Background: Patients with human epidermal growth factor receptor 2 (HER2)-positive early breast cancer with residual invasive disease after neoadjuvant systemic therapy have a high risk of recurrence and death. The primary analysis of KATHERINE, a phase 3, open-label trial, showed that the risk of invasive breast cancer or death was 50% lower with adjuvant trastuzumab emtansine (T-DM1) than with trastuzumab alone.

Methods: We randomly assigned patients with HER2-positive early breast cancer with residual invasive disease in the breast or axilla after neoadjuvant systemic treatment with taxane-based chemotherapy and trastuzumab to receive T-DM1 or trastuzumab for 14 cycles.

Precision models in hepatocellular carcinoma.

Nat Rev Gastroenterol Hepatol

December 2024

Department of Oncological Sciences, Icahn School of Medicine at Mount Sinai, New York, NY, USA.

Hepatocellular carcinoma (HCC) represents a global health challenge, and ranks among one of the most prevalent and deadliest cancers worldwide. Therapeutic advances have expanded the treatment armamentarium for patients with advanced HCC, but obstacles remain. Precision oncology, which aims to match specific therapies to patients who have tumours with particular features, holds great promise.

Article Synopsis
  • The study compares the effectiveness and toxicity of two treatments, rituximab (RTX) and cyclophosphamide (CYC), for pediatric patients with granulomatosis with polyangiitis (GPA) and microscopic polyangiitis (MPA).
  • It analyzes data from 104 patients and finds no significant difference in remission rates or severe adverse events between the two treatment groups.
  • Limitations of the study include the lack of standardized treatment protocols and the retrospective nature of the analysis.

Background: Previous investigations into clinical signs and symptoms associated with influenza types and subtypes have not definitively established differences in the clinical presentation or severity of influenza disease.

Methods: The study population included children 0 through 17 years old enrolled at 8 New Vaccine Surveillance Network sites between 2015 and 2020 who tested positive for influenza virus by molecular testing. Demographic and clinical data were collected for study participants via parent/guardian interview and medical chart review.

Hepatic Nuclear Receptors in Cholestasis-to-Cholangiocarcinoma Pathology.

Am J Pathol

September 2024

Department of Pathology, University of Pittsburgh School of Medicine, Pittsburgh, Pennsylvania; Pittsburgh Liver Research Center, University of Pittsburgh Medical Center and University of Pittsburgh School of Medicine, Pittsburgh, Pennsylvania. Electronic address:

Cholestasis, characterized by impaired bile flow, is associated with an increased risk of cholangiocarcinoma (CCA), a malignancy originating from the biliary epithelium and hepatocytes. Hepatic nuclear receptors (NRs) are pivotal in regulating bile acid and metabolic homeostasis, and their dysregulation is implicated in cholestatic liver diseases and the progression of liver cancer. This review elucidates the role of various hepatic NRs in the pathogenesis of cholestasis-to-CCA progression.

Accurate screening of COVID-19 infection status for symptomatic patients is a critical public health task. Although molecular and antigen tests now exist for COVID-19, in resource-limited settings, screening tests are often not available. Furthermore, during the early stages of the pandemic tests were not available in any capacity.

Article Synopsis
  • The study aimed to compare the risk of arthritis flares when starting urate-lowering therapy (ULT) with allopurinol versus febuxostat in gout patients, focusing on the first 24 weeks of treatment.
  • It analyzed data from a trial involving 940 male participants, examining flare occurrences and various predictors such as treatment type and serum urate levels.
  • The findings indicated that the risk of gout flares was similar for both medications when using effective treatment strategies, with notable predictors of flare being younger age, higher initial urate levels, and the absence of tophi.

Article Synopsis
  • Gain-of-function mutations in the CTNNB1 gene, responsible for β-catenin production, are present in 25-30% of hepatocellular carcinomas (HCC), significantly influencing treatment resistance and drug sensitivity.
  • A study involving 134 HCC patients aimed to create a non-invasive imaging model to assess β-catenin activation, as liver biopsies pose risks.
  • The developed model demonstrated improved predictive accuracy for β-catenin activation when combining qualitative LI-RADS descriptors and quantitative radiomics features, suggesting strong potential for precision medicine strategies in treating HCC.

Methionine as a regulator of bone remodeling with fasting.

JCI Insight

May 2024

Neuroendocrinology Unit, Division of Endocrinology and Metabolism, Department of Medicine, University of Pittsburgh School of Medicine.

Caloric restriction improves metabolic health but is often complicated by bone loss. We studied bone parameters in humans during a 10-day fast and identified candidate metabolic regulators of bone turnover. Pro-collagen 1 intact N-terminal pro-peptide (P1NP), a bone formation marker, decreased within 3 days of fasting.

Semaglutide for management of obesity in adolescents: efficacy, safety, and considerations for clinical practice.

Curr Opin Pediatr

August 2024

Division of Pediatric Endocrinology, Diabetes, and Metabolism and Center for Pediatric Research in Obesity and Metabolism, UPMC Children's Hospital of Pittsburgh and University of Pittsburgh School of Medicine, Pittsburgh, Pennsylvania, USA.

Purpose Of Review: The purpose of this review is to describe the existing limited data related to the use of semaglutide in adolescents with obesity, supplementing with findings from adult studies of semaglutide use.

Recent Findings: Semaglutide, as a once weekly subcutaneous injection for weight management, effectively reduces body mass index (BMI) while improving hyperglycemia, elevated alanine aminotransferase levels, hyperlipidemia, and quality of life in youth with obesity. As of this review, only one large randomized clinical trial of semaglutide in youth has been completed, with a follow-up duration of 68 weeks.

Objective: The objective of this study is to determine the associations of protein-specific anti-malondialdehyde-acetaldehyde (MAA) antibodies with prevalent and incident rheumatoid arthritis-interstitial lung disease (RA-ILD).

Methods: Within a multicenter, prospective cohort of US veterans with RA, RA-ILD was validated by medical record review of clinical diagnoses, chest imaging, and pathology. Serum antibodies to MAA-albumin, MAA-collagen, MAA-fibrinogen, and MAA-vimentin (IgA, IgM, and IgG) were measured by a standardized enzyme-linked immunosorbent assay.

In the United States, annual influenza vaccination is recommended for all persons aged ≥6 months. Using data from four vaccine effectiveness (VE) networks during the 2023-24 influenza season, interim influenza VE was estimated among patients aged ≥6 months with acute respiratory illness-associated medical encounters using a test-negative case-control study design. Among children and adolescents aged 6 months-17 years, VE against influenza-associated outpatient visits ranged from 59% to 67% and against influenza-associated hospitalization ranged from 52% to 61%.

Family and friend caregivers play critical roles in ensuring that persons with serious illness receive high-quality care, and their responsibilities often increase as patients transition from receiving solely curative-focused care to primarily palliative-focused care. Integrating family caregivers into the health care team and supporting them in their role has significant benefits for family caregivers, patients, health care systems, communities, and society. Palliative care clinicians across all disciplines are uniquely suited to provide necessary training and support to family caregivers as they navigate the demands of their role.
