Infectious virus shedding from individuals infected with severe acute respiratory syndrome-coronavirus 2 (SARS-CoV-2) is used to estimate human-to-human transmission risk. Control of SARS-CoV-2 transmission requires identifying the immune correlates that protect infectious virus shedding. Mucosal immunity prevents infection by SARS-CoV-2, which replicates in the respiratory epithelium and spreads rapidly to other hosts.

Introduction: Necitumumab plus gemcitabine and cisplatin (GCN) is a standard therapy for patients with advanced lung squamous cell carcinoma (LSqCC). However, the efficacy and tolerability of GCN in second-line or later treatment for patients previously treated with immune checkpoint inhibitors (ICIs) remain unknown.

Methods: This multicenter, retrospective, cohort study assessed the efficacy and tolerability of GCN initiated between November 1, 2019 and March 31, 2022 as second-line to fourth-line treatment in patients with advanced LSqCC who had been pretreated with ICIs.

Introduction: Since May 2019, comprehensive genomic profiling (CGP) has been covered by Japan's health insurance system for patients with solid tumours that have progressed on standard chemotherapy, rare tumours or tumours of unknown primary origin. Although CGP has the potential to identify actionable mutations that can guide the selection of genomically matched therapies for patients with advanced cancer and limited treatment options, less than 10% of patients benefit from CGP testing, which may have a negative impact on patients' mental status. The aim of this study is to investigate the prevalence of psychological distress and associated factors among patients with advanced cancer who are undergoing CGP testing across Japan.

Risk factors of severe coronavirus disease 2019 (COVID-19) have been previously reported; however, histological risk factors have not been defined thus far. The aim of this study was to clarify subclinical hidden interstitial lung disease (ILD) as a risk factor of severe pneumonia associated with COVID-19. We carefully examined autopsied lungs and chest computed tomography scanning (CT) images from patients with COVID-19 for interstitial lesions and then analyzed their relationship with disease severity.

  • Allogeneic hematopoietic stem cell transplantation (HCT) is a potential curative treatment for patients with myelodysplastic syndrome with a complex karyotype (CK-MDS), although there are limited studies focusing on this specific group.
  • In a study involving 691 CK-MDS patients, the overall survival (OS) rate after 3 years was found to be 29.8%, with various factors impacting survival rates.
  • Key risk factors linked to reduced OS included older age, male sex, poor comorbidity index, a need for red or platelet blood transfusions, incomplete remission, a high number of karyotype abnormalities, and the presence of a monosomal karyotype.

  • A multicenter study in Japan was conducted to evaluate adverse events (AEs) related to the tag-less PillCam patency capsule (PC) used to prevent small bowel capsule endoscope (CE) retention.
  • Out of 2578 patients, 74 AEs were identified, primarily involving residual parylene coating, small bowel obstruction, and CE retention, with a 2.37% incidence rate of AEs overall.
  • The study concluded that while the PC is generally safe, careful assessment of patient history and accurate localization of the capsule are important to mitigate risks.

  • Amyloidosis is a group of disorders where amyloid protein builds up in various tissues, causing functional issues.
  • A 70-year-old Japanese woman experienced left hydronephrosis due to a lower ureteral obstruction, leading to a suspicion of a tumor despite biopsies showing no malignancy.
  • After surgery to remove the affected section of her ureter, she was diagnosed with localized ureteral amyloidosis and remained free of recurrence for 9 months after the procedure.

Article Synopsis
  • - The study conducted by the Yokohama Medical Control Council aimed to evaluate the safety and effectiveness of prehospital CPAP therapy for patients with severe respiratory failure, despite Japanese laws restricting paramedics from administering this treatment.
  • - From October 2020 to January 2022, 661 Doctor Car dispatches occurred, with CPAP administered to only 13 patients due to COVID-19 concerns; however, significant improvements in oxygen saturation levels were observed post-treatment.
  • - This research represents the first prospective observational study on prehospital CPAP administration in Japan, showing no worsening in patient conditions after treatment, primarily for heart failure-related respiratory distress.

  • - The JIPANG study is a phase III trial comparing the effectiveness of pemetrexed plus cisplatin (PemP) to vinorelbine plus cisplatin (NP) in treating stage II-IIIA nonsquamous non-small-cell lung cancer.
  • - The trial included 804 patients, with results showing that the median recurrence-free survival (RFS) was 37.5 months for NP and 43.4 months for PemP, revealing no significant difference.
  • - After a median follow-up of 77.3 months, overall survival (OS) rates were comparable between the two treatments, indicating that PemP has similar efficacy to NP in long-term outcomes for these patients.

Background: 5-aminolevulinic acid (5-ALA), a natural amino acid that is marketed alongside sodium ferrous citrate (SFC) as a functional food, blocks severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) proliferation in vitro and exerts anti-inflammatory effects. In this phase II open-label, prospective, parallel-group, randomized trial, we aimed to evaluate the safety and efficacy of 5-ALA in patients with mild-to-moderate coronavirus disease 2019.

Methods: This trial was conducted in patients receiving 5-ALA/SFC (250/145 mg) orally thrice daily for 7 days, followed by 5-ALA/SFC (150/87 mg) orally thrice daily for 7 days.

Article Synopsis
  • - The study investigates the connection between resistance to osimertinib (an EGFR-TKI) and PD-L1 expression in patients with EGFR-mutated non-small cell lung carcinoma (NSCLC).
  • - Analyzing 64 patients, researchers found that those with a PD-L1 tumor proportion score (TPS) ≥20% had a median progression-free survival (PFS) of 9.1 months, compared to 28.1 months for those with TPS <20%.
  • - Results suggest that a higher PD-L1 TPS may indicate early resistance to osimertinib, highlighting the importance of PD-L1 status in managing treatment for EGFR-mutated NSCLC.
