Introduction: Non-alcoholic fatty liver disease, now known as metabolic dysfunction-associated steatotic liver disease (MASLD), is a phenotype of the metabolic syndrome in the liver and is clearly associated with metabolic abnormalities such as hyperglycaemia and dyslipidaemia. Although the prevalence of MASLD is increasing worldwide, there is currently no consensus on the efficacy and safety of the drugs used to treat MASLD/metabolic dysfunction-associated steatohepatitis (MASH). Pemafibrate, a selective peroxisome proliferator-activated receptor alpha modulator, was designed to have higher peroxisome proliferator-activated receptor alfa (PPARα) agonist activity and selectivity than existing PPARα agonists, and in development trials, without increasing creatinine levels, lipid parameters and alanine aminotransferase (ALT) were significantly improved.

Background And Aims: Although numerous noninvasive diagnostic methods have been developed to predict liver fibrosis in metabolic dysfunction-associated steatotic liver disease (MASLD), they lack markers for predicting lobular inflammation, hepatocellular ballooning, or changes related to metabolic dysfunction-associated steatohepatitis (MASH). We examined serum cytokeratin 18 fragment (CK18F) as a noninvasive marker for predicting treatment response and "at-risk MASH" and "MASH resolution" in patients with MASLD.

Methods: One-hundred-and-ten patients with MASLD who underwent repeated biopsy were enrolled (age, 4 [0.

Article Synopsis
  • The study aimed to determine whether sarcopenia, a condition characterized by loss of skeletal muscle mass, affects survival rates in patients with platinum-resistant recurrent ovarian cancer.
  • It involved 52 patients whose body composition was analyzed using abdominal CT scans, revealing that sarcopenia negatively impacts overall survival, with a hazard ratio of 1.93 indicating almost double the risk of shorter survival.
  • Subgroup analyses showed sarcopenia was a significant risk factor especially in patients with elevated CA125 levels and a higher neutrophil-to-lymphocyte ratio, further emphasizing its role in patient prognosis.

Purpose: In this study, we aimed to evaluate the safety and effectiveness of naldemedine for treating opioid-induced constipation (OIC) in patients with advanced cancer, who are receiving palliative care, and particularly explored its early effects.

Methods: Palliative care teams and inpatient palliative care units across 14 institutions in Japan were included in this multicenter, prospective, observational study. Patients who were newly prescribed a daily oral dose of 0.

Background: Manual chart review using validated assessment tools is a standardised methodology for detecting diagnostic errors. However, this requires considerable human resources and time. ChatGPT, a recently developed artificial intelligence chatbot based on a large language model, can effectively classify text based on suitable prompts.

Article Synopsis
  • A study compared kidney outcomes in patients aged 75 and older with diabetic kidney disease (DKD) using SGLT2 inhibitors versus other glucose-lowering medications.
  • The research used data from the Japan Chronic Kidney Disease Database, focusing on changes in estimated glomerular filtration rate (eGFR) and severe kidney progression.
  • Results indicated that those using SGLT2 inhibitors experienced a slower decline in eGFR and better overall kidney health compared to those on other medications, showing benefits for older DKD patients.

Objectives: The superiority and usefulness of liquid material over particles for embolization have been a topic of debate due to differences in materials and techniques. This study aimed to identify the complications and outcomes associated with both embolization materials.

Methods: This retrospective multicenter cohort study included 93 patients from an endovascular treatment registry, treated from January 1, 2018 to May 31, 2022.

The combination therapy of v-Raf murine sarcoma viral oncogene homolog B1 (BRAF) and mitogen-activated protein kinase kinase (MEK) inhibitors is approved for treating patients with BRAF V600E-positive tumours, including melanoma and lung cancer. Several case reports indicated autoimmune side effects associated with the use of BRAF and MEK inhibitors. Still, the effects of these drugs on the immune system were not fully elucidated.

In clinical cases of pancreas divisum, endoscopic retrograde cholangiopancreatography often necessitates cannulation of the pancreatic duct through the minor papilla. Nevertheless, this procedure can be challenging because of the small size of the minor papilla and the difficulty in visualizing the ductal orifice. A new image-enhanced endoscopy technique called texture and color enhancement imaging (TXI) has been developed, which enhances texture, brightness, and color compared with white-light imaging, resulting in subtle differences in the surface mucosa.

Article Synopsis
  • The study compared the effectiveness of two types of self-expandable metallic stents (L-type and B-type) for treating distal malignant biliary obstruction (DMBO), focusing on reducing recurrent biliary obstruction (RBO).
  • It found that B-type stents had a significantly lower rate of RBO within one year (17.2%) compared to L-type stents (44.4%), although both types had similar safety profiles.
  • Overall survival times were not significantly different between the groups, with median survival times of 158 days for L-type and 204 days for B-type stents.

Background/aims: The precise incidence of symptomatic uncomplicated diverticular disease (SUDD) and its effects on the quality of life (QOL) remain unclear, particularly in Asian patients with right-sided SUDD. We assess the prevalence of SUDD and its impact on QOL in a real-world population.

Methods: Five institutional cohorts of patients who received outpatient treatment for unexplained abdominal symptoms from January 15, 2020 to March 31, 2022, were included.

Background And Aim: Ischemic colitis (IC) is a benign disease associated with acute lower gastrointestinal bleeding and frequent recurrence. While several studies have investigated risk factors for IC onset, few have assessed the risk factors for recurrent IC. This study aimed to identify risk factors for recurrent IC.

Article Synopsis
  • * The study examines mitochondrial function's role in embryo development, showing that euploid embryos increased and mosaic embryos decreased during culture, alongside a lower number of mitochondrial DNA mutations in euploid embryos.
  • * Findings indicate that aneuploidy might reduce in embryos after implantation, and assessing mtDNA mutations could be a new strategy for selecting viable mosaic embryos for transfer.

Introduction: In general, caesarean sections are performed under spinal anaesthesia. Hypotension after spinal anaesthesia adversely affects both the mother and fetus. Although several studies have used pulse oximetry-derived indices, such as pulse perfusion index (PI) and Pleth variability index (PVI), to predict hypotension after spinal anaesthesia, the predictive ability of the PI and PVI remain controversial.
