Changing practices, procedures, and perceptions: A multi-year person-centered planning initiative in group homes for residents with intellectual and developmental disabilities.

Person-centered planning has been shown to benefit people with disabilities and their quality of life. However, we have little knowledge of how person-centered planning can benefit staff and administration within a group home organization, as well as the extent to which it results in changes to organizational practices and procedures, as well as perceptions of people with disabilities. In this qualitative study, we explored the perspectives of organizational employees, an affiliating behavioral consultant, and residents with intellectual and developmental disabilities, taking into consideration key insights from person-centered planning consultant-coaches, to understand the effects of a person-centered planning initiative on the group home organization.

Purpose: Dynamic eye-tracking paradigms are an engaging and increasingly used method to study social attention in autism. While prior research has focused primarily on younger populations, there is a need for developmentally appropriate tasks for older children.

Methods: This study introduces a novel eye-tracking task designed to assess school-aged children's attention to speakers involved in conversation.

Enhanced myocardial perfusion in late gestation fetal lambs with impaired left ventricular inflow.

Robust preclinical models of asymmetric ventricular loading in late gestation reflecting conditions such as hypoplastic left heart syndrome are lacking. We characterized the morphometry and microvascular function of the hypoplastic left ventricle (LV) and remaining right ventricle (RV) in a sham-controlled late gestation fetal lamb model of impaired left ventricular inflow (ILVI). Singleton fetuses were instrumented at ∼120 days gestational age (dGA; term is ∼147 days) with vascular catheters, an aortic flow probe and a deflated left atrial balloon.

Fire in the tree: The origin and distribution of fire-adapted traits within conifers and their influence on speciation rates across the conifer phylogeny.

Premise: Considering rapidly changing fire regimes due to anthropogenic disturbances to climate and fuel loads, it is crucial to understand the underpinnings driving fire-adapted trait evolution. Among the oldest lineages affected by fire is Coniferae. This lineage occupies a variety of fire prone and non-fire prone habitats across all hemispheres and has four fire-adapted traits: (1) thick bark; (2) serotiny; (3) seedling grass stage; and (4) resprouting ability.

Background: Application of enhanced recovery after surgery (ERAS) pathways in robotic lobectomy have been associated with decreased length of stay (LOS). We evaluated differences in patient characteristics and achievements of ERAS benchmarks by discharge groups at a tertiary referral center.

Materials And Methods: We performed a retrospective analysis of a prospectively maintained ERAS database of patients undergoing robotic lobectomy for pulmonary malignancy.

Background: X chromosome inactivation (XCI) is a female-specific process in which one X chromosome is silenced to balance X-linked gene expression between the sexes. XCI is initiated in early development by upregulation of the lncRNA Xist on the future inactive X (Xi). A subset of X-linked genes escape silencing and thus have higher expression in females, suggesting female-specific functions.

A bipartite bacterial virulence factor targets the complement system and neutrophil activation.

The complement system and neutrophils constitute the two main pillars of the host innate immune defense against infection by bacterial pathogens. Here, we identify T-Mac, a novel virulence factor of the periodontal pathogen Treponema denticola that allows bacteria to evade both defense systems. We show that T-Mac is expressed as a pre-protein that is cleaved into two functional units.

Vessel strike encounter risk model informs mortality risk for endangered North Atlantic right whales along the United States east coast.

Vessel strikes are a critical threat to endangered North Atlantic right whales (Eubalaena glacialis), significantly contributing to their elevated mortality. Accurate estimates of these mortality rates are essential for developing effective management strategies to aid in the species' recovery. This study enhances existing vessel strike models by incorporating detailed regional data on vessel traffic characteristics as well as whale distribution and behavior.

Machine learning based prediction model for bile leak following hepatectomy for liver cancer.

Objective: We sought to develop a machine learning (ML) preoperative model to predict bile leak following hepatectomy for primary and secondary liver cancer.

Methods: An eXtreme Gradient Boosting (XGBoost) model was developed to predict post-hepatectomy bile leak using data from the ACS-NSQIP database. The model was externally validated using data from hepatocellular carcinoma (HCC) and intrahepatic cholangiocarcinoma (ICC) multi-institutional databases.

Lung-kidney axis in cystic fibrosis: Early urinary markers of kidney injury correlate with neutrophil activation and worse lung function.

Background: Adult people with cystic fibrosis (PwCF) have a higher risk of end-stage kidney disease than the general population. The nature and mechanism of kidney disease in CF are unknown. This study quantifies urinary kidney injury markers and examines the hypothesis that neutrophil activation and lung infection are associated with early kidney injury in CF.

Behavioral and sociodemographic correlates of sleep duration among children in Samoa.

Objectives: To describe sleep duration, adherence to sleep recommendations, and behavioral and sociodemographic correlates of sleep among Samoan children.

Methods: In a longitudinal cohort study of Samoan children aged 2-9years (n = 481; 50% female), primary caregivers reported usual number of hours of nighttime sleep during 2015, 2017/2018, and 2019/2020 data collection waves. Associations between behavioral and sociodemographic characteristics and sleep duration were assessed using generalized linear and mixed effect regressions.

The impact of risk compensation adaptive behavior on the final epidemic size.

Public health interventions reduce infection risk, while imposing significant costs on both individuals and the society. Interventions can also lead to behavioral changes, as individuals weigh the cost and benefits of avoiding infection. Aggregate epidemiological models typically focus on the population-level consequences of interventions, often not incorporating the mechanisms driving behavioral adaptations associated with interventions compliance.

Sugar sweetened-beverage (SSB) consumption contributes to poor diet quality and diet-related chronic diseases. One effective public health strategy to reduce SSB consumption is to tax SSB. Laboratory approaches can complement existing methods to improve understanding of how taxes on SSB influence purchasing.

The opioid crisis has claimed approximately one million lives in the United States since 1999, underscoring a significant public health concern. This surge in opioid use disorder (OUD) fatalities necessitates improved therapeutic options. Current OUD therapies often require daily clinical visits, leading to poor patient compliance and high costs to the health systems.

Objective: Malalignment following cervical spine deformity (CSD) surgery can negatively impact outcomes and increase complications. Despite the growing ability to plan alignment, it remains unclear whether preoperative goals are achieved with surgery. The objective of this study was to assess how good surgeons are at achieving their preoperative goal alignment following CSD surgery.

Improved Patient-Reported Outcomes With Post-Transplant Cyclophosphamide: A Quality-of-Life Evaluation and 2-Year Outcomes of BMT CTN 1703.

The BMT CTN 1703 phase III trial confirmed that graft-versus-host disease (GVHD) prophylaxis with post-transplantation cyclophosphamide (PTCy), tacrolimus (Tac), and mycophenolate mofetil (MMF) results in superior GVHD-free, relapse-free survival (GRFS) compared with Tac/methotrexate (MTX) prophylaxis. This companion study assesses the effect of these regimens on patient-reported outcomes (PROs). Using the Lee Chronic GVHD Symptom Score and PROMIS subscales (physical function, GI symptoms, social role satisfaction) as primary end points and hemorrhagic cystitis symptoms and Lee subscales as secondary end points, responses from English and Spanish speakers were analyzed at baseline and days 100, 180, and 365 after transplant.

Stimulating Medical Student Professional Identity Formation Through Mentored Longitudinal Partnerships With Patient Teachers.

The authors describe the novel Patient Student Partnership (PSP) program, which provides authentic roles for students in mentored longitudinal patient relationships while maintaining a traditional block clerkship model.ApproachThe PSP program at the University of Virginia School of Medicine pairs all matriculating medical students with a patient living with chronic illness to follow across multiple health care settings until graduation.

The omicron variant of SARS-CoV-2 drove broadly increased seroprevalence in a public university setting.

Omicron is the comparatively most transmissible and contagious variant of severe acute respiratory syndrome-coronavirus-2 (SARS-CoV-2). We conducted a seroprevalence study from March 1-3, 2022, to investigate the seroprevalence of SARS-CoV-2 antibodies among individuals aged 18 years and older after the Omicron outbreak. The seroprevalence of anti-receptor binding domain (RBD) antibodies was found to be 96.

Wavebreakers in excitable systems and possible applications for corrosion mitigation.

Traveling waves of excitation arise from the spatial coupling of local nonlinear events by transport processes. In corrosion systems, these electro-dissolution waves relay local perturbations across large portions of the metal surface, significantly amplifying overall damage. For the example of the magnesium alloy AZ31B exposed to sodium chloride solution, we report experimental results suggesting the existence of a vulnerable zone in the wake of corrosion waves where local perturbations can induce a unidirectional wave pulse or segment.

Prevalence Estimation Methods for Time-Dependent Antibody Kinetics of Infected and Vaccinated Individuals: A Markov Chain Approach.

Immune events such as infection, vaccination, and a combination of the two result in distinct time-dependent antibody responses in affected individuals. These responses and event prevalence combine non-trivially to govern antibody levels sampled from a population. Time-dependence and disease prevalence pose considerable modeling challenges that need to be addressed to provide a rigorous mathematical underpinning of the underlying biology.

Immunometabolism In Brain Aging and Neurodegeneration: Bridging Metabolic Pathways and Immune Responses.

The complex set of interactions between the immune system and metabolism, known as immunometabolism, has emerged as a critical regulator of disease outcomes in the central nervous system. Numerous studies have linked metabolic disturbances to impaired immune responses in brain aging, neurodegenerative disorders, and brain injury. In this review, we will discuss how disruptions in brain immunometabolism balance contribute to the pathophysiology of brain dysfunction.
