The Cost of Inflammatory Bowel Disease Care - How to Make it Sustainable.

Clin Gastroenterol Hepatol

August 2024

Division of Gastroenterology and Hepatology, Cumming School of Medicine, University of Calgary, Calgary, Alberta, Canada.

Article Synopsis
  • - The rising rates of inflammatory bowel diseases (IBDs) like Crohn's and ulcerative colitis are leading to significant healthcare costs ranging from $9,000 to $12,000 per year in wealthy areas, but this doesn't fully capture factors like disease severity and regional healthcare differences.
  • - Indirect costs from lost productivity due to IBD are substantial yet often underestimated, and challenges such as access disparities and lack of standardized care guidelines complicate efforts to manage costs effectively.
  • - To make IBD care sustainable, strategies like using biologic therapies early, increasing access to biosimilars, and implementing multidisciplinary care teams with a focus on technology and patient feedback are essential for improving outcomes and reducing financial burdens.

Article Synopsis
  • Managing diagnostic uncertainty is a significant challenge in primary care, especially with unclear symptoms and no definitive tests, which can harm patient trust and lead to diagnostic errors.
  • The study aimed to investigate how UK GPs handle and communicate this uncertainty during consultations, using video and audio recordings to analyze their actions.
  • Key findings reveal that GPs employed different strategies for managing uncertainty, including monitoring symptoms and prescribing treatments, but the specificity and negotiation around management plans varied significantly among them.

Self-Correcting Method for Highly Effective Office-Based Helicobacter pylori Therapy Using Cumulative Test of Cure Data.

Helicobacter

August 2024

Department of Medicine, Michael E. DeBakey Veterans Affairs Medical Center, Baylor College of Medicine, Houston, Texas, USA.

Background And Aim: Helicobacter pylori infections have become resistant to many previously highly effective antimicrobial regimens resulting in clarithromycin, metronidazole, or fluoroquinolone-containing therapies becoming unsuccessful. Pretreatment susceptibility testing is only widely available in the United States but is still rarely done. Here, we propose a framework to monitor H.

Taking leftover prescribed antibiotics without consulting a healthcare professional is problematic for the efficacy, safety, and antibiotic stewardship. We conducted a cross-sectional survey of adult patients in English and Spanish between January 2020 and June 2021 in six safety-net primary care clinics and two private emergency departments. We assessed the reasons for stopping prescribed antibiotics early and what was done with the leftover antibiotics.

Previously, we showed that water extract (soymilk, except pH was increased to 8 from 6.5) of whole soybean could be used directly as a raw material for producing edible soy films by deposition of the film-forming solution (soy extract with enhancers). However, the strength of such soy films needed improvement because they were weak.

Background: A HIV vaccine is not available yet, but perceptions of HIV vaccines will be important to explore before their roll-out for effective vaccine promotion. This article presents the findings of a rapid scoping review of the literature to identify individual, social, and vaccine-related factors associated with the acceptability of a future HIV vaccine.

Methods: We searched 5 databases (Medline OVID, Embase, PsycINFO, Web of Science, and Cochrane) using relevant keywords and Medical Subject Headings.

Background: Emergency departments (EDs) are complex and fast-paced clinical settings where a diagnosis is made in a time-, information-, and resource-constrained context. Thus, it is predisposed to suboptimal diagnostic outcomes, leading to errors and subsequent patient harm. Arriving at a timely and accurate diagnosis is an activity that occurs after an effective collaboration between the patient or caregiver and the clinical team within the ED.

Article Synopsis
  • The COVID-19 pandemic significantly impacted sleep testing services, with notable declines in utilization for both Home Sleep Apnea Testing (HSAT) and Polysomnography (PSG) across various stages of the pandemic.
  • A study of 261,371 veterans revealed PSG usage fell drastically during different pandemic phases, while HSAT saw an initial drop followed by a recovery and increase post-vaccination.
  • By analyzing data from the Veterans Health Administration between 2019 and 2023, the study concluded that HSAT became more prominent in sleep testing services, particularly after vaccination efforts began.

DeBakey Veterans Affairs Medical Center, Houston, Texas. Electronic address:

Background & Aims: In patients with cirrhosis, continued heavy alcohol consumption and obesity may increase risk of hepatocellular carcinoma (HCC). We examined whether germline susceptibility to hepatic steatosis not only independently predisposes to HCC but may also act synergistically with other risk factors.

Methods: We analyzed data from 1911 patients in 2 multicenter prospective cohort studies in the United States.

Antibiotic Stewardship Interventions for Urinary Tract Infections in Outpatient Settings: A Narrative Review.

Infect Dis Clin North Am

June 2024

Center for Innovations in Quality, Effectiveness and Safety (IQuESt), Michael E. DeBakey Veterans Affairs Medical Center, Houston, TX, USA; Section of Health Services Research, Department of Medicine, Baylor College of Medicine, IQuESt (152), 2002 Holcombe Boulevard, Houston, TX 77030, USA.

Inappropriate antibiotic choice or duration of therapy for urinary tract infections (UTIs) in outpatients is common and is a major contributor to antibiotic overuse. Most studies on outpatient antibiotic stewardship for UTIs follow a pre-design or post-design with a multifaceted intervention; these trials generally have found improvement in appropriateness of antibiotic use for UTI. Audit and feedback was one of the most commonly employed strategies across these trials but may not be sustainable.

This commentary discusses how clinicians and various stakeholders can utilize the recently published American Association for the Study of Liver Diseases nonalcoholic fatty liver disease (AASLD NAFLD) Practice Guidance in light of the change in the nomenclature to steatotic liver disease and its subcategories. The new terminologies explained in this commentary make it easier for the readers to interchangeably use metabolic dysfunction-associated steatotic liver disease (MASLD) in place of NAFLD and metabolic-dysfunction associated steatohepatitis (MASH) instead of nonalcoholic steatohepatitis (NASH), respectively, as they read the NAFLD Practice Guidance. The guidance document is relevant and can be utilized for the diagnosis, risk stratification, and management of patients with MASLD.
