Article Synopsis
  • The study aimed to identify predictors of multiple recurrent Clostridioides difficile infections (mrCDI) in adults at the time they first present with an initial CDI (iCDI).
  • Of 18,829 patients with iCDI, 4.7% developed mrCDI, with older age, recent hospitalization, chronic hemodialysis, and nitrofurantoin use being significant risk factors.
  • Findings suggest that patients at higher risk for mrCDI may benefit from early interventions to prevent further recurrences, potentially improving clinical management strategies.

Parkinson's Disease in Sub-Saharan Africa: Pesticides as a Double-Edged Sword.

J Parkinsons Dis

April 2024

Department of Neurology, Radboud University Medical Centre, Donders Institute for Brain, Cognition and Behaviour, Centre of Expertise for Parkinson and Movement Disorders, Nijmegen, The Netherlands.

Long-term exposure to pesticides used in agriculture is increasingly being identified as a risk factor for developing Parkinson's disease. How chronic pesticide exposure might contribute to the growth of Parkinson's disease in the mainly agricultural communities of Sub-Saharan Africa has thus far received limited attention. There are specific concerns in this area of the world: aging of the population, in combination with chronic exposure to widely used pesticides, including those that have been restricted elsewhere in the world because of neurotoxicity and other health risks.

Article Synopsis
  • The study investigates the link between sphingolipid metabolism and childhood asthma, focusing on how different classes of sphingolipids interact with asthma risk factors.
  • Researchers analyzed blood samples from nearly 1,000 children to explore associations between sphingolipids, asthma, and specific risk factors like genetic markers, vitamin D levels, and gut health.
  • The findings indicate that while overall sphingolipid levels correlate with asthma, specific subclasses (like ceramides) have distinct roles, with some linked to increased asthma risk factors rather than the disease itself.

Respiratory infections are a leading cause of morbidity and mortality in early life, and recurrent infections increase the risk of developing chronic diseases. The maternal environment during pregnancy can impact offspring health, but the factors leading to increased infection proneness have not been well characterized during this period. Steroids have been implicated in respiratory health outcomes and may similarly influence infection susceptibility.

Recurrent respiratory infections are a leading cause of morbidity and mortality in early life, but there is no broadly accepted means to identify infection-prone children during this highly vulnerable period. In this study, we investigated associations between steroid metabolites and incident respiratory infections in two pre-birth cohorts to identify novel metabolomic signatures of early infection proneness. Children from the Vitamin D Antenatal Asthma Reduction Trial and the Copenhagen Prospective Studies on Asthma in Childhood were included, and profiling was performed on plasma samples collected at ages 1 and 6 years.

Departments of Microbiology, Immunology and University of Rochester Medical Center, Rochester, New York; Departments of Surgery, University of Rochester Medical Center, Rochester, New York; Departments of Radiation Oncology, University of Rochester Medical Center, Rochester, New York. Electronic address:

Purpose: Many solid tumors present with perineural invasion (PNI), and innervation correlates with worsened prognosis. The effects that commonly administered therapies such as radiation therapy (RT) have on PNI status remain unknown. We investigated the contribution of RT on the nervous system and elucidated the implications that increased nerve signaling can have on tumor burden using our previously developed orthotopic murine model of rectal cancer (RC) and our targeted and clinically relevant short-course RT (SCRT) regimen.

Background: Although resting blood pressures following aortic arch repair or the extended end-to-end anastomosis (EEA) repair for coarctation can be physiologic, factors associated with an abnormal blood pressure response after exercise are unknown. We measured blood pressure gradients following exercise in children who had undergone previous repair in accordance with a surgical selection algorithm and sought to identify factors associated with an abnormal blood pressure response.

Methods: In accordance with our practice's surgical algorithm for repair of coarctation, infants were stratified to aortic arch repair when the distal transverse arch-to-left carotid artery ratio (DTA:LCA) ≤ 1.

This case reports an ethical dilemma in which a mother who had recently delivered a baby through cesarean section after sustaining life-threatening injuries in a car accident did not have documented wishes whether she wanted to breastfeed. Her medical condition rendered her temporarily mentally incapacitated and critically ill, and the lactation medicine team was consulted about whether lactation choice should be preserved by pumping. Complicating considerations in this case were (1) lack of family or designated decision-makers available at the time of injury and emergent delivery, (2) lack of prenatal care, and (3) complex social situation, including prior history of illicit substance use, and state removal of other children into foster care.
