Minimally invasive surgery, especially robotic surgery, has become increasingly popular in colorectal surgery over the last decade. Robotic-assisted surgery has shown better outcomes than conventional laparoscopic surgery because of superior ergonomics, high-resolution three-dimensional cameras, and articulating instruments; however, issues like the long operative time and cost-effectiveness remain unresolved. This study compares the robotic and laparoscopic approach of abdominoperineal resection (APR) for low rectal cancer to evaluate the superiority of robotic surgery in short-term and oncological outcomes.

Approximately 11% of all new breast cancer cases annually are diagnosed in young women, and this continues to be the leading cause of death in women age 20 to 49. Young, premenopausal breast cancer patients present with more advanced stages and with a higher proportion of aggressive subtypes such as triple negative and HER2-enriched tumors. Recently, the United States Preventive Services Task Force (USPSTF) lowered the age threshold to initiate screening mammograms to age 40 to aid in earlier detection.

Article Synopsis
  • Researchers explored how natural language processing (NLP) could better identify worsening heart failure (WHF) events outside of hospitals compared to traditional diagnostic coding.
  • In their study, 8407 patient encounters were evaluated, and findings indicated that NLP identified a significant number of cases (22%) that met WHF criteria, with dyspnoea being the most frequent diagnosis.
  • The study concludes that relying solely on standard claims-based methods might misclassify WHF events and underestimate their impact in emergency departments and observation stays.

Background: Hospital at Home (H@H) programs-which seek to deliver acute care within a patient's home-have become more prevalent over time. However, existing literature exhibits heterogeneity in program structure, evaluation design, and target population size, making it difficult to draw generalizable conclusions to inform future H@H program design.

Objective: The objective of this work was to develop a quality improvement evaluation strategy for a H@H program-the Kaiser Permanente Advanced Care at Home (KPACAH) program in Northern California-leveraging electronic health record data, chart review, and patient surveys to compare KPACAH patients with inpatients in traditional hospital settings.

Article Synopsis
  • Early-onset preeclampsia can be predicted using a combination of clinical risk factors, single blood pressure measurements, and other biomarkers, but predicting later-onset preeclampsia and gestational hypertension remains challenging.
  • A study involving nearly 250,000 women evaluated the effectiveness of BP patterns and clinical data for predicting different types of preeclampsia and gestational hypertension.
  • The findings indicate that incorporating early pregnancy BP patterns significantly improves the accuracy of risk prediction compared to using just clinical factors alone.

Multiple sclerosis: Exploring the limits and implications of genetic and environmental susceptibility.

PLoS One

June 2023

INSERM, Center for Research in Transplantation and Translational Immunology, UMR 1064, Nantes Université, Nantes, France.

Article Synopsis
  • The study investigates the genetic and environmental factors influencing susceptibility to multiple sclerosis (MS) specifically in the Canadian population.
  • It distinguishes between directly observable epidemiological data (like the prevalence of MS among siblings and sex ratios) and inferred data (like genetic susceptibility) to better understand the disease's dynamics.
  • Using advanced modeling techniques, the research reveals that only a small fraction of the population (≤ 52%) and an even smaller fraction of women (< 32%) are genetically susceptible to MS, suggesting that most individuals are unlikely to develop the disease regardless of environmental factors.

Background: Management of adults with atrial fibrillation (AF) or atrial flutter in the emergency department (ED) includes rate reduction, cardioversion, and stroke prevention. Different approaches to these components of care may lead to variation in frequency of hospitalization and stroke prevention actions, with significant implications for patient experience, cost of care, and risk of complications. Standardization using evidence-based recommendations could reduce variation in management, preventable hospitalizations, and stroke risk.

Background: Cancer patients who exhibit cachexia lose weight and have low treatment tolerance and poor outcomes compared to cancer patients without weight loss. Despite the clear increased risk for patients, diagnosing cachexia still often relies on self-reported weight loss. A reliable biomarker to identify patients with cancer cachexia would be a valuable tool to improve clinical decision making and identification of patients at risk of adverse outcomes.
