Background: Same day discharge (SDD) diagnostic coronary angiography and percutaneous coronary interventions (PCIs) are increasingly performed, and indications extend to more complex procedures and more fragile patients. We report the evolution of SDD interventional cardiology activity in our centre since 2016, particularly before and after the COVID-19 pandemic. Secondarily, we analysed the feasibility and safety of SDD PCI.

[Maternal mortality due to venous thromboembolism in France 2016-2018].

Gynecol Obstet Fertil Senol

April 2024

Département de Maïeutique UFR Simone Veil-Santé, université de Versailles Saint-Quentin en Yvelines - Paris Saclay, Montigny-le-Bretonneux, France; Service de gynecologie - obstétrique, centre hospitalier de Rambouillet, Rambouillet, France.

Pregnancy and the post-partum period represent a thromboembolic risk situation, with pulmonary embolism (PE) remaining one of the leading causes of direct maternal deaths in developed countries. Between 2016 and 2018 in France, twenty maternal deaths were caused by venous thromboembolic complications (VTE), yielding a Maternal Mortality Ratio (MMR) of 0.9 per 100,000 live births (95%CI 0.

[Maternal mortality and health care organization in France 2016-2018].

Gynecol Obstet Fertil Senol

April 2024

Équipe épidémiologie obstétricale périnatale et pédiatrique (EPOPé), CRESS U1153, Inserm, université Paris Cité, Paris, France.

Article Synopsis
  • When assessing mortality cases, the organization of care is a crucial element, focusing on factors like the right place of care, timely transfers, adequate resources, and communication between caregivers.
  • From 2016-2018, 24% of assessed cases showed preventability factors related to care organization, with communication issues being the most common, followed by inappropriate care settings and insufficient resources.
  • To improve care, better emergency response in maternity units and enhanced coordination with other medical sectors are needed, particularly in managing patients with combined psychiatric and somatic issues.

[Maternal mortality due to obstetric haemorrhage in France 2016-2018].

Gynecol Obstet Fertil Senol

April 2024

Pôle femme et enfant, hôpital Estaing, CHU de Clermont-Ferrand, Clermont-Ferrand, France.

Article Synopsis
  • Between 2016 and 2018, there were 20 maternal deaths due to obstetric haemorrhage, resulting in a mortality ratio of 0.87 per 100,000 live births and accounting for 7.4% of all maternal deaths within a year.
  • The rate of maternal mortality from obstetric haemorrhage has decreased significantly since the early 2000s, but it remains a leading direct cause of maternal deaths, with many cases considered preventable due to shortcomings in care.
  • Most deaths from haemorrhage during this period occurred during caesarean deliveries, often linked to complications like uterine rupture and surgical injuries, highlighting the need for hospitals to be prepared for such emergencies.

Commercial human frozen plasmas for local, cross-site and long-term comparability of coagulation analysers.

Blood Coagul Fibrinolysis

January 2024

Laboratoire de Biologie Médicale (secteur hémostase), Hôpital André Mignot - Versailles, Le Chesnay.

Laboratory compliance implies to correlate instruments for coagulation parameter with a wide range of measure using patient samples or commercialized sets of frozen plasmas. The aim of this study was to evaluate the intra, inter-reproducibility and long-term stability of ExpertCor Routine (ECR) plasma sets (Stago) on different parameters. The study was realized in two laboratories on four different instruments.

Aims: Heart failure (HF) is the leading cause of hospitalisation in the elderly in France. Early rehospitalisations are common, often through an emergency department. The aim of this study was to assess the impact of a primary care-hospital coordination network, with interventions by coordination nurses (IDEC), on the rehospitalisations after a first hospitalisation for acute decompensation in frail elderly HF patients.

Many health establishments have difficulty recruiting the nursing staff necessary for the proper functioning of their structures, particularly nurses and care assistants. In the Ile-de-France region, within the Groupement Hospitalier de Territoire 78 Sud, a survey was carried out among students at two nursing training institutes with the aim of gaining a better understanding of the criteria used to choose a first job at the end of training. This should provide guidance to human resources departments and care managers in their recruitment strategies.

Background: The efficacy of high flow nasal canula oxygen therapy (HFNO) to prevent invasive mechanical ventilation (IMV) is not well established in severe coronavirus disease 2019 (COVID-19). The aim of this study was to compare the risk of IMV between two strategies of oxygenation (conventional oxygenation and HFNO) in critically ill COVID 19 patients.

Methods: This was a bicenter retrospective study which took place in two intensive care units (ICU) of tertiary hospitals in the Paris region from March 11, to May 3, 2020.

Understanding the clinical presentation of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection and prognosis in children is a major issue. Children often present mild symptoms, and some severe forms require paediatric intensive care, with in some cases a fatal prognosis. Our aim was to identify the epidemiological characteristics, clinical presentation, and prognosis of children with coronavirus disease 2019 (Covid-19) hospitalized in Paris suburb hospitals.

Background: Systemic sclerosis-associated interstitial lung disease (ILD) carries a high mortality risk; expert guidance is required to aid early recognition and treatment. We aimed to develop the first expert consensus and define an algorithm for the identification and management of the condition through application of well established methods.

Methods: Evidence-based consensus statements for systemic sclerosis-associated ILD management were established for six domains (ie, risk factors, screening, diagnosis and severity assessment, treatment initiation and options, disease progression, and treatment escalation) using a modified Delphi process based on a systematic literature analysis.

[Optimal duration of dual antiplatelet therapy after coronary stent placement or acute coronary syndrome. Is customisation possible?].

Ann Cardiol Angeiol (Paris)

November 2019

Service de cardiologie et cardiologie interventionnelle, hôpital André-Mignot, centre hospitalier de Versailles, 177, rue de Versailles, 78150 Le Chesnay, France.

The recommended 6-month dual antiplatelet therapy (DAPT) after coronary angioplasty with implantation of a drug eluting stent is based on solid evidence, but must take into account continuous improvements in stent technology leading to reduced thrombogenicity. In stable patients with a high hemorrhagic risk, it is possible to reduce DAPT duration at 3 months without significant increase in the risks of ischemic events or stent thrombosis. Further reduction toward a 1-month DAPT is likely to involve new strategies of stopping aspirin at 1 month, and continuing long-term monotherapy with inhibitors of P2Y12 receptor.
