We present three patients with respiratory failure in whom conventional mechanical lung ventilation resulted in unacceptably high levels of carbon dioxide, severe acidosis and high vasopressor requirements. A pumpless arteriovenous extracorporeal carbon dioxide removal device (Novalung) was inserted. Arterial carbon dioxide levels were reduced rapidly with a corresponding increase in pH, reduction in vasopressor requirements and reduction in inspiratory pressures.

We report the case of an intra-abdominal sclerosing epithelioid fibrosarcoma, which to best of the authors' knowledge has not been described previously. The patient discussed here developed lung metastases but is still alive 1-year post-diagnosis.

Case Report: A 19-year-old woman presented to our department following accidental ingestion of a teaspoon. On further questioning, she admitted to a habit of binge-eating followed by self-induced vomiting, in order to avoid weight gain; she blamed this behaviour on a stressful relationship with her partner.
