Background: Day-case laparoscopic cholecystectomy (DCLC) has gained traction globally due to its benefits, including shorter hospital stays, reduced costs, and enhanced patient experience. While concerns about patient safety, particularly related to bleeding and bile duct injury persist, the literature supports the efficacy and advantages of DCLC highlighting the need for its wider adoption in healthcare settings to optimise resources and improve patient outcomes.

Methods: This was a literature review that aims to assess the feasibility and safety of day-case laparoscopic cholecystectomy for symptomatic gallstone patients, focusing on incidence and aetiology of unexpected admissions and readmissions, as well as conversion-to-open rates.

  • Cardiovascular events are a major cause of death in men with advanced prostate cancer, and new treatments like androgen receptor signaling inhibitors (ARSI) may have unknown cardiovascular side effects.
  • The study aims to evaluate how the addition of ARSI affects the incidence of cardiovascular events in patients with locally advanced and metastatic prostate cancer.
  • A systematic review of 24 studies found that ARSI therapy significantly increases the risk of cardiovascular events, with a risk ratio indicating a higher incidence of both all-grade and severe cardiovascular issues.

Article Synopsis
  • Inflammation of the vas deferens, known as vasitis, can either be acute and painful or asymptomatic, with acute cases often requiring imaging for accurate diagnosis.
  • A male patient in his 40s presented with right groin pain, initially suspected to be an incarcerated inguinal hernia, but a CT scan revealed he actually had acute vasitis instead.
  • This case highlights the importance of considering various diagnoses for acute groin pain and emphasizes that imaging can prevent unnecessary surgical procedures.

Article Synopsis
  • Current research indicates that axial skeleton magnetic resonance imaging (AS-MRI) is more effective than traditional bone scintigraphy (BS) for identifying bone metastases in high-risk prostate cancer patients, though BS remains common.
  • A study reviewing 503 high-risk prostate cancer patients showed that AS-MRI detected bone metastases in 17.5% of cases, significantly differing in PSA and clinical variables compared to those without metastases.
  • The study suggests that AS-MRI not only performs as well as or better than BS for detection, but also offers substantial cost savings for the healthcare system, with all patients receiving scans promptly within 14 days.

Background: Gastroesophageal reflux disease (GERD) remains one of the most commonly encountered gastrointestinal disorders. Proton pump inhibitors still show an inadequate effect on about 10% to 40% of the patients. Laparoscopic antireflux surgery is the surgical alternative for managing GERD in patients who are not responding to proton pump inhibitors.

Complex abdominal wall reconstruction is an emerging subspecialty yet, despite the abundance of abdominal wall hernias requiring treatment and the increasing complexity of this type of surgery, there are few opportunities for surgeons to gain subspecialist training in this field. In this paper we discuss the need for focused training in complex abdominal wall reconstruction, outline some of the problems that may be hindering the availability of such opportunities and propose potential solutions to these issues.

Background: Antibiotic impregnated cement coated nails have been described in literature with promising success rates. However, they need removal at a later stage. This study focuses on use of a PLA-precoated antibiotic nail that stops eluting antibiotic after a while and can be retained later as it behaves as a standard interlocking nail in the long run.

Recent years have seen considerable increases in both the demand for, and complexity of, ventral hernia repairs. This has led to calls for abdominal wall surgery to become a recognized subspecialty in the United States and Europe, with some centres responding by forming specialized, multidisciplinary teams for abdominal wall reconstruction. At present, however, no Canadian city has followed suit.

Article Synopsis
  • The study reviews existing literature on antifungal prophylaxis during penile prosthesis (PP) surgery to evaluate its effectiveness in reducing post-surgery fungal infections.
  • A systematic search of databases revealed nine studies showing that fungal infections accounted for 11.1% of all PP infections, with higher risks in patients with diabetes, obesity, and in warmer climates.
  • Current guidelines from the American and European urology associations do not include antifungal treatment, but combining antifungals with antibiotics could lower infection rates, suggesting that broader use of antifungal prophylaxis might be beneficial.
