Article Synopsis
  • Acute supraventricular arrhythmias can worsen hemodynamic stability in patients with septic shock, and a study compared the effects of intravenous propafenone and amiodarone on arrhythmias in this population.
  • In a trial with 209 patients, those receiving propafenone had a quicker return to sinus rhythm (3.7 hours) compared to those on amiodarone (7.3 hours), although the overall 24-hour sinus rhythm rates were similar.
  • Propafenone led to fewer cases of arrhythmia recurrence than amiodarone, especially in patients without a dilated left atrium, indicating it may be a more effective option for short-term management.

Article Synopsis
  • Amiodarone, but not propafenone, causes significant mitochondrial dysfunction in human cardiomyocytes, leading to reduced ATP production and altered cellular metabolism.
  • * Prolonged exposure to amiodarone downregulates fatty acid oxidation and increases anaerobic glycolysis, while propafenone shows no adverse effects on cellular bioenergetics.
  • * The study indicates that the detrimental impacts of amiodarone on mitochondria are consistent across both human and mouse cardiomyocytes, highlighting potential risks in using this drug at therapeutic doses.

Background: Several studies explored the interdependence between Paco2 and bicarbonate during respiratory acid-base derangements. The authors aimed to reframe the bicarbonate adaptation to respiratory disorders according to the physical-chemical approach, hypothesizing that (1) bicarbonate concentration during respiratory derangements is associated with strong ion difference; and (2) during acute respiratory disorders, strong ion difference changes are not associated with standard base excess.

Methods: This is an individual participant data meta-analysis from multiple canine and human experiments published up to April 29, 2021.

Background And Aims: The European Atherosclerosis Society (EAS) Lipid Clinics Network promoted a survey in order to identify and understand how and when lipoprotein(a) [Lp(a)] is tested and clinically evaluated in lipid clinics throughout Europe, and the challenges that may prevent evaluation from being carried out.

Methods: This survey was divided into three areas of inquiry: background and clinical setting information of clinicians, questions for doctors who claimed not to measure Lp(a), in order to understand what were the reasons for not ordering the test, and questions for doctors who measure Lp(a), to investigate the use of this value in the management of patients.

Results: A total of 151 centres clinicians filled in the survey, out of 226 invited.

Effect of noradrenaline on propofol-induced mitochondrial dysfunction in human skeletal muscle cells.

Intensive Care Med Exp

November 2022

Department of Anaesthesia and Intensive Care of the Third Faculty of Medicine and Královské Vinohrady University Hospital, OXYLAB-Laboratory for Mitochondrial Physiology, Charles University, Prague, Czech Republic.

Background: Mitochondrial dysfunction is a hallmark of both critical illness and propofol infusion syndrome and its severity seems to be proportional to the doses of noradrenaline, which patients are receiving. We comprehensively studied the effects of noradrenaline on cellular bioenergetics and mitochondrial biology in human skeletal muscle cells with and without propofol-induced mitochondrial dysfunction.

Methods: Human skeletal muscle cells were isolated from vastus lateralis biopsies from patients undergoing elective hip replacement surgery (n = 14) or healthy volunteers (n = 4).

The IL-17 cytokine family encompasses six different homodimers and heterodimers referred to as IL-17A-F. Due to some differences in the mechanism of IL-17 inhibition, aninsufficient effect of one IL-17 inhibitor does not necessarily imply lack of efficacy of the other agent of the same class. Aim of study was analysis of the success rate of switches among IL-17 inhibitors (secukinumab, ixekizumab, and brodalumab) in patients treated in the Czech Republic.
