Background: The concept of the metaverse is a virtual world that immerses users, allowing them to interact with the digital environment. Due to metaverse's utility in collaborative and immersive simulation, it can be advantageous for medical education in high-stakes care settings such as emergency, critical, and acute care. Consequently, there has been a growth in educational metaverse use, which has yet to be characterized alongside other simulation modalities literature.

Introduction: Measurements of health-related quality of life (HRQoL) are important for capturing disease impact beyond physical health and relative to other diseases but have rarely been assessed in primary progressive aphasia (PPA).

Methods: HRQoL was characterized overall, by sex and subtype in PPA ( = 118) using the Health Utilities Index-2/3 (HUI2/3). Multiple linear regression assessed associations between HRQoL and language severity.

The use of a rapid, easy-to-use intervention could improve needle-related procedural pain management practices in the context of the Emergency Department (ED). As such, the Buzzy device seems to be a promising alternative to topical anesthetics. The aim of this study was to determine if a cold vibrating device was non-inferior to a topical anesthetic cream for pain management in children undergoing needle-related procedures in the ED.

Article Synopsis
  • This study examined how phenylephrine (PE) affects both brain and skeletal muscle microcirculation in rats using a combination of hyperspectral near-infrared spectroscopy and diffuse correlation spectroscopy.
  • Results showed that PE caused a decrease in oxygenation and blood flow in skeletal muscle while increasing cerebral oxygenation and perfusion initially, despite a later drop in brain blood flow.
  • The findings suggest that while PE induces vasoconstriction in skeletal muscles, it can redirect and enhance blood flow to the brain, indicating a complex interplay between peripheral and cerebral circulation.

Peptides that increase pro-reparative responses to injury and disease by modulating the functional organization of hyaluronan (HA) with its cell surface binding proteins (e.g., soluble receptor for hyaluronan-mediated motility [RHAMM] and integral membrane CD44) have potential therapeutic value.

Article Synopsis
  • This study systematically reviewed the impact of postoperative computed tomography imaging factors on outcomes following acute type A aortic dissection repair, focusing on measurements like aortic diameter and growth rates.
  • A total of 68 studies involving 7,885 patients were analyzed, revealing that extended repairs led to better false lumen thrombosis and reduced reintervention rates, but did not significantly improve survival rates.
  • Key prognostic indicators identified for improved outcomes include complete false lumen thrombosis and stabilization of aortic diameter, with aortic growth rates particularly significant in the descending thoracic region; future research should further explore these factors for better patient management.

Objective: Health care is a significant contributor to the climate crisis. Multidisciplinary clinics (MDC) may reduce carbon emissions by combining multiple appointments into one. This is the first program evaluation study to quantify the carbon footprint associated with multidisciplinary pediatric airway clinics.

Background: The current study sought to examine whether psycholinguistic assessments could discriminate children and adolescents with developmental language disorder (DLD) from those with attention-deficit/hyperactivity disorder (ADHD; combined or inattentive subtype) and comorbid DLD + ADHD.

Methods: The Clinical Evaluation of Language Fundamentals-Screening Test (CELFST; Wiig et al., 2013), the Comprehensive Test of Phonological Processing ( subtest; Wagner et al.

Article Synopsis
  • The study aimed to assess if lung ultrasound (LUS) scores could predict clinical outcomes in emergency department patients with suspected COVID-19.
  • Researchers conducted a multicenter study comparing various LUS scoring protocols on 129 patients and followed their outcomes, including intubation and ICU admission.
  • The results showed no significant correlation between LUS scores and negative clinical events, suggesting potential changes in disease dynamics over time and possible limitations in the study's capacity to find weaker associations.

Background: Among youth, symptoms of depression, anxiety, and alcohol use are associated with considerable illness and disability. Youth face many personal and health system barriers in accessing mental health care. Mobile applications (apps) offer youth potentially accessible, scalable, and anonymous therapy and other support.

An unusual case of pleomorphic adenoma of the nasolacrimal duct.

Clin Case Rep

February 2024

Schulich School of Medicine & Dentistry Lawson Health Research Institute, Western University London Ontario Canada.

Key Clinical Message: Pleomorphic adenoma is a common epithelial tumor but is unusual to involve the nasolacrimal duct and lacrimal sac. Current reported cases are sparse but may be helpful for delineating patterns of malignant transformation in the future.

Abstract: This is a report of a 66-year-old patient with a pleomorphic adenoma involving the nasolacrimal duct and lacrimal sac, which is an unusual location for these tumors.

Long-term patient and graft survival has been achieved in organ transplantation but at the expense of toxic side effects that are associated with long-term use of nonspecific immunosuppressive drugs. Discovering new regulators of dendritic cells is the key for development of an ideal treatment to prevent immune rejection. We hypothesized that knockdown of circMAP2K2 induces immunosuppressive DCs and that treatment with circMAP2K2 silenced-DCs can prevent alloimmune rejection.

Prevention of Falls in Parkinson's Disease: Guidelines and Gaps.

Mov Disord Clin Pract

October 2023

Department of Rehabilitation Sciences, Neurorehabilitation Research Group (eNRGy) KU Leuven Leuven Belgium.

Background: People living with Parkinson's disease (PD) have a high risk for falls.

Objective: To examine gaps in falls prevention targeting people with PD as part of the Task Force on Global Guidelines for Falls in Older Adults.

Methods: A Delphi consensus process was used to identify specific recommendations for falls in PD.

Non-immune hydrops is a prenatal finding which can occur due to an underlying genetic diagnosis such as common chromosomal aneuploidy (Trisomy 21, Turner syndrome etc.). It is extremely rare to have more than one genetic cause of hydrops fetalis in a single pregnancy.

We present a patient with cri-du-chat syndrome secondary to a rare cytogenetic mechanism. Our patient was the product of a dichorionic diamniotic twin pregnancy initially flagged with soft markers on ultrasound and uninformative single-nucleotide polymorphism (SNP)-based noninvasive prenatal testing (NIPT) for chromosome 18. Subsequent NIPT using proprietary-targeted amplification methodology returned low risk for chromosomal aneuploidies 13, 18, and 21.

Background And Aims: Quetiapine is an atypical antipsychotic predominantly metabolized by the cytochrome P450 3A4 (CYP3A4) enzyme. We studied the risk of adverse events following coprescription of clarithromycin (a strong CYP3A4 inhibitor) versus azithromycin (not a CYP3A4 inhibitor) in quetiapine users.

Materials And Methods: This was a population-based retrospective cohort study from 2004 to 2020 in Ontario, Canada in adult quetiapine users newly co-prescribed clarithromycin ( = 16,909) or azithromycin ( = 25,267).

Key Clinical Message: Multiple genetic disorders can coexist in one patient. When the phenotype is not fully explained with one diagnosis, it is recommended to perform further genetic investigations in search for coexisting second diagnosis.

Abstract: Craniofrontonasal dysplasia (CFND) (MIM: 304110) is an X-linked dominant disorder that shows paradoxically greater severity in heterozygous females than in hemizygous males.

Purpose: The electronic health record (EHR) is frequently identified as a source of assessment data regarding residents' clinical performance. To better understand how to harness EHR data for education purposes, the authors developed and authenticated a prototype resident report card. This report card used EHR data exclusively and was authenticated with various stakeholders to understand individuals' reactions to and interpretations of EHR data when presented in this way.
