Central nervous system vasculitis in VEXAS syndrome: A rare involvemen.

Clin Neurol Neurosurg

July 2024

Service de médecine interne, Hôpital Saint Antoine, Paris, France.

Introduction: VEXAS (Vacuoles, E1 Enzyme, X-linked, autoinflammatory, Somatic) syndrome is a recently described severe adult-onset autoinflammatory disorder mediated by X-linked gene UBA1 somatic mutations, responsible of recurrent fever, skin involvement, chondritis, macrocytic anemia and inflammatory syndrome. Neurological manifestations are rarely described, and predominantly involve peripheral nervous system (PNS) impairment.

Results: We report the first central nervous system (CNS) vasculitis in VEXAS syndrome, characterized by headache, cognitive dysfunction and focal signs (cerebellar ataxia).

Article Synopsis
  • The study investigates the prevalence and incidence of hepatitis E virus (HEV) infection in patients with inflammatory bowel disease (IBD) undergoing immunomodulatory treatment, given that these medications can compromise the immune system.
  • Conducted across 16 French centers, the research involved 488 IBD patients, revealing very few cases of acute HEV infection, with a seroprevalence of 14.2% for chronic infection, particularly among older patients and those with higher seafood intake.
  • The findings suggest that routine HEV screening isn't necessary for IBD patients on immunomodulators, but HEV should be tested if liver abnormalities occur.

Objectives: Both peripherally inserted central catheters (PICCs) and implanted port catheters (PORTs) are commonly used for the delivery of immunochemotherapy. We compared the safety of the two types of devices in a homogeneous and monocentric population of diffuse large B-cell lymphoma (DLBCL) patients who were treated with first-line immunochemotherapy by evaluating the numbers of catheter-related venous thromboses (VTs) and infections that occurred in the six months after implantation according to the type of device.

Methods: Using a propensity score, the adjusted relative risk (ARR) between the type of catheter and the occurrence of catheter-related complications (infection and/or VT) of interest was retrospectively determined.

Postoperative delirium is common after hip fracture surgery in elderly patients and is associated with poor outcome (higher risk of mortality, institutionalization and length of stay). The relationship between modifiable intra-operative risk factors, such as intra-operative hypotension, and postoperative delirium is unclear. The main objective of this study was to evaluate the association between intra-operative hypotension and the development of a postoperative delirium in older patients with hip fracture.

Objectives: There are conflicting recommandations about late medical abortion: the French National College of Gynaecologists and Obstetricians (CNGOF) and the French Superior Health Authority are opposed. The aim was to assess misoprostol's efficiency in medical abortions at 9 to 14 weeks gestational age (GA) METHODS: This is an epidemiologic retrospective study led from March 2017 till January 2019, in Evreux's hospital, France, including all patients undergoing a medical abortion from 9 to 14 weeks GA. We followed the CNGOF's recommandations published in 2016.

Article Synopsis
  • * Out of 449 patients, 12% were diagnosed with NSTEMI, with similar prevalence rates across different time frames of chest pain onset (under 2 hours, 2-4 hours, and over 4 hours).
  • * The findings suggest that a single measurement of HS-cTn, especially alone or with copeptin, is not reliable for ruling out NSTEMI in patients who present very early (within 2 hours)

[A rare cause of eosinophilic asthma: Hyper-IgG4 syndrome (IgG4-related sclerosing disease)].

Rev Mal Respir

November 2015

Service de pneumologie et oncologie thoracique, centre hospitalier Eure-Seine, rue Léon-Schwartzenberg, 27015 Évreux, France.

Introduction: The hyper-IgG4 syndrome is an emerging disease with various thoracic manifestations: eosinophilic asthma, adenomegalies or mediastinal fibrosis, asthma, infiltrative pneumonia, nodules or pseudo-tumors with bronchial and pleural localizations.

Case Report: We report the case of a 38-year-old woman who was admitted for acute exacerbation of eosinophilic asthma with high total IgE levels. A medical history of idiopathic acute pancreatitis, associated with sclerosing cholangitis and renal failure, suggested a diagnosis of hyper-IgG4 syndrome in this woman with late-onset severe and non-atopic asthma.
