Japanese patients with polycystic ovary syndrome (PCOS) exhibit distinct body type characteristics, such that the rate of overweight/obese women is remarkably low. In addition, hyperandrogenism is relatively rare among Japanese PCOS patients. Therefore, these factors are considered in PCOS diagnostic criteria used in Japan.

Article Synopsis
  • * Data from a nationwide survey included 986 PCOS cases and 965 controls, revealing that 75% of Japanese PCOS patients were non-obese/overweight, with differing hormone levels and incidence rates of symptoms when compared to obese/overweight patients.
  • * The findings suggest that including hirsutism in diagnostic criteria could improve the accuracy of PCOS diagnoses, particularly aligning with international standards, and highlight the importance of measuring basal luteinizing hormone (LH) levels for understanding the hormonal profile of these patients.

Insertion of a spacer block translates the tibia anteriorly during evaluation of soft tissue balance in cruciate-retaining total knee arthroplasty.

Knee

June 2023

Department of Orthopedics, Institute of Biomedical Sciences, Tokushima University Graduate School, 3-18-15 Kuramoto, Tokushima City, Tokushima 770-8503, Japan. Electronic address:

Background: Soft tissue balance is an important determinant of the outcome of total knee arthroplasty (TKA). However, there are differences in the joint gap and ligament balance between the osteotomized femoral and tibial surfaces and those after TKA. The aim of this study was to compare the relationship between the femur and tibia at insertion of a spacer block with that after cruciate-retaining (CR) TKA.

Article Synopsis
  • A study was conducted using the sequence kernel association test (SKAT-O) to explore the potential genetic factors that contribute to polycystic ovary syndrome (PCOS) through rare gene variants.
  • The analysis involved comparing exome data from 44 Japanese women with PCOS against 301 control participants, revealing that certain rare variants were more prevalent in the PCOS group.
  • Although no single gene was found to have a major impact on the overall risk of PCOS, certain rare damaging variants could be risk factors for some individuals with the condition.

We report a case of subacute combined spinal cord degeneration (SCD) caused by vegetarianism and autoimmune gastritis, which is rarely reported in Japan, and which showed improvement in symptoms and imaging findings after vitamin B administration. As delayed treatment can lead to irreversible damage, we suggest that patients with characteristic abnormal signals in the posterior cervical cord should be examined while considering the possibility that SCD may occur even in the absence of a history of gastrectomy or heavy drinking. We also describe the patient's reversible abnormal signals in the cerebral white matter on magnetic resonance imaging, indicative of an early sign of leukoencephalopathy associated with vitamin B deficiency.

Article Synopsis
  • This study evaluates how the position of the baseplate in reverse shoulder arthroplasty (RSA) impacts the placement and length of superior screws, aiming to reduce the risk of suprascapular nerve injury.
  • Researchers used 3D computer simulations based on CT scans of cadaver shoulders to analyze various baseplate positions and their effects on screw measurements.
  • Results suggested that using a 1-7 o'clock rotation with a 0-degree inferior tilt for the baseplate helps increase the distance from the superior screw to the suprascapular notch, minimizing nerve injury risks despite a shorter inferior screw length.
