Article Synopsis
  • Current prognostic scores for acutely decompensated cirrhosis patients, especially those with acute-on-chronic liver failure, often underestimate mortality risk due to not accounting for systemic inflammation's impact.
  • A study used serum metabolomics from two large European cohorts to identify key metabolites linked to short-term mortality, leading to the creation of two new predictive models.
  • Both models significantly outperformed the existing MELDNa score in predicting death within 28 days of hospital admission, demonstrating better accuracy in patients with acute-on-chronic liver failure.

Background And Aims: Colorectal cancer (CRC) is largely preventable with routine screening and surveillance colonoscopy; however, interval cancers arising from precancerous lesions missed by standard colonoscopy still occur. An increased adenoma detection rate (ADR) has been found to be inversely associated with interval cancers. The G-EYE device includes a reusable balloon integrated at the distal tip of a standard colonoscope, which flattens haustral folds, centralizes the colonoscope's optics, and reduces bowel slippage.

Background: Target Controlled Infusion is a sophisticated tool for providing optimal sedation regimen avoiding under or oversedation in gastrointestinal endoscopy.

Aims: To compare standard moderate sedation vs. non-anaesthesiologist-administered propofol sedation during gastrointestinal endoscopy.

The after-effects of bilingual language production.

Neuropsychologia

January 2014

Center for Brain and Cognition (CBC), Universitat Pompeu Fabra, Barcelona, Spain; Institució Catalana de Recerca i Estudis Avançats (ICREA), Barcelona, Spain.

We explored the temporal course of bilingual language control after-effects to shed light on the scope of language control (local vs. global) and on the way in which language control is implemented (L1 inhibition or L2 over-activation). High-proficient bilinguals named objects across three blocks, first in their L1, then in their L2, and then again in their L1 (and conversely) while event-related brain potentials (ERPs) were recorded.

Unlabelled: BACKGROUND AND STUDY: Combined use of opiates and benzodiazepines often results in delayed discharge after colonoscopy.

Aims: To compare sedation quality of two dosages of patient controlled analgesia remifentanil with one another and with that of a midazolam-meperidine association during colonoscopy.

Methods: Ninety patients undergoing colonoscopy were randomly assigned to three groups.

Background: New therapies are needed for pancreatic cancer.

Objective: To determine the feasibility and safety of a new endoscopic treatment. Secondary endpoints were to determine effects on tumor growth measured with CT scan and to find the overall survival.

Background: Best strategy of sedation/analgesia in gastrointestinal (GI) endoscopy is still debated.

Aims Of The Study: To evaluate sedation and monitoring practice among Italian gastroenterologists and to assess their opinion about non-anaesthesiologist propofol administration.

Methods: A 19-item survey was mailed to all 1192 members of the Italian Society of Digestive Endoscopy (SIED).

Endoscopic retrograde cholangio-pancreatography (ERCP) is one of the most technically challenging procedures in therapeutic endoscopy; difficulties in biliary cannulation and post-ERCP pancreatitis are still significant problems. Deep cannulation of Vater's papilla may fail in up to 5% of cases; selective biliary cannulation reportedly fails in 15-35% of cases, even in experienced hands; repeated and prolonged attempts at cannulation increase the risk of post-procedure pancreatitis. Therefore, cannulation technique plays a pivotal role in successful cannulation and occurrence of post-procedure pancreatitis.
