Objective: To assess the prognostic value of clinical and biological features in patients treated with second-line targeted therapies (TT) for a metastatic renal cell carcinoma (mRCC).

Material And Methods: A retrospective study was performed including 60 patients treated with second-line TT from 2006 to 2013. Clinical and biological features were collected, including TT-induced toxicities, Heng and MSKCC prognostic scores, and renal function.

[Fluorescence endoscopy of the respiratory tract].

Rev Mal Respir

October 2007

Clinique Pneumologique, Hôpital Charles-Nicolle, Centre Hospitalier et Universitaire de Rouen, Rouen cedex, France.

Autofluorescence endoscopy has been used for more than 10 years in the diagnosis of early lung cancers and precancerous lesions of the bronchial tree. The technique has been extensively evaluated during the past decade and two recent large randomised studies have shown a 2 to 5 times increase in the detection of high grade pre-cancerous lesions compared with conventional white light endoscopy. This paper reviews the principal applications and results of the use of autofluorescence endoscopy in high risk individuals as well as innovative endoscopic approaches using the fluorescent properties of the respiratory tract.

Objective: to evaluate whether faecal incontinence can be improved by treatment of constipation in elderly patients with faecal incontinence associated with impairment of rectal emptying.

Design: a prospective randomized study with a 2-month follow-up.

Setting: five long-term care units.

Purpose: This study was conducted to evaluate the incidence, identify the risk factors, and assess the prognosis of elderly institutionalized patients who develop fecal incontinence.

Patients And Methods: We enrolled 1,186 patients 60 years of age and older living in long-term care facilities who did not have fecal incontinence. We assessed their medical history, treatment, mobility, and cognitive function.

The aim of this retrospective study was to determine the clinical spectrum of group B streptococcal (GBS) bacteraemia in patients over 70 years old. Sixty-six adults with GBS bacteraemia were reviewed over a 5-year period. Disease characteristics, clinical diagnoses and underlying disease were compared in 33 older patients (mean age 82.

Background: After heart transplantation, accelerated coronary vasculopathy is a major factor that limits long-term survival and is usually detected by serial coronary angiography. The aim of this study was to determine whether dobutamine stress echocardiography could accurately identify the progression of cardiac allograft vasculopathy.

Methods: Two sequential controls by dobutamine stress echocardiography were performed at an 18-month interval in 37 heart transplant recipients at the time of their routine coronary angiography.

Background: Atrioventricular association is a key criterion for arrhythmia diagnosis. Its use in a defibrillator should significantly reduce the incidence of inappropriate shocks. Therefore, we evaluated the diagnostic accuracy of an algorithm that uses dual-chamber sensing and analysis of atrioventricular association to discriminate ventricular from supraventricular arrhythmias in a prototype of an implantable defibrillator.

Electrocardiographic patterns and results of radiofrequency catheter ablation of clockwise type I atrial flutter.

J Cardiovasc Electrophysiol

October 1996

Service de Cardiologie, Centre Hospitalier et Universitaire de Rouen, Hospital Charles Nicolle, France.

Introduction: Counterclockwise right atrial propagation is usually observed in common atrial flutter, but little is known regarding flutter with clockwise right atrial rotation. The aim of this study is to describe the ECG characteristics and results of catheter ablation of atrial flutter with clockwise right atrial rotation.

Methods And Results: Among the 38 patients with type I atrial flutter in this study population, right atrial impulse propagation was counterclockwise in 20 and clockwise in 8.

Radiofrequency catheter ablation of atrial tachycardias.

Am Heart J

March 1996

Service de Cardiologie, Centre Hospitalier et Universitaire de Rouen, Hopital Charles Nicolle, Groupe de Recherche VACOMED, France.

Atrial tachycardias are frequently unresponsive to medical therapy. His bundle ablation has been proposed as a palliative treatment to treat symptoms and prevent development of tachycardia-mediated cardiomyopathy. Experience with catheter ablation directed at the atrial origin of the tachycardia remains limited.

Actimetry in newborns is relevant if two actimeters are placed, one on each ankle. This study has been conducted on nine normal three days old newborns. It has shown an indisputable link between the lack of activity and the observed sleep during the night that was missing during the day.

Background: After standard orthotopic heart transplantation (Sd HT), the enlarged resultant atria may promote atrial thrombosis. The purpose of this study was to compare the incidence of spontaneous echo contrast and left atrial thrombosis after Sd HT and total orthotopic (Tot HT) heart transplantation.

Methods And Results: Transesophageal echocardiography (TEE) was performed in 75 patients with Sd HT and in 20 patients with Tot HT.

Background: Radiofrequency energy has demonstrated its efficacy in catheter ablation of atrial flutter (AFl). However, long-term recurrences of AFl have been reported frequently after initial, apparently successful ablation. To date, criteria for prediction of late recurrences are lacking.

Evaluation of large-needle biopsy for the diagnosis of cancer.

Acta Cytol

June 1995

Laboratoire d'Histologie, Centre Hospitalier et Universitaire de Rouen, Hôpital Charles Nicolle, France.

The arguments for a choice between a large or fine needle in the diagnosis of tumors are still unclear. This paper reviews the advantages and disadvantages of large-needle biopsy and fine needle aspiration. Reports indicate that although the procedures have the same diagnostic efficacy, the risk of tumor seeding is far higher following large-needle biopsy.

The aim of the study was the detection of spontaneous echo contrast (SEC) and left atrial thrombus by transoesophageal echocardiography (TEE) in patients who had undergone orthotopic heart transplantation. TEE was prospectively performed in 64 heart transplant recipients (53 males, 11 females, mean age 51 years). Since surgery (mean time: 31 months), all heart transplant recipients had received either aspirin (39), or dipyridamole (22), or both (3).

Objectives: In two patients with orthotopic heart transplantation, the surface electrocardiogram suggested interaction between the donor right atrium and the recipient right atrium. An electrophysiologic investigation was performed to assess possible atrioatrial conduction.

Background: After orthotopic heart transplantation, both recipient and donor atrial activities are usually independent, but in humans they may synchronize for short periods during exercise.

The case of a 68-year-old woman with polyarthropathy and skin nodules is presented. Histologic findings of histiocytes with ground-glass cytoplasm favored the diagnosis of multicentric reticulohistiocytosis. In this article we report new electron microscopic findings of intra- and extracytoplasmic long spacing collagen (type VI).

The instantaneous amplitude of different EEG patterns found in a 36 week newborn baby were mapped by isopotential display, using 3 different references. We found that in certain cases, the nose reference recalculated average and source derivations give the same results. If the different EEG activities on the skull have too an high amplitude, and are not in phase opposition, the average reference differs from the zero potential of the common reference.
