Article Synopsis
  • Neonates with symptomatic tetralogy of Fallot (sTOF) often require early surgical intervention, which can involve either staged repair (SR) or primary repair (PR), but both strategies expose them to low-dose ionizing radiation that may raise cancer risk.
  • This study analyzed the cumulative radiation exposure and lifetime attributable risk (LAR) of cancer in sTOF patients who underwent either SR or PR, using data from 242 neonates across multiple centers.
  • Results showed that SR resulted in significantly higher radiation exposure compared to PR and highlighted an increased LAR of cancer, particularly for females, emphasizing the need to consider radiation risk when choosing treatment options for this vulnerable group.*

Article Synopsis
  • The study aimed to compare the outcomes of neonates with symptomatic tetralogy of Fallot (TOF) based on whether they had a ductus arteriosus (DA) or absent DA (ADA).
  • Researchers conducted a retrospective multi-center study involving 519 neonates to assess the risks of death and need for reintervention, focusing on ADA and critically deficient pulmonary blood flow (CDPBF).
  • Findings indicated that neonates with ADA faced a significantly higher mortality risk compared to those with DA, highlighting the rare but critical nature of CDPBF among these patients.

Background: Infants with hypoplastic left heart syndrome (HLHS) or a variant are at risk of ventricular dysfunction (VD) and atrioventricular valve regurgitation (AVVR) prior to superior cavopulmonary connection (SCPC). Although the impact of these complications in isolation has been described, their effect in combination on attrition is poorly defined.

Methods: A retrospective observational study of patients with HLHS or variants undergoing a Norwood procedure between 2008 and 2020 at a single center was performed.

Background: Bacterial lymphadenitis is a common reason for antibiotic treatment and hospitalization in children. The literature available addressing the bacterial etiology of lymphadenitis recommends the use of narrow-spectrum agents to cover common pathogens. We suspect that patients at our institution receive unnecessarily broad-spectrum antimicrobial agents.

Telegenetics has shifted some genetic testing performance to the patient's own home, with the patient collecting his/her own sample. Little is known regarding the rate of test completion of such home-based genetic testing. This study compared the completion rate of home-based genetic tests before and after a reminder system was implemented.

The evolving breadth and complexity of the contemporary pediatric cardiology specialty requires regular, systematic analysis of the practice to ensure that training and certification requirements address the demands of real-world clinical experience. We report the process of the American Board of Pediatrics (ABP) for conducting such a practice analysis and revising the test content outline (TCO) for the pediatric cardiology subspecialty certification exam. A panel of 15 pediatric cardiologists conducted seven 2-h virtual meetings, during which they identified 37 unique tasks that represent the work a pediatric cardiologist may reasonably expect to perform within the first 5 years after training.

Objectives: In May 2016, the American Academy of Pediatrics published a clinical practice guideline (CPG) defining apparent life-threatening events (ALTEs) as brief resolved unexplained events (BRUEs) and recommending risk-based management. We analyzed the association of CPG publication on admission rate, diagnostic testing, treatment, cost, length of stay (LOS), and revisits in patients with BRUE.

Methods: Using the Pediatric Health Information Systems database, we studied patients discharged from the hospital with a diagnosis of ALTE/BRUE from January 2012 to December 2019.
