Popliteal Artery Entrapment Syndrome-Return to Sport Considerations: A Case Report.

PM R

April 2019

Spaulding Rehabilitation Hospital/Harvard Medical School, Department of Physical Medicine and Rehabilitation, Spaulding Wellesley Outpatient Care Center, Newton Wellesley Hospital.

This is a case of a 19-year-old female collegiate field hockey player with left leg pain, numbness, and tingling. A comprehensive workup including dynamic ultrasound, magnetic resonance imaging (MRI), and magnetic resonance angiogram (MRA) revealed a type VI functional popliteal artery entrapment syndrome (PAES). There are many options for treatment of functional PAES including activity modification, botulinum toxin injection, and surgery.

Effect of Fixation Type and Bone Graft on Tarsometatarsal Fusion.

Foot Ankle Int

December 2018

3 Massachusetts General Hospital, Newton-Wellesley Hospital, Harvard Medical School, Boston, MA, USA.

Background:: End-stage tarsometatarsal (TMT) arthritis is commonly treated with arthrodesis of involved joints. Fixation hardware can consist of varying combinations of screws, plates, and staples with or without supplemental bone graft. There are limited data to demonstrate either superiority of a given fixation method or the impact of bone graft on fusion rates.

Low-Dose Computed Tomography Colonography Technique.

Radiol Clin North Am

September 2018

Department of Radiology, Montefiore Medical Center, Albert Einstein College of Medicine, 111 East 210th Street, Bronx, NY 10467, USA.

Significant anxiety has been expressed by some over the radiation risks associated with computed tomography (CT), particularly when it applies to a screening examination such as CT colonography. These theoretic risks are far outweighed by the significant benefits colorectal cancer screening offers. Regardless of how significant the theoretic risk of CT radiation is in the older population, the ALARA principle maintains that radiation dose should be reduced to As Low As Reasonably Achievable.

Aim: To investigate the role of SDH2 in Candida albicans filamentation and virulence.

Materials & Methods: Caenorhabditis elegans and mouse candidiasis models were used to assess the virulence of a sdh2Δ/Δ mutant. Various hypha-inducing media were used to evaluate the hyphal development of C.

Amphetamine and its related derivatives and analogues (ADRA) are highly addictive central nervous system stimulants that are used commonly in the treatment of attention-deficit/hyperactivity disorder and narcolepsy. These medications are associated with many side effects but reports of peripheral arterial manifestations associated with ADRA usage are scarce. We retrospectively reviewed the records of 16 patients (median age 37 years (IQR 31-47), 13 females) referred to a single tertiary referral service while receiving ADRA.

Contemporary Management and Outcomes of Patients with Massive and Submassive Pulmonary Embolism.

Am J Med

December 2018

Harvard Medical School, Boston; Division of Cardiology, The Fireman Vascular Center, Department of Medicine. Electronic address:

Background: Few contemporary studies have assessed the management and outcomes of patients with massive and submassive pulmonary embolism. Given advances in therapy, we report contemporary practice patterns and event rates among these patients.

Methods: We analyzed a prospective database of patients with massive and submassive pulmonary embolism.

Purpose: To evaluate the effect of ACL deficiency on the in vivo changes in end-to-end distances and to determine appropriate graft fixation angles for commonly used tunnel positions in contemporary ACL reconstruction techniques.

Methods: Twenty-one patients with unilateral ACL-deficient and intact contralateral knees were included. Each knee was studied using a combined magnetic resonance and dual fluoroscopic imaging technique while the patients performed a dynamic step-up motion (~50° of flexion to extension).

Objectives: Many septic patients receive care that fails the Centers for Medicare and Medicaid Services' SEP-1 measure, but it is unclear whether this reflects meaningful lapses in care, differences in clinical characteristics, or excessive rigidity of the "all-or-nothing" measure. We compared outcomes in cases that passed versus failed SEP-1 during the first 2 years after the measure was implemented.

Design: Retrospective cohort study.

Purpose: The objective of this paper was to compare the reoperation rates, timing and causes between decompression alone and decompression plus fusion surgeries for degenerative lumbar diseases through a systematic review of the published data.

Methods: A search of the literature was conducted on PubMed/MEDLINE, EMBASE and the Cochrane Collaboration Library. Reports that included reoperations after decompression alone and/or decompression plus fusion surgeries were selected using designed eligibility criteria.

Two-Stage Revision Arthroplasty for the Treatment of Prosthetic Joint Infection.

Curr Rev Musculoskelet Med

September 2018

Department of Orthopaedic Surgery, Massachusetts General Hospital, Newton Wellesley Hospital, Harvard Medical School, 55 Fruit Street, Boston, MA, 02114, USA.

Purpose Of Review: To review the diagnosis and treatment of prosthetic joint infection (PJI) with a focus on two-stage revision arthroplasty. The text will discuss different spacer constructs in total knee and total hip arthroplasty and will present clinical outcome data for these various options.

Recent Findings: There is no appreciable difference in infection eradication between mobile and static antibiotic spacers.

Investigation of the development of cartilage degeneration after ACL reconstruction is important for improving current surgical treatment of ACL injuries to prevent long-term knee joint degeneration. This pilot study examined the relationship between the changes in weight-bearing knee contact kinematics 6 months after ACL reconstruction and the biochemical composition changes in the knee cartilage measured using T2 relaxation values 3 years after the surgery in seven patients. The analysis indicated that the change of the knee contact kinematics in short-term after ACL reconstruction is associated with an increase of T2 values of the cartilage in longer follow up times.

Background: Venous thromboembolism (VTE) is the third leading cause of vascular disease and accounts for $10 billion in annual US healthcare costs. The nationwide burden of 30-day readmissions after such events has not been comprehensively assessed.

Methods And Results: We analyzed adults ≥18 years of age with hospitalizations associated with acute VTE between January 1, 2010, and December 31, 2014, in the Nationwide Readmissions Database.

Background: High levels of sedentary behavior are linked to increased mortality. In the United States, individuals spend 55-70% of their waking day being sedentary. Since most individuals spend large portions of their daily lives at work, quantifying the time engaged in sedentary behavior at work is emerging as an important health determinant.

Background: Although 2-stage exchange arthroplasty is the most effective treatment among available strategies for managing chronic periprosthetic joint infection (PJI), rates of its success vary greatly. The purpose of our study was to examine whether objective measurements collected at the time of the diagnosis of PJI could be used to identify patients at risk of failure of 2-stage exchange.

Methods: We identified 205 patients across 4 institutions who underwent 2-stage exchange arthroplasty for the treatment of PJI following total hip or total knee arthroplasty.

This study describes a simulation of diagnostic coding using an EHR. Twenty-three ambulatory clinicians were asked to enter appropriate codes for six standardized scenarios with two different EHRs. Their interactions with the query interface were analyzed for patterns and variations in search strategies and the resulting sets of entered codes for accuracy and completeness.

Background: Controversy persists as to whether Lisfranc injuries are best treated with open reduction internal fixation (ORIF) versus primary arthrodesis (PA). Reoperation rates certainly influence this debate, but prior studies are often confounded by inclusion of hardware removal as a complication rather than as a planned, staged procedure inherent to ORIF. The primary aim of this study was to evaluate whether reoperation rates, excluding planned hardware removal, differ between ORIF and PA.
