Time and event-specific deep learning for personalized risk assessment after cardiac perfusion imaging.

NPJ Digit Med

May 2023

Departments of Medicine (Division of Artificial Intelligence in Medicine), Imaging, and Biomedical Sciences, Cedars-Sinai Medical Center, Los Angeles, CA, USA.

Article Synopsis
  • A new explainable deep learning model has been developed to predict specific cardiovascular risks (like death, acute coronary syndrome, and need for revascularization) based on myocardial perfusion imaging (MPI) combined with clinical data.
  • The model was tested with a large group of patients and showed better performance in predicting short-term risks in the first six months post-scan, outperforming traditional methods.
  • It provides individualized risk assessments and visual explanations for patients, potentially helping to focus on modifiable risk factors and improve shared decision-making in healthcare.

J Am Coll Cardiol

November 2022

The Zena and Michael A Wiener Cardiovascular Institute, Icahn School of Medicine at Mount Sinai, New York, New York, USA.

Background: In the multicenter, randomized, sham-controlled FAVOR (Comparison of Quantitative Flow Ratio Guided and Angiography Guided Percutaneous Intervention in Patients with Coronary Artery Disease) III China trial, quantitative flow ratio (QFR)-based lesion selection improved 1-year clinical outcomes compared with conventional angiographic guidance for percutaneous coronary intervention (PCI).

Objectives: The purpose of this study was to determine whether the benefits of QFR guidance persist at 2 years, particularly for patients in whom QFR changed the revascularization strategy.

Methods: Eligible patients were randomized to a QFR-guided strategy (PCI performed only if QFR ≤0.

Importance: Although durable left ventricular assist device (LVAD) therapy has emerged as an important treatment option for patients with advanced heart failure refractory to pharmacological support, outcomes, including survival, beyond 2 years remain poorly characterized.

Objective: To report the composite end point of survival to transplant, recovery, or LVAD support free of debilitating stroke (Modified Rankin Scale score >3) or reoperation to replace the pump 5 years after the implant in participants who received the fully magnetically levitated centrifugal-flow HeartMate 3 or axial-flow HeartMate II LVAD in the MOMENTUM 3 randomized trial and were still receiving LVAD therapy at the 2-year follow-up.

Design, Setting, And Participants: This observational study was a 5-year follow-up of the MOMENTUM 3 trial, conducted in 69 US centers, that demonstrated superiority of the centrifugal-flow LVAD to the axial-flow pump with respect to survival to transplant, recovery, or LVAD support free of debilitating stroke or reoperation to replace the pump at 2 years.

Article Synopsis
  • Valvular heart disease, specifically aortic stenosis (AS), aortic regurgitation (AR), and mitral regurgitation (MR), is a significant but often underdiagnosed issue that deep learning analysis of electrocardiograms (ECGs) can potentially address.
  • The study involved 77,163 patients and developed deep learning algorithms to accurately identify moderate to severe cases of AS, AR, and MR using ECG data, achieving good performance metrics, especially for AS (AU-ROC: 0.88).
  • Results suggest that deep learning could be integrated into screening programs for valvular heart disease, showing high sensitivity and specificity, particularly at low disease prevalence rates.

Temporal Trends in Cervical Cancer Screening Practices and Associated Downstream Abnormalities and Procedures Among Women With Insurance in the United States.

Obstet Gynecol

July 2022

Columbia University College of Physicians and Surgeons, the Joseph L. Mailman School of Public Health, Columbia University, the Herbert Irving Comprehensive Cancer Center, and NewYork-Presbyterian Hospital, New York, New York.

Objective: To examine temporal trends in cervical cancer screening practices and associated downstream abnormalities and procedures.

Methods: Women aged 18-64 years with commercial insurance or Medicaid insurance from 2008 to 2019 were identified using the IBM MarketScan databases. The annual rates of screening overall and by type of test (cytology, co-testing, or primary human papillomavirus testing) were examined.

Surveillance pouchoscopy is recommended for patients with restorative proctocolectomy with ileal pouch-anal anastomosis in ulcerative colitis or familial adenomatous polyposis, with the surveillance interval depending on the risk of neoplasia. Neoplasia in patients with ileal pouches mainly have a glandular source and less often are of squamous cell origin. Various grades of neoplasia can occur in the prepouch ileum, pouch body, rectal cuff, anal transition zone, anus, or perianal skin.

Purpose: Bone marrow biopsies (BMB) are performed before/after therapy to confirm complete response (CR) in patients with lymphoma on clinical trials. We sought to establish whether BMB add value in assessing response or predict progression-free survival (PFS) or overall survival (OS) outcomes in follicular lymphoma (FL) subjects in a large, multicenter, multitrial cohort.

Methods: Data were pooled from seven trials of 580 subjects with previously untreated FL through Alliance for Clinical Trials in Oncology (Alliance) and SWOG Cancer Research Network (SWOG) completing enrollment from 2008 to 2016.

Worldwide Disparities in Recovery of Cardiac Testing 1 Year Into COVID-19.

J Am Coll Cardiol

May 2022

Division of Human Health, Department of Nuclear Sciences and Applications, International Atomic Energy Agency, Vienna, Austria.

Article Synopsis
  • The study investigates how the COVID-19 pandemic affected cardiac diagnostic services globally, examining changes in testing practices and provider stress levels.
  • Researchers conducted a survey from 669 centers across 107 countries and found a 64% reduction in cardiac procedure volumes during the pandemic, with recovery varying significantly between high-income and lower-income countries.
  • Results indicated a shift from traditional stress testing to advanced imaging techniques, alongside widespread psychological stress among healthcare providers, which hindered the recovery of cardiac testing services, especially in lower-income regions.

In this study of 45 patients with COVID-19 undergoing tracheostomy, nasopharyngeal and tracheal cycle threshold (Ct) values were analyzed. Ct values rose to 37.9 by the time of tracheostomy and remained >35 postoperatively, demonstrating that persistent test positivity may not be associated with persistent transmissible virus in this population.

Mobile stroke units (MSUs) are specialized ambulances equipped with the personnel, equipment, and imaging capability to diagnose and treat acute stroke in the prehospital setting. Over the past decade, MSUs have proliferated throughout the world, particularly in European and US cities, culminating in the formation of an international consortium. Randomized trials have demonstrated that MSUs increase stroke thrombolysis rates and reduce onset-to-treatment times but until recently it was uncertain if these advantages would translate into better patient outcomes.

Efforts to end the HIV and hepatitis C virus (HCV) epidemics begin with ascertainment of a person's infection status through screening. Despite its importance as a site of testing, missed opportunities for screening in the Emergency Department (ED) are common. We describe the impact of implementing an individualized provider feedback intervention on HIV and HCV testing in a quaternary ED.

Children with congenital and acquired heart disease may be exposed to relatively high lifetime cumulative doses of ionizing radiation from necessary medical invasive and non-invasive imaging procedures. Although these imaging procedures are all essential to the care of these complex paediatric population and have contributed to meaningfully improved outcomes in these patients, exposure to ionizing radiation is associated with potential risks, including an increased lifetime attributable risk of cancer. The goal of this manuscript is to provide a comprehensive review of radiation dose management and cardiac computed tomography performance in the paediatric population with congenital and acquired heart disease, to encourage informed imaging to achieve indication-appropriate study quality at the lowest achievable dose.
