The medical community recently endured a heartbreaking tragedy with the brutal rape and murder of a young medical student in Kolkata. The tragic event at R G Kar Medical College in Kolkata is particularly distressing. While the reasons behind it may be complex-whether linked to a drug racket, a personal vendetta, or even the accused's questionable history as a womanizer and porn addict-the fact remains that justice appears elusive.

Fixed-dose Combination of Torsemide and Mineralocorticoid Receptor Antagonists.

J Assoc Physicians India

September 2024

Cluster Medical Head, Medical Affairs, Cipla Ltd., Mumbai, Maharashtra, India.

The combination of torsemide and spironolactone presents a promising approach to managing conditions such as edema and hypertension. Torsemide, a loop diuretic, enhances diuresis by inhibiting sodium reabsorption in the kidneys, while spironolactone, a potassium-sparing diuretic and mineralocorticoid receptor antagonist (MRA), complements this effect by preventing potassium loss and offering additional cardiovascular benefits. This review examines clinical evidence supporting their combined effectiveness in treating fluid retention and improving outcomes in conditions like heart failure (HF).

Torsemide in the Management of Pulmonary Edema.

J Assoc Physicians India

September 2024

Country Head, Medical Affairs, Cipla Ltd., Mumbai, Maharashtra, India.

Article Synopsis
  • - Pulmonary edema occurs when fluid builds up in the lungs' alveoli, and can be classified as either cardiogenic (related to heart issues) or noncardiogenic (not related to heart problems).
  • - Cardiogenic pulmonary edema is often linked to congestive heart failure (CHF) and is typically treated with loop diuretics, with torsemide and furosemide being effective options due to their ability to reduce heart pressure levels.
  • - Torsemide may be preferable over furosemide because it has better absorption and efficacy, showing good results when given intravenously for cardiogenic pulmonary edema, but further studies are necessary to evaluate its effectiveness in non-cardiogenic cases.

Loop Diuretics Unique Mechanism of Action.

J Assoc Physicians India

September 2024

Country Head, Medical Affairs, Cipla Ltd., Mumbai, Maharashtra, India.

Loop diuretics, including torsemide, furosemide, bumetanide, and piretanide, act by inhibiting the sodium-potassium-chloride (Na/K/2Cl) cotransporter in the thick ascending limb of the loop of Henle within the nephron. This mechanism is pivotal in managing fluid retention associated with conditions such as heart failure, cirrhosis, chronic kidney disease, and hypertension. A comprehensive understanding of how these diuretics uniquely target this transporter provides crucial insights into effectively addressing fluid overload across diverse clinical conditions.

An embolized clot that travels to the lungs from the legs or, less commonly, other parts of the body (known as deep vein thrombosis or DVT) causes pulmonary embolism (PE), which is characterized by obstruction of blood flow to the pulmonary artery. As PE has the propensity to masquerade as various illnesses affecting both the cardiovascular (CV) and the respiratory system, it is crucial to identify PE at the earliest. Appropriate diagnosis of PE may lead to earlier treatment and improved patient outcomes.

Introduction: Pneumococcal diseases pose a significant public health concern in India, with substantial morbidity and mortality, with the elderly and those with coexisting medical conditions being most at risk. Pneumococcus was also seen to be one of the main reasons for co-infection, pneumonia and complications in COVID. Current guidelines recommend vaccination for specific adult populations, but there is a lack of uniformity and guidance on risk stratification, prioritisation and optimal timing.

Connecting the Dots: Hydration, Micronutrients, and Immunity.

J Assoc Physicians India

June 2024

Medical Lead, Medical Affairs, JNTL Consumer Health (India) Private Limited, Mumbai, Maharashtra, India.

Dehydration, often unnoticed, can occur in adults due to heat-related illnesses, viral infections, and upper respiratory tract infections, leading to deficits in fluids, electrolytes, and energy (FEE), even without diarrhea. This fluid deficit, in turn, impairs immunity at cellular and molecular levels. Physiologically, the immune system rapidly reacts to threats by adjusting its structural permeability, allowing water and small solutes to be passed, facilitated by proteins like aquaporins (AQPs).

Allergic rhinitis (AR) is considered a trivial disease and is often self-treated with over-the-counter drugs and home remedies. Purpose: Treatment of antibiotic-resistant Gram-positive infections (GPIs), including methicillin-resistant Staphylococcus aureus (MRSA) is becoming increasingly difficult, particularly in patients with multiple co-morbidities who require antibiotics with greater safety and a consistent pharmacokinetic/pharmacodynamic (PK/PD) profile. Such difficult-to-treat GPIs are often associated with poor outcomes, extended hospital stay and increased expenditure. This can be partly attributed to the limited safety and aberrant PK/PD profile of existing anti-MRSA antibiotics.

Monkeypox was a zoonotic infection, first detected in parts of northern Africa in the 1970s. Monkeypoxvirus, the causative agent of monkeypox, is a species of genus Orthopoxvirus and is closely related to long-eradicated smallpox caused by variola virus. Outbreaks in the West (in USA, UK, and Ireland) along with periodic re-emergence of the disease in parts of Africa have generated concern among global health bodies due to the existent deficiency of guidelines for management of the disease.

Apart from the individual diseases, some patients also show overlapping manifestations of asthma and COPD. Nevertheless, the diagnosis of COPD is often delayed due to inaccessibility to spirometry; the prevalence of the asthma COPD overlap phenotype is rather high given the exposure to biomass smoke. Furthermore, the rates of exacerbations are twice as high compared to the patients with either of the diseases.
