Although mutations in dozens of genes have been implicated in familial forms of amyotrophic lateral sclerosis (fALS) and frontotemporal degeneration (fFTD), most cases of these conditions are sporadic (sALS and sFTD), with no family history, and their etiology remains obscure. We tested the hypothesis that somatic mosaic mutations, present in some but not all cells, might contribute in these cases, by performing ultra-deep, targeted sequencing of 88 genes associated with neurodegenerative diseases in postmortem brain and spinal cord samples from 404 individuals with sALS or sFTD and 144 controls. Known pathogenic germline mutations were found in 20.

No official clinical reference values have been established for MostGraph, which measures total respiratory resistance and reactance using the forced oscillation technique, complicating result interpretation. This study aimed to establish a reference range for MostGraph measurements and examine its usefulness in discriminating participants with asthma from controls (participants without any respiratory diseases). The study also aimed to investigate the effectiveness of deep learning in discriminating between the two aforementioned groups.

Background: With a prevalence of only 1% among all breast cancers in Japan, apocrine carcinoma (AC) is a rare type of breast cancer, and its clinicopathological characteristics remain unclear. The aim of this study was to evaluate the characteristics and prognosis of AC, in relation to the presence or absence of androgen receptor (AR).

Methods: We conducted a retrospective multi-center case-control study (Yokohama Clinical Oncology Group (YCOG): YCOG1701 study) in Japan.

Objective: The association between the severity of COVID-19 and gastrointestinal (GI) bleeding is unknown. This study aimed to determine whether the severity of COVID-19 is a risk factor for GI bleeding.

Design: A multicentre, retrospective cohort study was conducted on hospitalised patients with COVID-19 between January 2020 and December 2021.

Context: Physical symptoms such as pain and cancer-related fatigue limit physical function and activities of daily living among patients with terminal cancer, which can lead to a decline in quality of life. Therefore, comprehensive functional impairments should be evaluated to determine the progression of the disease and the effectiveness of palliative treatment.

Objective: To validate the psychometric properties of the Japanese version of the Edmonton Functional Assessment Tool 2 (EFAT2-J).

Article Synopsis
  • The study aimed to test whether adjuvant chemotherapy for stage III colon cancer could be effectively shortened from the conventional 6 months to 3 months using the SOX regimen (S-1 and oxaliplatin).
  • In a randomized phase II trial, patients received either 3 months or 6 months of SOX treatment, with the primary goal of measuring 3-year disease-free survival (DFS).
  • Results showed that the 3-year DFS rates were similar for both groups (75% for 6 months and 76.9% for 3 months), indicating that shortening the treatment did not significantly improve outcomes, and the primary endpoint was not met.

November 2023

Department of Cardiovascular Surgery, Yokohama City Minato Red Cross Hospital, 3-12-1 Shinyamashita Naka-ku, Yokohama, Kanagawa 231-8682, Japan. Electronic address:

Introduction: Patients undergoing hemodialysis exhibit a high incidence of subclavian steal syndrome. Many cases of endovascular treatment for subclavian artery stenosis were only reported recently; however, the long-term results of surgical treatment are also important. Herein, we report a case of subclavian steal syndrome treated with common carotid-axillary bypass surgery in a patient undergoing hemodialysis.

Objective: Basilar artery occlusion (BAO) is an infrequent form of acute life-threatening stroke and may occur secondary to vertebral artery dissection (VAD). VAD, which occurs spontaneously and sometimes results from mechanical stress or blunt force trauma to the neck, sometimes occurs in the V1-V2 junction, but there are not many reported cases of those. Herein, we report a pictorially illustrative and clinically informative case of VAD in the V1-V2 junction following BAO.

  • The 2021 WHO classification of CNS tumors combines histopathology and molecular profiling to effectively differentiate between adult malignant brain tumors, particularly gliomas and primary CNS lymphomas (PCNSL).
  • A new intraoperative integrated diagnostic (i-ID) system has been developed that enhances traditional frozen-section diagnosis with a qPCR-based genotyping assay, crucial for therapeutic decision-making.
  • After testing the i-ID system on 101 patients, it showed high reliability, matching permanent diagnoses in nearly all cases, thus proving to be a valuable tool for accurate classification of adult malignant CNS tumors.

  • Pegfilgrastim, a granulocyte-colony stimulating factor, is used to reduce the risk of febrile neutropenia during chemotherapy, and its biosimilar, MD-110, was tested for safety and efficacy in early-stage breast cancer patients undergoing chemotherapy.
  • In a study with 101 participants receiving docetaxel and cyclophosphamide, the mean duration of severe neutropenia was only 0.2 days, significantly lower than the safety threshold.
  • Common side effects included alopecia, constipation, and malaise, but MD-110 showed no additional safety issues compared to the original pegfilgrastim.

  • Diabetic ketoacidosis (DKA) and hyperglycemic hyperosmolar syndrome (HHS) are serious diabetes complications that need more research on their clinical profiles.
  • A study analyzed the medical records of 771 hospitalized adults in Japan from 2012 to 2016 to compare outcomes among different severity levels of DKA and HHS.
  • Findings revealed that common triggers included poor medication adherence and infections, with ICU admission rates at 44.4% and mortality rates of 2.8% for DKA and 7.1% for HHS, indicating no significant difference in mortality among DKA severity levels.

  • This observational study evaluated the outcomes of peritoneal dialysis (PD) catheter placement in Japan to see if they align with the International Society for Peritoneal Dialysis guidelines.
  • The study tracked 409 adult patients who underwent PD catheter placement, finding a low rate of catheter patency failure (6.2%) and a technique failure rate of 15.3% after 12 months.
  • Factors influencing technique failure included lower serum albumin levels and type of catheter used, while diabetes increased the risk of perioperative infections; however, overall complications met ISPD audit standards.

Article Synopsis
  • This study aimed to improve the detection of somatic UBA1 variants in patients suspected of having VEXAS syndrome and to create a scoring system to identify those with pathogenic variants.
  • Researchers analyzed 89 Japanese patients (mostly male, median age 69.3 years) using advanced genetic techniques such as PNA-PCR and deep sequencing to screen for UBA1 variants.
  • They found that 44.9% of patients had pathogenic variants, and their scoring system, which included factors like age and specific symptoms, effectively predicted the presence of these variants with a high accuracy (AUC of 0.908).

Key Clinical Message: Epidural pneumatosis and pneumomediastinum are rare, benign complications of diabetic ketoacidosis. As they can mimic serious conditions including esophageal rupture, diagnostic evaluation, and attentive monitoring are crucial.

Abstract: Diabetic ketoacidosis can rarely present with epidural pneumatosis and pneumomediastinum, possibly due to forceful vomiting and Kussmaul breathing.

A woman in her late 40s presented with multiple abnormal shadows on high-resolution CT (HRCT), was treated with abemaciclib for recurrent right breast cancer post-surgery and chemoradiation therapy. During the 10-month chemotherapy, HRCT revealed a recurrent pattern of a partly appearing and disappearing organising pneumonia pattern without clinical symptoms. Bronchoalveolar lavage analysis revealed lymphocytosis, while transbronchial lung biopsy revealed alveolitis with epithelial cell injury.
