Background: Transcatheter Aortic Valve Implantation (TAVI) and Surgical Aortic Valve Replacement (SAVR) have different levels of invasiveness which can result in different levels of functional status after the procedure.

Methods: We performed a systematic review and meta-analysis to detect studies showing direct comparison between TAVI and SAVR regarding postoperative functional status. The primary endpoint was the incidence of postoperative delirium (POD) after TAVI or SAVR, assessed using the Confusion Assessment Method (CAM).

Update on MR Imaging of Hip Arthroplasty.

Magn Reson Imaging Clin N Am

February 2025

Department of Radiology and Imaging, Hospital for Special Surgery, 535 East 70th Street, New York, NY 10021, USA; Weill Medical College of Cornell University, New York, USA; HSS MRI Laboratory, New York, USA. Electronic address:

MR imaging of hip arthroplasty can be technically challenging due to artifacts associated with metallic implant components; however, utilization of specific parameter modifications and advanced metal reduction sequences allows acquisition of high-quality diagnostic images in patients with metallic implants. Given the increasing prevalence of hip arthroplasties within the population, familiarity with appropriate imaging techniques and expected pathology within this patient population is important knowledge for radiologists who encounter patients with hip arthroplasty in clinical practice.

MYB fusions are recurrently found in select cancers, including blastic plasmacytoid DC neoplasm (BPDCN), an acute leukemia with poor prognosis. They are markedly enriched in BPDCN compared with other blood cancers and, in some patients, are the only obvious somatic mutation detected. This suggests that they may alone be sufficient to drive DC transformation.

The expansion of neural progenitors and production of distinct neurons are crucial for architectural assembly and formation of connectivity in human brains. Subplate neurons (SPNs) are among the firstborn neurons in the human fetal cerebral cortex, and play a critical role in establishing intra- and extracortical connections. However, little is known about SPN origin and developmental lineages.

Retinocytomas are benign tumors that arise from mutations in the  gene. Previous research describes the appearance of retinocytomas as that of treated retinoblastoma (Rb) lesions, with characteristics such as chorioretinal atrophy, calcification, and a lack of necrosis or mitotic activity on histopathology. We present the unusual case of an asymptomatic seven-year-old girl with two independent translucent masses in the peripheral retina of the right eye (OD) and extensive intraretinal tumor and vitreous seeds.

Round table discussion. The management of proximal scaphoid nonunion.

J Hand Surg Eur Vol

November 2024

Hand and Upper Extremity Service, Hospital for Special Surgery, Weill Medical College of Cornell University, New York, NY, USA.

The management of scaphoid nonunion, although relatively common, continues to pose challenges, especially if complicated by avascular necrosis of the proximal pole, humpback or dorsal intercalated segment instability deformities, and early scaphoid nonunion advanced collapse. In this round table article, four surgeons were asked to answer key questions on a clinical example of a proximal scaphoid nonunion to outline their approach and rationale in their preferred treatment choices. The current panel of experts leans towards arthroscopic methods of treatment and most agree that arthroscopic bone grafting has become an important and revolutionary treatment option, which has yielded excellent results.

Article Synopsis
  • Surgical evacuation of intracerebral hemorrhage (ICH) performed early can lead to better functional outcomes, but doing it too quickly may cause significant risks like postoperative rebleeding.
  • The study developed a 5-point grading scale to assess the severity of intraoperative bleeding during minimally invasive endoscopic techniques for ICH evacuation.
  • Among 142 patients evaluated, results indicated that while early evacuation is associated with increased bleeding scores, it did not lead to worse long-term outcomes or increased postoperative rebleeding.

Double-Anchor Stapled Repair of the Medial Meniscus Posterior Root.

Arthrosc Tech

October 2024

Orthopaedic Sports Medicine and Trauma Service, Department of Orthopaedic Surgery, Hospital for Special Surgery, NewYork-Presbyterian, Weill Medical College of Cornell, University, New York, New York, U.S.A.

There are various treatment options for medial meniscus posterior root tears, such as conservative management, meniscectomy, pull-out repair, and suture anchor repair. However, the ultimate repair technique for optimal meniscal healing remains a topic of discussion, as each technique has its own set of risks and pitfalls. This technique provides a stable and straightforward approach that minimizes fixation-related concerns.

Killian-Jamieson diverticulum (KJD) is a rare type of esophageal diverticulum that occurs in a specific area called the Killian-Jamieson space. Although surgery has been the conventional treatment for symptomatic KJD, minimally invasive endoscopic techniques, particularly Killian-Jamieson per-oral endoscopic myotomy (KJ-POEM), have emerged as an alternative. The aim of the study was to evaluate the effectiveness and safety of KJ-POEM in treating KJD.

Article Synopsis
  • Advanced biliary tract cancer (BTC) patients may benefit from fluoropyrimidine-based chemotherapy, but no direct comparisons between different regimens had been previously conducted.
  • An individual patient-level meta-analysis was performed using data from two clinical trials to evaluate overall survival (OS) across different chemotherapy regimens, including nal-IRI plus 5-FU/LV, mFOLFOX, and mFOLFIRI.
  • Results showed that nal-IRI plus 5-FU/LV significantly improved OS compared to both mFOLFOX and mFOLFIRI, with notable side effects differing among regimens; thus, considering both effectiveness and safety is essential in treatment decisions.

Adipocytes store energy as triglycerides, while mobilizing energy when needed via lipolysis. Triglyceride lipolysis releases fatty acids and glycerol into the circulation to fuel other tissues. However, a significant fraction of fatty acids released by lipolysis are retained within the white adipose tissue and handled by adipocytes.

An effective human immunodeficiency virus 1 (HIV-1) vaccine will most likely have to elicit broadly neutralizing antibodies (bNAbs) to overcome the sequence diversity of the envelope glycoprotein (Env). So far, stabilized versions of Env, such as SOSIP trimers, have been able to induce neutralizing antibody (NAb) responses, but those responses are mainly strain-specific. Here we attempted to broaden NAb responses by using a multivalent vaccine and applying a number of design improvements.

Med Clin North Am

November 2024

Division of Hospital Medicine, Department of Internal Medicine, University of Nebraska Medical Center, 986435 Nebraska Medical Center, Omaha, NE 68198-6435, USA.

Preoperative medical evaluation can minimize inefficiencies and improve outcomes. Thoughtful use of preoperative testing can aid in that effort, but, conversely, indiscriminate testing can detract from it. The United Kingdom National Institute for Health Care and Excellence, European Society of Anaesthesiology, and American Society of Anesthesiologists (ASA) have all stated that routine preoperative testing is not supported by evidence.
