Primary omental torsion is an unusual condition, known for its rarity and for the particularity of being intraoperatively diagnosed, in nearly all cases. At the clinical evaluation, this pathology commonly mimics other etiologies of acute abdomen. Hemoperitoneum and necrosis of the omentum are rarely associated with the omental torsion, but when the association is found, then it means that the vascular injuries are irreversible and the required surgical procedure may be far more complex than simple devolvulus.

A Reliable and Rapid Language Tool for the Diagnosis, Classification, and Follow-Up of Primary Progressive Aphasia Variants.

Front Neurol

January 2021

Department of Neurology, National Reference Center for "PPA and rare dementias", Institute for Memory and Alzheimer's Disease, Pitié Salpêtrière Hospital, AP-HP, Paris, France.

Article Synopsis
  • Primary progressive aphasias (PPA) lack consistent language tests for diagnosis and follow-up, prompting the development of a new rapid test, "PARIS," aimed at improving reliability.
  • The "PARIS" test showed 88% inter-rater consistency and effectively distinguished PPA from typical Alzheimer's disease (AD), while also identifying two common PPA variants—semantic and logopenic.
  • With a quick application time of about 10 minutes, the "PARIS" test demonstrated high sensitivity for detecting language decline, making it a promising tool for diagnosis and therapeutic research in neurodegenerative diseases impacting language.

Factors Associated With Severe SARS-CoV-2 Infection.

Pediatrics

March 2021

French Pediatric Infectious Disease Group, Paris, France;

Background: Initial reports on severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infections in children suggested that very young age and comorbidities may increase risk of severe evolution, but these findings remained to be confirmed. We aimed to analyze the clinical spectrum of hospitalized pediatric SARS-CoV-2 infection and predictors of severe disease evolution.

Methods: We conducted a French national prospective surveillance of children hospitalized with SARS-CoV-2 infection.

September 2021

Département d'Endocrinologie, Diabétologie et Nutrition, l'institut du Thorax, Inserm, CNRS, UNIV Nantes, CHU Nantes, Hôpital Guillaume et René Laennec, 44093 Nantes Cedex 01, France.

Article Synopsis
  • The study aimed to investigate the effects of prior metformin use on the health outcomes of type 2 diabetes patients hospitalized for COVID-19.
  • An analysis of 2,449 patients showed that metformin users generally had better survival rates and lower mortality compared to non-users, particularly noted at days 7 and 28 after admission.
  • The conclusion suggests that metformin may reduce the risk of death in diabetes patients with COVID-19, indicating its potential beneficial effects during hospitalization.

[Classification of vascular tumours and vascular malformations. Contribution of the ISSVA 2014/2018 classification].

Ann Pathol

February 2021

Consultation des angiomes, service de neuroradiologie, hôpital Lariboisière, APHP, 2, rue Ambroise-Paré 75475 Paris cedex, France.

The study of vascular anomalies, "angiomas", vascular tumours and vascular malformations is made difficult by the great variety and confusion of the names used in the literature for these diseases, some of which are rare. The great merit of the classification proposed by the International Society for the Study of Vascular Anomalies (ISSVA), adopted in 2014 and modified in 2018, is to propose a unambiguous nomenclature and to try to group these lesions in a logical way, contrasting with the lists of the usual "classifications". This classification is based on the distinction between proliferative lesions (tumours and reactive lesions) and those which are due to a congenital anomaly of vascular morphogenesis (vascular malformations).

Background: Upper-limb robot-mediated therapy is usually carried out in active-assisted mode because it enables performance of many movements. However, assistance may reduce the patient's own efforts which could limit motor recovery.

Objective: The aim of this study was to compare the effects of active-assisted and active-unassisted robotic interactions on motor recovery in subacute stroke patients with moderate hemiparesis.

Background: Placental passage of drugs in twins is poorly understood, and is unknown regarding antiretrovirals (ARVs). In the event of large differences in the exposure of 2 twins to the same maternal therapy, this could have a clinical impact in terms of prevention of perinatal HIV transmission or adverse effects.

Objective: To describe the frequency of differential transplacental passage of antiretrovirals between twins.

Access time to extracorporeal cardiopulmonary resuscitation (ECPR) refractory out of hospital cardiac arrest (OHCA) is a crucial factor. In our region, some patients are not eligible to this treatment due to the impossibility to reach the hospital with reasonable delay (ideally 60 min). In order to increase accessibility for patients far from ECPR centers, we developed a helicopter-borne ECPR-team which is sent out to the patient for ECPR implementation on the scene of the OHCA.

Introduction: Mainstreamed genetic testing (MGT) obviates the need for a cancer genetics consultation, since trained oncologists (O) and gynaecologists (G) provide counseling, prescribe testing and deliver results. We report results from our MGT program and emphasize its utility during the COVID-19 lockdown, when cancer genetics clinics had suspended their activity.

Methods: An MGT pathway for breast and ovarian cancer (BC/OC) patients was established in Jan-2018 between the Assistance Publique - Hôpitaux de Paris.

Health professionals are currently facing the challenge of managing an increasing number of old patients presenting with acute stroke, due to rapid aging of the population. Compared to their younger counterparts, elderly patients differ in many ways in the setting of acute stroke. Apart from a striking high stroke incidence, which increases exponentially as age increases, cardioembolism also becomes, as patients age, the main cause of ischemic stroke.
