Article Synopsis
  • Thoraco-abdominal normothermic regional perfusion (TA-NRP) combined with ECMO is a promising method for reviving hearts in donors who have been declared dead by circulatory criteria (DCDD).
  • This process involves restoring blood flow to the heart after death and restarting the donor's circulation and oxygenation using their own organs, but complications can arise if the donor's lungs cannot oxygenate blood effectively.
  • The paper presents three cases where a split ECMO circuit was used, allowing successful heart recovery and transplantation from DCDD donors despite challenges with pre-existing respiratory failure.

Purpose: To describe the clinical outcomes beyond pain relief of high-frequency spinal cord stimulation at 10 kHz (10 kHz SCS) in a patient with cervical myelopathy and drug-resistant chronic neuropathic pain with spastic tetraparesis.

Methods: A patient with C3-C6 myelomalacia and spastic tetraparesis previously treated with decompressive laminectomy underwent implantation of 10 kHz SCS for pain management through a trial procedure followed by permanent implantation. Due to the presence of epidural fibrotic scar tissue in the area of the previous C3-C6 laminectomy, the leads could not be implanted at the cervical level; therefore, the leads were positioned at the thoracic level.

  • A study was conducted to compare robotic minor liver resections (RMLR) with laparoscopic minor liver resections (LMLR) in patients undergoing surgery on the anterolateral liver segments.
  • The analysis included over 10,000 patients and employed propensity score matching to balance the groups for accuracy in comparisons.
  • Results indicated RMLR had benefits like less blood loss, lower major morbidity, and shorter hospital stays than LMLR, although the difference in 30-day readmission rates suggested RMLR may have some drawbacks.

Article Synopsis
  • - Surgeries for abdominal wall sarcomas often involve extensive resections that create complex defects, but using both porcine dermal xenografts and composite meshes can help with repair while reducing complications.
  • - A study involving 19 patients (average age 53.2 years) identified various types of sarcomas and demonstrated that surgical resections and reconstructions using these materials can be effective.
  • - After an average follow-up of 38 months, complications occurred in about 31.5% of patients, including wound issues and one death, but overall, the technique showed promising medium-term results.

Peritoneal sarcomatosis is a rare malignant disease with a poor prognosis, secondary to peritoneal dissemination of abdominopelvic soft tissue sarcomas. Its rarity, together with the characteristic histological heterogeneity and the historically poor response to systemic treatments, has prevented the establishment of widely accepted treatment criteria with curative intent. In this sense, radical cytoreductive surgery (CRS) with peritonectomy procedures and hyperthermic intraperitoneal chemotherapy (HIPEC), widely used in peritoneal carcinomatosis with excellent results, have not had the same evolutionary development in patients with peritoneal sarcomatosis.

Bioengineered Mesenchymal Stem/Stromal Cells in Anti-Cancer Therapy: Current Trends and Future Prospects.

Biomolecules

June 2024

Hematopoietic Transplant and Cellular Therapy Unit, Instituto Murciano de Investigación Biosanitaria (IMIB) Pascual Parrilla, Virgen de la Arrixaca University Hospital, University of Murcia, 30120 Murcia, Spain.

Mesenchymal stem/stromal cells (MSCs) are one of the most widely used cell types in advanced therapies due to their therapeutic potential in the regulation of tissue repair and homeostasis, and immune modulation. However, their use in cancer therapy is controversial: they can inhibit cancer cell proliferation, but also potentially promote tumour growth by supporting angiogenesis, modulation of the immune milieu and increasing cancer stem cell invasiveness. This opposite behaviour highlights the need for careful and nuanced use of MSCs in cancer treatment.

  • People who get solid organ transplants often have problems with erectile dysfunction, which can be affected by their other health issues and problems with their organ.
  • This study looked at how safe it is to use penile prostheses in these patients and found that about 11% of them had complications, mostly infections and device failures.
  • Even though there are some risks, doctors can feel more confident about using these prostheses in transplant recipients since the study showed similar reoperation rates compared to non-transplanted patients.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is a chronic liver condition that often progresses to more advanced stages, such as metabolic dysfunction-associated steatohepatitis (MASH). MASH is characterized by inflammation and hepatocellular ballooning, in addition to hepatic steatosis. Despite the relatively high incidence of MASH in the population and its potential detrimental effects on human health, this liver disease is still not fully understood from a pathophysiological perspective.
