Department of Plastic and Reconstructive Surgery, Yokohama City Minato Red Cross Hospital, Kanagawa, Japan; and Department of Plastic and Reconstructive Surgery, Kyoto University, Kyoto, Japan.

Unlabelled: We devised an improved type of the V-Y subcutaneous pedicle flap with the elements of both advancement and rotation flaps. This flexible curved V-Y subcutaneous flap was used for facial skin defect reconstruction in 15 patients. Curved flaps were designed according to the elasticity of the surrounding skin and the postoperative scar direction.

Background: The aim of this study was to examine volumetric changes in fat flaps after stresses as well as their relationship with capillary density (CD) in a Zucker fatty rat model.

Methods: A total of 12 male Zucker-fa/fa rats were randomly divided into two stress groups. Superficial epigastric fat flaps were evaluated on the right side as control and on the left side as the stress (ischemic or congestive) condition in the same rat in order to avoid biases.

Background: Anaphylaxis is a serious type I allergic reaction that occurs suddenly and can result in death, but it is sometimes difficult to differentiate from other diseases, and physicians must rely on symptoms alone for its diagnosis. Meanwhile, fractional exhaled nitric oxide (FeNO) concentration, used in assessing airway inflammation in bronchial asthma, is known to be affected by atopic disposition. The possible role of FeNO measurements was evaluated in patients with anaphylaxis.

Objective: Endovascular treatment (EVT) using a popliteal approach is effective for superficial femoral artery (SFA) chronic total occlusion (CTO); however, its effectiveness, safety, and consequent complications are unclear.

Materials And Methods: We studied 324 consecutive EVTs (in 187 patients) performed at three centers between April 2008 and March 2013, and selected all EVTs that included SFA CTO regions. A total of 91 EVTs (in 65 patients) were included and divided into two groups; "with popliteal approach" (WPA) and "without popliteal approach" (WOPA).

Effect of Ezetimibe on LDL-C Lowering and Atherogenic Lipoprotein Profiles in Type 2 Diabetic Patients Poorly Controlled by Statins.

PLoS One

May 2016

Toho University Ohashi Medical Center, Department of Diabetes and Metabolism, Tokyo, Japan; Mitsui Memorial Hospital, Division of Diabetes and Metabolism, Tokyo Japan.

Background: There exists a subpopulation of T2DM in whom first-line doses of statin are insufficient for optimally reducing LDL-C, representing a major risk of CVD. The RESEARCH study focuses on LDL-C reduction in this population along with modifications of the lipid profiles leading to residual risks.

Methods: Lipid changes were assessed in a randomized, multicenter, 12-week, open-label study comparing a high-potency statin (10mg of atorvastatin or 1mg of pitavastatin) plus ezetimibe (EAT: n = 53) with a double dose of statin (20mg of atorvastatin or 2mg of pitavastatin) (DST: n = 56) in DM subjects who had failed to achieve the optimal LDL-C targets.

Conclusion: The frequency of headaches in patients complaining of chronic dizziness is closely related to the severity of the dizziness impairments and mood states, such as anxiety. It is, therefore, important to treat co-morbid headache in patients with chronic dizziness. The severity of the dizziness impairments and other mood states, including anxiety, did not differ between patients with migraine or non-migraine headaches.

Rotational Distraction for the Treatment of Severe Mandibular Retrognathia.

Plast Reconstr Surg Glob Open

Department of PRS, Yokohama City Minato Red Cross Hospital, Kanagawa, Japan; Department of OMFS, Kochi Health Science Center, Kochi, Japan; Department of OMFS, University of Washington, Seattle, Wash.; and Department of PRS, Tokyo Medical and Dental University, Tokyo, Japan.

Backgrounds: The main problem with intraoral distraction of the mandible is the inability to achieve the three-dimensional mandibular correction as planned preoperatively. We developed a technique that allows spontaneous changes in the direction of mandibular elongation using an intraoral distractor.

Methods: After mandibular osteotomy, the distractor is fixed to the distal segment of the mandible using a single bicortical screw, allowing anterior-posterior, vertical and limited lateromedial changes in the vector of distraction.
