Soft-tissue sarcoma (STS) is not a single entity but, rather, a family of diseases with differing biologic behaviors and anatomic site- and histotype-specific responses to treatment. Whereas surgery remains the mainstay of treatment of primary, localized disease, evolving evidence is establishing the role of multimodality treatment of these tumors. This article summarizes prospective evidence to date informing our treatment of STS.

Mesentery in Transanal TME.

Clin Colon Rectal Surg

July 2022

Division of Colorectal Surgery, Colorectal and Minimally Invasive Surgery, Toronto General Hospital and Princess Margaret Cancer Centre, Toronto, Canada.

Oncological adequacy in rectal cancer surgery mandates not only a clear distal and circumferential resection margin but also resection of the entire ontogenetic mesorectal package. Incomplete removal of the mesentery is one of the commonest causes of local recurrences. The completeness of the resection is not only determined by tumor and patient related factors but also by the patient-tailored treatment selected by the multidisciplinary team.

Pretreatment classification tools are used in prostate cancer to inform patient management. The effect of cribriform pattern 4 (CC) and intraductal carcinoma (IDC) on such nomograms is still underexplored. We analyzed the Cancer of Prostate Risk Assessment (CAPRA) and National Comprehensive Cancer Network (NCCN) risk scores in cases with and without CC/IDC to assess impact on biochemical recurrence (BCR) and metastases/death of prostate cancer (event free survival-EFS) after prostatectomy.

Testing terror management theory in advanced cancer.

Death Stud

December 2021

Department of Supportive Care, Princess Margaret Cancer Centre, University Health Network, Toronto, Ontario, Canada.

Death anxiety is common in advanced cancer due to heightened mortality salience. We tested terror management theory (TMT) in this population ( = 305) by evaluating the buffering effect of the distal defenses (attachment security, meaning, self-esteem) on the impact of physical impairment (a proxy for mortality salience) on death anxiety. Patients with greater numbers of strong distal defenses reported lower death anxiety than those with no strong defenses or one strong defense.

Clinical guidelines for guiding clinical decision-making must be trustworthy, especially when there is controversy or a shifting paradigm. In urology, the management of small renal masses is one area in which there is currently a paradigm shift towards active surveillance. A number of international guidelines address this paradigm shift to some extent, with varying degrees of rigor.
