Mesenchymal stem cells and local tacrolimus delivery synergistically enhance neurite extension.

Biotechnol Bioeng

November 2021

Division of Hand and Microvascular Surgery, Department of Orthopedic Surgery, Mayo Clinic, Rochester, Minnesota, USA.

Background: The aim of this study was to investigate the combined effect of mesenchymal stem cells (MSC) and local delivery of tacrolimus (FK506) on nerve regeneration when applied to nerve autografts and decellularized allografts.

Methods: A three-dimensional in vitro compartmented cell culture system consisting of a neonatal dorsal root ganglion adjacent to a nerve graft was used to evaluate the regenerating neurites into the peripheral nerve scaffold. Nerve autografts and allografts were treated with (i) undifferentiated MSCs, (ii) FK506 (100 ng/mL) or (iii) both (N = 9/group).

Article Synopsis
  • The study investigates the effects of pathogenic variants in the SETD1B gene, linked to a neurodevelopmental disorder characterized by intellectual disability, language delays, and seizures in affected individuals.
  • Researchers analyzed a larger group of 36 patients to better understand the molecular and phenotypic variations associated with SETD1B mutations, revealing a common pattern of developmental delays and behavioral issues.
  • Findings indicate that males are more severely affected, leading to speculation about sex-linked factors influencing the disorder’s clinical presentation and impact on neurodevelopment even before seizure onset.

Division of Pediatric Critical Care Medicine, Department of Pediatrics, Indiana University School of Medicine and Riley Hospital for Children at Indiana University Health, 705 Riley Hospital Drive, Phase 2, Room 4900, Indianapolis, IN, 46202-5225, USA.

Article Synopsis
  • A study on pediatric patients receiving extracorporeal membrane oxygenation (ECMO) for cardiac reasons found that 18% experienced seizures, with many being subclinical and some progressing to status epilepticus.
  • Risk factors for seizures included prior cardiac arrest, the need for extracorporeal cardiopulmonary resuscitation, and specific cannulation methods.
  • Children with seizures faced longer ICU stays, lower pediatric cerebral performance scores, and trends suggesting worse survival rates compared to those without seizures.

Objectives: Abusive head trauma (AHT) is a very common and serious form of physical abuse, and a major cause of mortality and morbidity for young children. Early Recognition and supportive care of children with AHT is a common challenge in community emergency department (CEDs). We hypothesized that standardized, in situ simulation can be used to measure and compare the quality of resuscitative measures provided to children with AHT in a diverse set of CEDs.

Background: Invasive mechanical ventilation is a lifesaving intervention that is associated with short- and long-term morbidities. Extubation readiness protocols aim to decrease extubation failure rates and simultaneously shorten the duration of invasive ventilation. This study sought to analyze extubation readiness practices at one institution and identify barriers to extubation in pediatric patients who have passed an extubation readiness test (ERT).

Background: We sought to evaluate the institutional use of inhaled nitric oxide (INO) and to create a pathway to reduce waste using the Institute for Healthcare Improvement's model for improvement. Our aim was to reduce the use of INO by 20% within 8 months.

Methods: This was a prospective, respiratory therapist-driven, quality improvement project.

Guillain-Barré syndrome (GBS) is characterized by a monophasic, ascending, and symmetrical paralysis with areflexia that progresses over days to weeks. It is typically a postinfectious autoimmune process that leads to destruction of myelin. Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), originated in Wuhan, China, in late 2019 and rapidly spread around the world, causing a pandemic of novel coronavirus disease 2019 (COVID-19).

Majority of females with a life-long experience of CAH and parents do not consider females with CAH to be intersex.

J Pediatr Urol

April 2021

Division of Adolescent Medicine, Department of Pediatrics, Indiana University School of Medicine and Department of Sociology, Indiana University Purdue University Indianapolis, Indianapolis, IN, USA.

Introduction: To assess opinions of females with CAH, and parents of females with CAH, about designating this population "intersex," particularly in legislation about genital surgery during childhood.

Methods: We conducted a mixed-methods (quantitative and qualitative) anonymous cross-sectional online survey of females with CAH (46XX, 16+years old) and independently recruited parents of girls with CAH (2019-2020) diagnosed in first year of life from the United States. A multidisciplinary CAH team drafted the survey in collaboration with women with CAH and parents.

Article Synopsis
  • A study was conducted across 28 healthcare institutions in 6 countries to understand how pediatric anaphylaxis is managed, focusing on the administration of epinephrine and medication errors.
  • Results showed significant variability, with only 41% of institutions having guidelines, and 54% lacking epinephrine autoinjectors, which are crucial for treatment.
  • The study found that 68% of simulations had at least one medication error, but healthcare providers with more experience in using epinephrine made fewer mistakes.
