Technology transfer, intellectual property, and the fight for the soul of WHO.

Debates over the scope, terms, and governance of technology transfer-the sharing of essential technical information, know-how, and materials needed to manufacture a health product-are prominent and controversial in international health diplomacy. These debates have become focal points in recent contentious negotiations to amend the International Health Regulations (IHR) and draft a global Pandemic Agreement. While some countries advocate for automatic or compulsory mechanisms to facilitate access to health technologies, especially in times of crisis, others oppose legal frameworks that mandate non-voluntary participation by the pharmaceutical industry.

Financial Toxicity Among Patients With Traumatic Finger Amputation: A Retrospective Study.

Background: Patients face increasing financial toxicity (FT), defined as emotional distress due to the cost of medical treatment. However, little is known regarding FT in the context of upper extremity trauma.

Methods: We surveyed patients who sustained traumatic finger amputation (October 21, 2011-January 1, 2021).

Neurofeedback: potential for abuse and regulatory frameworks in the United States.

Neurofeedback is a brain-training technique that continues to develop via ongoing innovations, and that has broadening potential impact. Once confined primarily to clinical and research settings, it is increasingly being used in the general population. Such development raises concerns about the current regulatory mechanisms and their adequacy in protecting patterns of economic and political decision-making from the novel technology.

AI can help humans find common ground in democratic deliberation.

Finding agreement through a free exchange of views is often difficult. Collective deliberation can be slow, difficult to scale, and unequally attentive to different voices. In this study, we trained an artificial intelligence (AI) to mediate human deliberation.

'Our project, your problem?' A case study of the WHO's mRNA technology transfer programme in South Africa.

In June 2021 the World Health Organization (WHO) and the Medicines Patent Pool (MPP) launched an mRNA technology transfer programme. With a South African consortium serving as the hub, the programme aimed to increase vaccine manufacturing capacity in low- and middle-income countries (LMICs) in view of the "vaccine apartheid" that was observed during COVID-19. Following Clarke's "situational analysis," the present study assessed whether the mRNA programme differs from the approach and practices that comprise current biopharmaceutical production.

How Should Treatment of Animals Beyond the Lab Factor Into Institutional Review?

Discussions of nonhuman research ethics tend to focus on what we owe nonhuman research subjects in laboratory settings only. But humans make critical decisions about these animals outside the lab, too, during breeding, transportation, and end-of-study protocols. This article reviews extra-lab risks and harms to nonhuman research subjects, focusing on the most commonly and intensively used animals like rodents and fishes, and argues that extra-lab risks and harms merit ethical consideration by researchers and institutional review.

A framework for promoting online prosocial behavior via digital interventions.

Digital interventions for prosocial behavior are increasingly being studied by psychologists. However, academic findings remain largely underutilized by practitioners. We present a practical review and framework for distinguishing three categories of digital interventions--proactive, interactive, and reactive--based on the timing of their implementation.

Shrinking the footprint of the criminal legal system through policies informed by psychology and neuroscience.

The footprint of the legal system in the United States is expansive. Applying psychological and neuroscience research to understand or predict individual criminal behavior is problematic. Nonetheless, psychology and neuroscience can contribute substantially to the betterment of the criminal legal system and the outcomes it produces.

Cognitive Motor Dissociation in Disorders of Consciousness.

Background: Patients with brain injury who are unresponsive to commands may perform cognitive tasks that are detected on functional magnetic resonance imaging (fMRI) and electroencephalography (EEG). This phenomenon, known as cognitive motor dissociation, has not been systematically studied in a large cohort of persons with disorders of consciousness.

Methods: In this prospective cohort study conducted at six international centers, we collected clinical, behavioral, and task-based fMRI and EEG data from a convenience sample of 353 adults with disorders of consciousness.

How Should a Medical-Legal Partnership Address Unique Needs of People With Criminal Legal System Involvement?

Medical-legal partnerships (MLPs) are well suited to address health-harming legal needs associated with the collateral consequences of mass incarceration in the United States, such as those that limit access to food, housing, employment, and family reunification postrelease. MLP innovations seek to expand the current model to address patients' criminal, as well as postrelease, civil legal needs by including community health workers and some patients as legal partners and creating coalitions to promote local and state policy change. Overall, this article explains how these MLP innovations can support rights of people returning to communities after incarceration and can be leveraged to mitigate criminal legal system involvement.

Public discourse narratives: from 'Secret Aid Worker' discontent to shifting power in humanitarian systems.

Public discourse is rich in meaning, reflecting consensus, dissent, and change. Yet, very little public discourse on the humanitarian sector has been authored by aid workers themselves. We conducted a thematic analysis of the 'Secret Aid Worker' (SAW) series, published in The Guardian newspaper between 2015 and 2018, the only corpus of data on humanitarian life experiences publicly accessible through mainstream media.

Ephemeral stream water contributions to United States drainage networks.

Ephemeral streams flow only in direct response to precipitation and are ubiquitous landscape features. However, little is known about their influence on downstream rivers. Here, we modeled ephemeral stream water contributions to the contiguous United States network of more than 20 million rivers, lakes, and reservoirs, finding that ephemeral streams contribute, on average, 55% of the discharge exported from regional river systems, as defined by the United States Geological Survey.

A thematic analysis of disinformation in gender-affirming healthcare bans in the United States.

Context: Bans on gender-affirming care (GAC) for transgender and gender-expansive (TGE) people are grounded in scientific disinformation and have been challenged in American courts.

Methods: Five legal filings by state officials in defense of GAC restriction from initial litigation were analyzed using reflexive thematic analysis. Themes and subthemes of disinformation were identified after review and analysis of these filings.
