Elective uterine preservation: a novel concept to tackle reproductive ageing. Fertility preservation beyond ovaries.

Reprod Biomed Online

With a growing number of women in developed countries having children later in life, it is time to rethink women's reproductive health. This trend of 'delayed parenthood' reflects a complex interplay of labour market rules, personal choices, rarefying social connections and the renowned possibilities of assisted reproductive technology (ART). While ART offers support, its success wanes with age, prompting a rise in additional strategies such as elective ovarian cryopreservation.

Article Synopsis
  • - Research in medicine is crucial for improving patient care, especially in the fast-evolving field of human reproduction, where treatment options are continually being developed.
  • - Randomized controlled trials are essential for establishing evidence-based treatment decisions, as they help evaluate the effectiveness of new medical approaches, provided they are well-designed and executed.
  • - While high-impact journals often indicate rigorous peer review and reliability of published data, readers should remain critical, as even top-tier studies can contain flaws—underscoring the importance of examining details closely.

A critical appraisal of safety data on dydrogesterone for the support of early pregnancy: a scoping review and meta-analysis.

Reprod Biomed Online

August 2022

Children's National Hospital, 111 Michigan Ave, NW Washington DC 20010, USA and University Children's Hospital Basel, University of Basel, Spitalstrasse 33, Basel 4056, Switzerland.

No data support the suggestion that first-trimester dydrogesterone use increases the risk of fetal abnormalities; however, two low-quality retrospective studies (one retracted by the journal) have suggested such a link. A scoping review and meta-analysis were carried out to address this discrepancy. The literature was reviewed but it was not possible to identify any evidence of a plausible mechanism for potential causality between dydrogesterone and fetal abnormalities.

Oocyte vitrification for elective fertility preservation: a SWOT analysis.

Reprod Biomed Online

June 2022

Oocyte vitrification, also known as egg freezing, is increasingly being used by women as a precautionary measure against the anticipated decline in fertility. In countries where this procedure is allowed, elective oocyte vitrification has become an integral part of the treatment portfolio of fertility clinics. The widespread tendency towards the postponement of motherhood and the advances in laboratory technologies are encouraging women to consider oocyte vitrification and, by doing so, increase their reproductive autonomy.

Personalized ovarian stimulation for assisted reproductive technology: study design considerations to move from hype to added value for patients.

Although most medical treatments are designed for the average patient with a one-size-fits-all-approach, they may not benefit all. Better understanding of the function of genes, proteins, and metabolite, and of personal and environmental factors has led to a call for personalized medicine. Personalized reproductive medicine is still in its infancy, without clear guidance on treatment aspects that could be personalized and on trial design to evaluate personalized treatment effect and benefit-harm balance.

Objective: To study whether preimplantation genetic screening with comprehensive chromosome screening (PGS-CCS) improves clinical implantation rates (IR) and sustained IR (beyond 20 weeks) compared with routine care for embryo selection in IVF cycles.

Design: Meta-analysis of randomized controlled trials (RCTs) and observational studies (OSs).

Setting: University-affiliated teaching hospital.

Deep endometriosis is a demanding condition that is associated with infertility. However, evidence supporting a direct link between deep endometriosis and infertility is weak. In fact, infertility in affected patients is more likely to be explained by the strong association between deep endometriosis and adhesions, superficial endometriotic implants, ovarian endometriomas, and adenomyosis.

Chronic kidney disease (CKD)-mineral and bone disorder (MBD) is characterized by increased circulating levels of parathormone (PTH) and fibroblast growth factor 23 (FGF23), bone disease, and vascular calcification, and is associated with adverse outcomes. We studied the prevalence of mineral metabolism disorders, and the potential relationship between decreased estimated glomerular filtration rate (eGFR) and CKD-MBD in coronary artery disease patients in a cross-sectional study of 704 outpatients 7.5 ± 3.

Embryonic aneuploidy is highly prevalent in IVF cycles and contributes to decreased implantation rates, IVF cycle failure and early pregnancy loss. Preimplantation genetic screening (PGS) selects the most competent (euploid) embryos for transfer, and has been proposed to improve IVF outcomes. Use of PGS with fluorescence-in-situ hybridization technology after day 3 embryo biopsy (PGS-v1) significantly lowers live birth rates and is not recommended for use.

Ovarian hyperstimulation syndrome (OHSS) is the most serious iatrogenic complication associated with ovarian stimulation. Despite the numerous breakthroughs that have been made in other areas of assisted reproduction technology, it is only in recent years that the understanding of OHSS has advanced sufficiently to develop treatment options aimed at reducing the incidence and effects of OHSS. However, suitable predictors and tests with which to identify susceptible patients remain unreliable, although anti-Müllerian hormone is currently a good risk-factor candidate.

Cephalic and extracephalic allodynia are recognized as a common sign of sensory sensitization during migraine episodes. However, the occurrence of body pain in migraine has not been thoroughly explored. Here we report three patients presenting with spontaneous body pain in association with migraine attacks.

Multiple pregnancies are a serious adverse event of IVF treatment. Several strategies have been developed to improve embryo selection and embryo developmental potential to increase pregnancy rates without increasing the risk of multiple pregnancies. The only way to effectively solve this problem is to reduce the number of embryos transferred to one.

Background: The clinical features of cluster headache (CH) disclose some vascular changes in the symptomatic region, but few instrumental studies have assessed orbital hemodynamics in patients with this disorder.

Methods: Orbital blood flow reactivity elicited by Valsalva maneuver (VM) was studied with ophthalmic artery eco-Doppler in 16 patients (14 men and 2 women; mean age: 41.2) suffering from episodic CH and in 18 healthy controls.
