Article Synopsis
  • A study was conducted to assess how a quality improvement program led by an intensivist could impact long-term mortality in high-risk patients who had prolonged ICU stays after cardiac surgery.
  • The research compared outcomes of two groups of patients: one group treated before the program was introduced (2004-2007) and one after (2009-2014), finding a significant decrease in mortality rates among patients with prolonged ICU stays after the program's implementation.
  • The findings indicate that the intensivist-led management approach effectively reduced long-term mortality rates, particularly for those staying in the ICU for seven days or more.

With the improvement in survival rate, cardiotoxicity has emerged as a significant adverse effect of cancer therapy. Early diagnosis of subclinical cardiac injury may allow the initiation of cardioprotective therapy and preventing the interruption of optimal cancer therapy and the development of irreversible cardiac dysfunction. In this article, we review the role of two-dimensional speckle tracking echocardiography (2D-STE), beyond the common left ventricle global longitudinal strain in the diagnosis of early subclinical cardiac injury in patients treated with cancer therapies.

Article Synopsis
  • Replication repair deficiency, resulting from mismatch repair deficiency (MMRD) and/or loss of DNA polymerase proofreading, leads to hypermutation in cancer, with microsatellite instability (MSI) being a key indicator of MMRD.
  • Genome-wide analysis reveals a connection between loss of polymerase proofreading and MSI, particularly when both replication repair mechanisms are compromised, highlighting distinct mutation signatures (MS-sigs).
  • The study emphasizes the clinical utility of MS-sigs in identifying replication repair deficiencies in cancer patients and predicting their responses to immunotherapy, enhancing diagnosis and treatment strategies.

Successful implantation is associated with a unique spatial pattern of vascular remodeling, characterized by profound peripheral neovascularization surrounding a periembryo avascular niche. We hypothesized that hyaluronan controls the formation of this distinctive vascular pattern encompassing the embryo. This hypothesis was evaluated by genetic modification of hyaluronan metabolism, specifically targeted to embryonic trophoblast cells.

Germline pathogenic sequence variants (PSVs) in BRCA1 substantially increase risk for developing breast (BC) and ovarian cancer (OvC). Yet, incomplete penetrance suggests that modifier factors affect phenotypic expression of mutant BRCA1 alleles. Analysis of identical BRCA1 PSV carriers of diverse ethnicities may provide further evidence for modifier factors.
