AAAAI Position Statement on Changing Electronic Health Record Allergy Documentation to "Alerts" to Lead to Easily Understood, Actionable Labels.

J Allergy Clin Immunol Pract

December 2024

Kaiser Permanente Southern California, Department of Research and Evaluation, Pasadena, Calif.

The term "allergy" is inaccurate for the vast majority of the contents in the current allergy fields of electronic health records (EHRs). While EHRs have transformed access to health information and streamlined the delivery of care, their ability to reliably indicate medications, vaccines, or foods that mandate avoidance versus preferences or mild intolerances, is suboptimal. The current systems are reactive instead of being proactive and frequently fail to communicate the appropriate course of action.

Left bundle branch block (LBBB) is frequently associated with structural heart disease, and predicts higher rates of morbidity and mortality. In patients with cardiomyopathy (ejection fraction <35%) and LBBB, current guidelines recommend cardiac resynchronisation therapy (CRT) after 3 months of medical therapy. However, studies have suggested that medical therapy alone would be less effective, and the majority of patients would still need CRT at the end of 3 months.

Numerous conditions and pathologies affect the biliary system, many of which have underlying benign courses. However, these overall benign conditions can predispose the patient to malignant pathologies, often due to malignancy arising from abnormal biliary ducts (such as with cholangiocarcinoma) or due to malignancy arising from end-stage liver disease caused by the biliary condition (such as with hepatocellular carcinoma). While these malignancies can at times be obvious, some pathologies can be very difficult to detect and distinguish from the underlying benign biliary etiology.

Atrioventricular node ablation for atrial fibrillation in the era of conduction system pacing.

Eur Heart J

December 2024

Department of Cardiology, Cardiovascular Research Institute Maastricht (CARIM), Maastricht University Medical Center, Maastricht, The Netherlands.

Article Synopsis
  • Management of persistent atrial fibrillation (AF) remains challenging despite improvements in catheter-based treatments and repeat ablation procedures.
  • Atrioventricular node ablation (AVNA) has regained importance as a treatment option due to advancements in cardiac pacing technology, which address pacing-induced cardiomyopathy concerns.
  • The review will explore the role of permanent pacemaker implantation and AVNA in AF management, highlighting its effects on haemodynamics, benefits of 'pace-and-ablate therapy', and identifying patient groups that might benefit from this approach earlier.

Article Synopsis
  • - The objective of the review was to examine the pharmacology, safety, and effectiveness of BCMA-directed immunotherapies, such as CAR-T therapy and bispecific antibodies, for treating relapsed/refractory multiple myeloma (RRMM).
  • - Evidence from phase III and II clinical trials shows that BCMA-directed therapies like idecabtagene vicleucel and teclistamab significantly improve response rates and progression-free survival compared to standard treatments for RRMM.
  • - Despite their benefits, these therapies pose safety risks (like cytokine release syndrome) and face challenges in patient access due to the need for specialized administration and monitoring facilities.

Orientia tsutsugamushi Ank5 promotes NLRC5 cytoplasmic retention and degradation to inhibit MHC class I expression.

Nat Commun

September 2024

Department of Microbiology and Immunology, Virginia Commonwealth University Medical Center, School of Medicine, Richmond, VA, USA.

Article Synopsis
  • Researchers found that the bacterium Orientia tsutsugamushi uses a protein called Ank5 to disrupt the MHC class I pathway, which is crucial for immune responses.
  • Ank5 binds to and degrades a key protein, NLRC5, that activates MHC class I gene expression, thereby reducing the presentation of these molecules on the cell surface.
  • The study highlights how O. tsutsugamushi enhances its survival by manipulating host immune mechanisms, shedding light on strategies used by intracellular pathogens to evade immune detection.

Background: Proton pump inhibitors (PPIs) are often prescribed in conjunction with nonsteroidal anti-inflammatory drugs after total hip arthroplasty (THA) and total knee arthroplasty (TKA) due to their gastroprotective effects. In animal studies, it has been suggested that PPIs have immunosuppressive effects and impair fracture healing; however, the association between PPI use and adverse events following THA and TKA has not been well-studied.

Methods: An administrative claims database was queried for patients who underwent elective THA from 2010 to 2019.
