Background: Acute variceal bleeding (AVB), a life-threatening complication of liver cirrhosis, can be effectively treated by endoscopy, but there is a risk of early rebleeding after endoscopic variceal treatment (EVT). Thrombocytopenia is the most common hemostatic abnormality in liver cirrhosis. However, it is still unclear about whether thrombocytopenia increases the failure of EVT in cirrhotic patients with AVB.

Stratified analysis of the correlation between wedged hepatic venous pressure and portal venous pressure in patients with portal hypertension.

Sci Rep

November 2024

Liver Disease Minimally Invasive Diagnosis and Treatment Center, Beijing Shijitan Hospital, Capital Medical University, No. 10 Tie Yi Road, Yangfangdian, Haidian District, Beijing, 100038, China.

To evaluate the differences in the agreement between wedged hepatic venous pressure (WHVP) and portal venous pressure (PVP) at different hepatic venous pressure gradient (HVPG) levels to identify specific HVPG thresholds where WHVP can reliably estimate PVP, thus enhancing the accuracy of risk stratification and treatment decision-making for portal hypertension (PHT) patients. A multicenter study of 616 patients with PHT from three centers was stratified into five groups by their HVPG: HVPG < 12 (group A), 12 ≤ HVPG < 16 mmHg (group B), 16 ≤ HVPG < 20 mmHg (group C), 20 ≤ HVPG < 24 mmHg (group D), HVPG ≥ 24 mmHg (group E). Concordance was analyzed using Pearson's correlation coefficient (R), the intraclass correlation coefficient (ICC), and Bland‒Altman analysis in each HVPG stratum.

Background: Numerous studies have confirmed that gut microbiota plays a crucial role in the progression of cirrhosis. However, the contribution of gut fungi in cirrhosis is often overlooked due to the relatively low abundance.

Methods: We employed 16S ribosomal RNA sequencing, internal transcribed spacer sequencing, and untargeted metabolomics techniques to investigate the composition and interaction of gut bacteria, fungi, and metabolites in cirrhotic patients.

Article Synopsis
  • Lipids are essential for growth and reproduction, and urine-derived extracellular vesicles (EVs) are being studied as potential biomarkers for tuberculosis (TB) due to their non-invasive nature and lipid content.
  • Researchers isolated urine-derived EVs from 41 participants (including healthy individuals and those with TB) to analyze their lipid profiles and identify biomarkers using advanced statistical methods.
  • The study found 226 lipids, identifying 7 potential lipid biomarkers for TB and 6 for latent TB, with one promising biomarker, free fatty acid (FFA 20:1), showing high diagnostic performance in distinguishing TB from latent TB infection.

  • This study investigates the effectiveness and risk factors associated with emergency endoscopic treatment for esophagogastric variceal bleeding (EGVB) in critically ill patients.
  • A total of 165 patients were analyzed to determine predictors of treatment failure and mortality using statistical models.
  • Key findings indicated that certain clinical factors like white blood cell count and Child-Turcotte-Pugh score could forecast complications like endoscopic hemostasis failure and early rebleeding, leading to useful nomograms for risk assessment in clinical settings.

Article Synopsis
  • There's no current scoring system to predict cirrhosis in patients with acute-on-chronic liver failure (ACLF) without cirrhosis, which this study aims to address by developing a nomogram based on psoas muscle index (PMI).
  • The study included 274 non-cirrhotic HBV-ACLF patients, separating them into training and validation groups, and found that a higher PMI was linked to a lower risk of developing cirrhosis over 360 days.
  • The developed nomogram showed strong predictive accuracy in assessing cirrhosis risk, with AUROC values of 0.812 and 0.824 in the training and validation groups, respectively, and confirmed its effectiveness through

Constipation may accelerate the progression of Parkinson's disease (PD). The quality of life in PD patients can be significantly improved when constipation is treated, hence the disease progression may be delayed. At present, the mechanism research is still at the initial stage for acupuncture in treatment of PD, focusing on neurohumoral factors, intestinal flora, bacterial fermentation products and intestinal inflammation; and there are the problems such as single intervention, thinking limitation and insufficient cooperation among disciplines.

To screen and identify differential proteins, analyze lipid metabolism-related proteins and pathways, and explore their functions and biological processes in liver tissue of patients with alcoholic liver disease using tandem mass tag (TMT) labeling technology. Liver tissues that met the inclusion criteria were collected. Eight samples from patients with alcoholic cirrhosis and three samples from the normal control group were screened out.

The pathogenesis of portal hypertension remains unclear, and is believed to involve dysfunction of liver sinusoidal endothelial cells (LSEC), activation of hepatic stellate cells (HSC), dysregulation of endogenous hydrogen sulfide (HS) synthesis, and hypoxia-induced angiogenic responses. HS, a novel gas transmitter, plays an important role in various pathophysiological processes, especially in hepatic angiogenesis. Inhibition of endogenous HS synthase by pharmaceutical agents or gene silencing may enhance the angiogenic response of endothelial cells.

Acute-on-chronic liver failure: clinical course and liver transplantation.

Expert Rev Gastroenterol Hepatol

March 2023

Fourth Department of Liver Disease (Difficult & Complicated Liver Diseases and Artificial Liver Center), Beijing You'an Hospital Affiliated to Capital Medical University, Beijing, China.

Introduction: Acute-on-chronic liver failure (ACLF) is a clinical syndrome characterized by intense systemic inflammatory response, multiple-organ failures, and high short-term mortality in patients with chronic liver disease. ACLF is dynamic and heterogeneous, and the prognosis is closely related to the clinical course. Currently, liver transplantation (LT) remains the only potential curative treatment that improves survival of ACLF patients.

Objective: Acute-on-chronic liver failure (ACLF) has a high prevalence and short-term mortality. Monocytes play an important role in the development of ACLF. However, the monocyte subpopulations with unique features and functions in ACLF and associated with disease progression remain poorly understood.

Background: The relationship between skeletal muscle mass index (SMI) and cirrhosis incidence in patients with non-cirrhotic acute-on-chronic (ACLF) has not been clarified. This study aimed to assess the predictive value of SMI on the incidence of long-term cirrhosis in male non-cirrhotic ACLF patients.

Materials And Methods: Male ACLF patients who were free of liver cirrhosis were retrospectively included in this study.

Background And Aims: Acute-on-chronic liver failure (ACLF) tends to progress rapidly with high short-term mortality. We aimed to create a widely applicable, simple prognostic (WASP) score for ACLF patients.

Methods: A retrospective cohort of ACLF cases recruited from three centers in China were divided into training and validation sets to develop the new score.

Fourth Department of Liver Disease (Difficult and Complicated Liver Diseases and Artificial Liver Center), Beijing You'an Hospital Affiliated to Capital Medical University, Beijing 100069, China.

Non-alcoholic fatty liver disease (NAFLD) is a disorder of lipid metabolism. The lipotoxic intermediates of lipid metabolism cause mitochondrial dysfunction and endoplasmic reticulum stress. Organelle-specific autophagy is responsible for the removal of dysfunctional organelles to maintain intracellular homeostasis.

Acute-on-chronic liver failure (ACLF) is an acute deterioration of chronic liver disease with high short-term mortality. The inclusion or exclusion of previously decompensated cirrhosis (DC) in the diagnostic criteria of ACLF defined by the Asian Pacific Association for the Study of the Liver (APASL-ACLF) has not been conclusive. We aimed to evaluate the prognostic impact of decompensated cirrhosis in ACLF.
