes, Metabolic Diseases and Nutrition, Hôpital de la Timone, Centre Hospitalier Universitaire de Marseille and Faculté de Médecine, Université de la Méditerranée, 264 rue St Pierre, cedex 5, 13385 Marseille, France.

Objective: Though transsphenoidal surgery remains the first-line treatment of Cushing's disease, recurrence occurs frequently. Conventional radiotherapy and anticortisolic drugs both have adverse effects. Stereotactic radiosurgery needs to be evaluated more precisely.

Background: State-of-the art cardiac pacemakers are protected against radiofrequency signals. Although there have been earlier clinical and in vitro reports of cellular phone interference with implantable devices, only a few studies have been performed in recent years. The ringing phase of digital GSM or PCS cellular phones includes a brief period of peak radiated power.

Outcome of gamma knife radiosurgery in 82 patients with acromegaly: correlation with initial hypersecretion.

J Clin Endocrinol Metab

August 2005

Federation of Endocrinology, Diabetes, Metabolic Diseases and Nutrition, Hôpital de la Timone, Centre Hospitalier Universitaire de Marseille and Faculté de Médecine, Université de la Méditerranée, Marseille, France.

Context: Because surgical and medical therapies of acromegaly all have specific limitations, radiotherapy has been used as an adjunctive strategy. Stereotactic radiosurgery has not yet been widely evaluated.

Objective: The objective was to perform an analysis of long-term hormonal effects and tolerance of gamma knife radiosurgery.

We report the case of a patient with a giant interatrial septum aneurysm who was admitted to our hospital for analysis of palpitations. Transthoracic echocardiography was not contributive and cardiac magnetic resonance imaging demonstrated a small interatrial septal aneurysm. In our study, only transesophageal echocardiography provided the correct diagnosis, showing a giant interatrial septal aneurysm protruding far away into the right atrium and mimicking a right atrial cystic tumor.

We report the cases of two patients with cerebral embolization and interatrial septal aneurysm. Transesophageal echocardiography required for stroke showed right-sided interatrial septal aneurysm in the first patient and left-sided interatrial septal aneurysm in the second. In the two patients, interatrial septal aneurysm was mimicking atrial cystic tumor or abscess.

To investigate the usefulness of Cyfra 21-1 as an indicator of therapy effectiveness and prognosis in advanced primary lung cancer, sixty-three patients were selected on the basis of a high Cyfra 21-1 serum level (> 3.3 ng/ml) at the time of diagnosis. Serial assays of Cyfra 21-1 were performed during the first three courses of chemotherapy among 63 patients.
