Any medical treatment, whether it be medication, behavioral therapy or medical procedure, can affect the patient in a very complex way. Each of these forms of medical treatment have both psychobiological and physiological effects on the patient, with interactions between those levels. When considering the specific efficacy of a new treatment, there is therefore a need to neutralize the psychobiological effect by comparing the effect of the treatment to the effect of a placebo - a treatment involving similar characteristics which does not produce that physiological effect.

Rash, a hypersensitivity reaction, is a common cause of withdrawal from an effective antiepileptic drug (AED) in patients with epilepsy. We present a case of successful desensitization to valproic acid in a 12-year-old male with childhood absence epilepsy and a hypersensitivity reaction, whose epilepsy did not respond to other AEDs. Desensitization is a practical therapeutic solution for patients who develop a non-life-threatening hypersensitivity reaction to an AED for which there may be no substitute.

Nevertheless, poor adherence, especially of immigrants from Africa, leads to a high rate of treatment failures. Background: Resistance to macrolides in beta-hemolytic streptococci and Streptococcus pneumoniae arises primarily due to Erm(B) or Mef(A). Erm(B) typically confers high level resistance to macrolides, lincosamides and streptogramin B (MLSB phenotype), whereas Mef(A) confers low level resistance to macrolides only (M phenotype).
Objectives: To investigate the incidence of macrolide resistance mechanisms in isolates of beta-hemolytic streptococci and pneumococci in Israel, with particular emphasis on inducible MLSB phenotype.
