Institute of Cancer Research and Royal Marsden Hospital NHS Foundation Trust, London, UK; MRC Clinical Trials Unit, Institute of Clinical Trials and Methodology, University College London, London, UK.

Enhancement of the participant experience in quality of life (QOL) research is imperative to improve recruitment and ongoing engagement in QOL studies. Implementation of recommendations made by the patient and public involvement representatives for STAMPEDE2 could optimise the impact of QOL studies, with a benefit for participants and collection of invaluable data for cancer care research.

Article Synopsis
  • A study was conducted to evaluate the use of oral ethanol as an alternative to benzodiazepines for managing alcohol withdrawal syndrome in patients within a UK health service setting.
  • The research included a retrospective review of patients treated with either ethanol or benzodiazepines, focusing on those at high risk for severe withdrawal symptoms such as delirium tremens.
  • Results indicated that patients managed with ethanol had a significantly lower likelihood of hospital admission, suggesting that oral ethanol could be a viable treatment option during alcohol withdrawal management.

Successfully completing the Institute of Biomedical Science (IBMS) registration portfolio is essential to becoming a Health and Care Professions Council (HCPC) registered Biomedical Scientist. In the West Midlands, a unique collaboration between four universities (Aston, Wolverhampton, Coventry, and Keele) and local NHS Trusts supports student placements and portfolio development. The universities support Training Officers in delivering components of the registration portfolio through the delivery of eight combined placement workshops.

Article Synopsis
  • Three main surgical approaches to the forearm (Henry's, Thompson's, ulnar) carry risks of nerve injury, affecting hand function.
  • A review of existing literature revealed specific "safe zones" for incisions that minimize nerve damage: radial to the flexor carpi radialis for the anterior approach, over Lister's tubercle for Thompson's, and on the ulnar side near the ulnar styloid for the ulnar approach.
  • Surgeons must be aware of anatomical variations and the branching of nerves to prevent intraoperative injuries, emphasizing the importance of following established surgical techniques.

Background: The multicentre European Bifurcation Club Trial (EBC TWO) showed no significant differences in 12-month clinical outcomes between patients randomised to a provisional stenting strategy or systematic culotte stenting in non-left main true bifurcations.

Aims: This study aimed to investigate the 5-year clinical results of the EBC TWO Trial.

Methods: A total of 200 patients undergoing stent implantation for non-left main bifurcation lesions were recruited into EBC TWO.

Article Synopsis
  • Adult spontaneous hypoglycaemia is rare but important to recognize for treatment and understanding its causes.
  • Whipple's triad helps to confirm hypoglycaemia, consisting of low blood glucose, neuroglycopaenia symptoms, and symptom resolution with normal blood glucose levels.
  • Investigations involve checking blood samples during symptoms and can identify specific types of hypoglycaemia while avoiding misdiagnoses in healthy individuals.

Introduction: Unicompartmental knee arthroplasty (UKA) is performed with yearly rate of 9% in UK, and <8% in USA. It has been shown to be a successful and less invasive alternative to complete knee arthroplasty in certain patients total knee arthroplasty (TKA). Fracture of the femoral component after (UKA) has never been reported in the literature.

Comminuted intra-articular fractures are among the most difficult to fix, with open reduction and internal fixation often being impossible. We report the case of a 15-year-old male who required an open reduction with external fixation after sustaining an extremely comminuted intra-articular fifth metacarpal head fracture of the right hand. The patient presented with swelling localised to the fourth and fifth dorsal metacarpals of the right hand, with radiographs demonstrating an intra-articular fracture with comminution and articular surface depression.

Tradition, leadership and innovation in public health education. Commemorating the 100th anniversary of ESPM.

Health is a global concern. No one country or health system can tackle public health problems on their own. Climate change, enironmental degradation, conflict, human trafficking and migration, substance addictions, the uncontrolled corporate health damage caused by multinational corporations, economic catastrophe, global market collapse- all these require concerted public health action from local to global level.

The objective of this study was to assess the diagnostic value of plasma neutrophil gelatinase-associated lipocalin (pNGAL) for the early diagnosis of acute kidney injury (AKI) in adult patients following cardiac surgery requiring cardiopulmonary bypass (CPB). Electronic databases and other resources were systematically searched for relevant studies. Risk of bias was assessed using the Quality Assessment for Diagnostic Accuracy Studies 2 (QUADAS-2) tool.

Unlabelled: Children recover faster than adults in repeated sprints, but it is unclear if their aerobic responses differ.

Purpose: This study tested the hypothesis that aerobic response (VO) during repeated sprints is greater in preadolescent boys than in men. Further, this study compared normalization with conventional ratio-scaling and scaling with the use of body mass (BM) as a covariate.

A Noncontrast CMR Risk Score for Long-Term Risk Stratification in Reperfused ST-Segment Elevation Myocardial Infarction.

JACC Cardiovasc Imaging

March 2022

British Heart Foundation Glasgow Cardiovascular Research Centre, University of Glasgow, Glasgow, Scotland; West of Scotland Heart and Lung Centre, Golden Jubilee National Hospital, Clydebank, Scotland. Electronic address:

Objectives: This study compared the prognostic value of a noncontrast CMR risk score for the composite of all-cause death, nonfatal myocardial infarction, and new congestive heart failure.

Background: A cardiovascular magnetic resonance (CMR) risk score including left ventricular ejection fraction (LVEF), myocardial infarct (MI) size, and microvascular obstruction (MVO) was recently proposed to risk-stratify patients with ST-segment elevation myocardial infarction (STEMI).

Methods: The Eitel CMR risk score and GRACE (Global Registry of Acute Coronary Events) score were used as a reference (Score 1: acute MI size ≥19% LV, LVEF ≤47%, MVO >1.

Comparative Evaluation of Two NGAL Automated Immunoassays in Urine and Plasma.

J Appl Lab Med

May 2022

Department of Blood Sciences, Royal Wolverhampton NHS Trust, New Cross Hospital, Wolverhampton, West Midlands, UK.

Background: Acute kidney injury (AKI), a frequent and serious complication of hospitalized patients, is associated with increased mortality and morbidity. Neutrophil gelatinase-associated lipocalin (NGAL) is a biomarker for the early identification of AKI. We report a comparative laboratory verification of the Abbott Diagnostics (ARCHITECT® urine NGAL) and BioPorto Diagnostics (NGAL TestTM) assays including an assessment of the Abbott assay's performance in EDTA plasma.

Glucocorticoid-induced osteoporosis (GIO) is one of the most common secondary forms of osteoporosis. GIO is partially due to the apoptosis of osteoblasts and osteocytes. In addition, high doses of dexamethasone (DEX), a synthetic glucocorticoid receptor agonist, induces neurodegeneration by initiating inflammatory processes leading to neural apoptosis.

Neurotherapeutics for Attention Deficit/Hyperactivity Disorder (ADHD): A Review.

Cells

August 2021

Department of Child and Adolescent Psychiatry and Psychotherapy, Medical Faculty Mannheim, Central Institute of Mental Health, Heidelberg University, 68159 Mannheim, Germany.

This review focuses on the evidence for neurotherapeutics for attention deficit/hyperactivity disorder (ADHD). EEG-neurofeedback has been tested for about 45 years, with the latest meta-analyses of randomised controlled trials (RCT) showing small/medium effects compared to non-active controls only. Three small studies piloted neurofeedback of frontal activations in ADHD using functional magnetic resonance imaging or near-infrared spectroscopy, finding no superior effects over control conditions.
