Importance: As few studies exist examining postoperative functional outcomes in patients undergoing robotic sacrocolpopexy and ventral rectopexy, results from this study can help guide surgeons in counseling patients on their outcomes.

Objective: The aim of the study was to evaluate functional outcomes and overall postoperative satisfaction as measured by the Pelvic Floor Disability Index 20 (PFDI-20), Pelvic Organ Prolapse/Urinary Incontinence Sexual Questionnaire (PISQ-12), and Patient Global Impression of Improvement Scale (PGI-I) in patients who underwent combined robotic ventral rectopexy and sacrocolpopexy for concomitant pelvic organ prolapse (POP) and rectal prolapse or intussusception (RP/I).

Methods: This was a retrospective cohort and survey study of patients with combined POP and RP/I who underwent the previously mentioned surgical repair between January 2018 and July 2021.

Article Synopsis
  • - The study aimed to compare the outcomes of three surgical approaches for pelvic organ prolapse (POP) over 12 months in women with uterovaginal or posthysterectomy vaginal prolapse, using data from a multicenter registry.
  • - Among 1,153 women, 777 chose surgical treatment, and 641 underwent apical repair. The recurrence rates of POP were similar across the three surgical techniques: native tissue repair, sacrocolpopexy, and colpocleisis, with overall recurrence rates of 6.5% subjective and 4.7% anatomic.
  • - Factors influencing recurrence included concurrent perineorrhaphy, which lowered the risk, while prior hystere

Article Synopsis
  • This study aimed to investigate how common polycystic ovarian syndrome (PCOS) and hyperandrogenism are in adolescent transmasculine patients seeking gender-affirming care at a tertiary center.
  • The findings indicated that 23.8% of these patients had PCOS, with those affected showing higher levels of androgens and body mass indexes compared to those without the condition.
  • The study concludes that PCOS may be more prevalent in transmasculine patients than in the general population, highlighting the importance of thorough counseling and screening for health risks associated with PCOS in this group.

Center for Urogynecology & Pelvic Reconstructive Surgery, Subspecialty Care for Women's Health, Women's Health Institute, Cleveland Clinic, 9500 Euclid Avenue, A81, Cleveland, OH, 44195, USA.

Introduction And Hypothesis: There are sparse data on the use of postoperative pelvic floor physical therapy (PFPT) in patients undergoing vaginoplasty. The primary objective of this study was to compare the impact of PFPT on the ease of vaginal dilation after vaginoplasty in transgender women. We hypothesized that patients undergoing PFPT would report better ease of vaginal dilation following surgery.

Purpose: The main goal of this study is to assess the impact of tumor manipulation on the presence of lympho-vascular space invasion and its influence on oncological results.

Methods: We performed a retrospective multi-centric study amongst patients who had received primary surgical treatment for apparently early-stage endometrial cancer. A multivariate statistical analysis model was designed to assess the impact that tumor manipulation (with the use of uterine manipulator or preoperative hysteroscopy) has on lympho-vascular development (LVSI) in the final surgical specimen.

Objective: Our objective was to perform a 5-year cost-effectiveness analysis of transvaginal hysteropexy (HP) via sacrospinous ligament fixation (SS) or uterosacral ligament suspension (US) versus vaginal hysterectomy (VH) with apical suspension via sacrospinous ligament fixation (SS) or uterosacral ligament suspension (US) for the treatment of uterine prolapse.

Design: A decision analytic model assessed the cost-effectiveness of the surgical intervention over a 5-year horizon.

Setting: This model was constructed using TreeAge® software.

Introduction And Hypothesis: To perform a cost-effectiveness analysis of concurrent posterior repair performed at the time of laparoscopic hysterectomy with sacrocolpopexy over a 7-year time period. We hypothesize it is not cost-effective to perform a posterior colporrhaphy.

Methods: We used TreeAge Pro® to construct a decision model with Markov modeling to compare sacrocolpopexy with and without concurrent posterior repair (SCP and SCP+PR) over a time horizon of 7 years.

Cara D. Dolin is a Maternal Fetal Medicine Specialist, Ob/Gyn and Women's Health Institute, Cleveland Clinic, Cleveland, Ohio.

Point-of-care ultrasonography (POCUS) is a tool that can be used to evaluate critically ill obstetric patients, in the same way as for nonpregnant patients. With knowledge of the physiology and anatomical changes of pregnancy, POCUS can provide meaningful information to help guide clinical management. A POCUS cardiothoracic evaluation for left and right ventricular function, pulmonary edema, pleural effusion, and pneumothorax can be performed in pregnancy.

Endometrial cancer is the most common gynecologic malignancy in developed countries, with increasing incidence and mortality rates worldwide. While most cases are successfully treated with surgery, first-line treatment options for metastatic or recurrent endometrial cancer involve significant toxicities. Imprecise classification of heterogeneous subgroups further complicates treatment decisions and interpretation of clinical trial results.

Section of Minimally Invasive Gynecologic Surgery, Obstetrics, Gynecology, and Women's Health Institute, Cleveland Clinic, Cleveland, Ohio, USA.

In this narrative review, we describe historical and contemporary influences that prevent patients with fibroids from getting appropriate medical care. Using patient stories as examples, we highlight how misogyny on all levels hurts patients and prevents medical teams from doing their best. Importantly, inequity and disparities result in massive gaps in care delivery.

Psychological and cultural evolutionary accounts of human sociality propose that beliefs in punitive and monitoring gods that care about moral norms facilitate cooperation. While there is some evidence to suggest that belief in supernatural punishment and monitoring generally induce cooperative behaviour, the effect of a deity's moral concerns on cooperation remains unclear. Here, we report a pre-registered set of analyses to assess whether perceiving a locally relevant deity as moralistic predicts cooperative play in two permutations of two economic games using data from up to 15 diverse field sites.

Divisions of Clinical Research & Family Planning, Department of Obstetrics and Gynecology, Washington University in St. Louis School of Medicine, St. Louis, MO, United States; Division of Family Planning, Department of Obstetrics, Gynecology and Reproductive Sciences, Yale School of Medicine, New Haven, CT, United States.

Article Synopsis
  • This study evaluated the quality of contraceptive counseling between telehealth and in-person visits at a Title X-funded clinic, where patients chose their counseling mode when scheduling.
  • Out of 360 eligible patients, 296 completed surveys, revealing high-quality counseling ratings in both telehealth (75%) and in-person (80%) without significant differences.
  • However, telehealth patients were less likely to have follow-up clinician visits compared to those who attended in-person sessions, suggesting that telephone counseling may be more appealing or accessible for some individuals.
