Cognitive dysfunction in systemic lupus erythematosus: how do we advance our understanding?

Lancet Rheumatol

April 2022

Centre for Inflammatory Diseases, School of Clinical Sciences, Monash University, Melbourne, VIC, Australia; Rheumatology Department, Monash Health, Melbourne, VIC, Australia.

Despite the high reported prevalence and burden of cognitive dysfunction in systemic lupus erythematosus (SLE), there is poor understanding of its aetiology, clinical assessment, and management. In part, this lack of understanding is due to conceptual disorganisation in the available literature. In this Review, we address key knowledge gaps in the nomenclature, assessment, and interpretation of cognitive dysfunction in SLE.

The survival of childhood Wilms tumor is currently around 90%, with many survivors reaching reproductive age. Chemotherapy and radiotherapy are established risk factors for gonadal damage and are used in both COG and SIOP Wilms tumor treatment protocols. The risk of infertility in Wilms tumor patients is low but increases with intensification of treatment including the use of alkylating agents, whole abdominal radiation or radiotherapy to the pelvis.

Objectives: A perception derived from cross-sectional studies of small systemic lupus erythematosus (SLE) cohorts is that there is a marked discrepancy between antinuclear antibody (ANA) assays, which impacts on clinicians' approach to diagnosis and follow-up. We compared three ANA assays in a longitudinal analysis of a large international incident SLE cohort retested regularly and followed for 5 years.

Methods: Demographic, clinical and serological data was from 805 SLE patients at enrolment, year 3 and 5.

Article Synopsis
  • This study examines data on pneumococcal pneumonia and invasive disease in Canadian hospitalized adults from 2010 to 2017, highlighting the importance of active surveillance for understanding disease trends.
  • It finds that adults aged 65 and older are significantly affected by both bacteremic and non-bacteremic community-acquired pneumonia (pCAP), with a notable decline in certain serotypes from 2010 to 2014, followed by a plateau from 2015 to 2017.
  • The study concludes that monitoring both types of pCAP is crucial, especially as current vaccination strategies show insufficient herd protection, necessitating further attention to vaccination efforts in adults.

Juvenile dermatomyositis (JDM) is a rare, severe autoimmune disease and the most common idiopathic inflammatory myopathy of children. JDM and adult-onset dermatomyositis (DM) have similar clinical, biological and serological features, although these features differ in prevalence between childhood-onset and adult-onset disease, suggesting that age of disease onset may influence pathogenesis. Therefore, a JDM-focused genetic analysis was performed using the largest collection of JDM samples to date.

Ventricular tachycardia (VT) is a potentially fatal cardiac rhythm disorder. Implantable cardioverter defibrillators (ICDs) are the primary management strategy for VT and have been shown to reduce the incidence of death but, ICDs do not reduce VT recurrences. Further, mounting evidence indicates that high VT burden, defined as the cumulative number of recurrent VTs or ICD shocks, is associated with an elevated risk of death; however, it is unclear if high VT burden is a cause of death or a marker of severe heart disease.

Article Synopsis
  • Catheter ablation is found to be better than taking higher doses of antiarrhythmic drugs for patients with fast heartbeats after a heart attack.
  • The study looked at 259 patients and discovered that those with a specific heart rhythm measure (VT cycle length over 400 ms) had worse results with the drug treatment compared to ablation.
  • For patients who had electrical storms (a type of rapid heartbeat), the outcomes were similar, but there might be a slight advantage for those on a specific drug called amiodarone when choosing catheter ablation over drugs.
