Screening women in young adulthood for disabling dysmenorrhoea: a nationwide cross-sectional study from the CONSTANCES cohort.

Reprod Biomed Online

July 2024

Team RISCQ 'Clinical risk and security on women's health and perinatal health', Université Paris-Saclay, UVSQ, Montigny-le-Bretonneux, France; Department of Gynecology and Obstetrics, Intercommunal Hospital Center of Poissy-Saint-Germain-en-Laye, Poissy, France.

Research Question: How do different warning indicators help to identify disabling dysmenorrhoea among women in young adulthood?

Design: A nationwide cross-sectional study of women aged 18-25 years from the CONSTANCES cohort was constructed. Disability was assessed with the Global Activity Limitation Indicator question 'For the past 6 months, have you been limited in routine activities?Yes, severely limited/Yes, limited/ No, not limited'. Dysmenorrhoea pain intensity and other chronic pelvic pain symptoms (dyspareunia and non-menstrual pain) were evaluated according to questions from a specific questionnaire.

Department of Obstetrics and Gynecology, La Miletrie University Hospital, Poitiers, France; Poitiers University, INSERM CIC 1402, Poitiers, France.

Background: It is likely that the pathophysiology of urinary incontinence (UI) differs between women who are incontinent before the first delivery and those whose incontinence occurs after. In this systematic review, we aimed to assess the association between the mode of delivery and the risk of postpartum UI in primiparous women with and without prenatal UI.

Methods: We searched MEDLINE, Cochrane, Web of Science, Embase and CINHAL databases.

Article Synopsis
  • * Researchers identified multiple FXI deficiency cases with significant gene deletions, including whole gene deletions and specific exon deletions, through techniques like Sanger sequencing and Multiplex Probe Ligation-dependent Assay (MLPA).
  • * The findings suggest that a notable 2.9% of patients had gene deletions, indicating that systematic use of MLPA or Next Generation Sequencing (NGS) could improve diagnosis in unsolved FXI cases.

Glucose homeostasis is a real challenge for extremely preterm infants (EPIs) who have both limited substrate availability and immature glucose metabolism regulation. In the first days of life, EPIs frequently develop transient glucose intolerance, which has a complex pathophysiology that associates unregulated gluconeogenesis, immature insulin secretion, and peripheral insulin resistance. In this population, glucocorticoid therapy is frequently administrated to prevent severe bronchopulmonary dysplasia.

Article Synopsis
  • A study was conducted to examine persistent symptoms in patients post-COVID-19 and whether they were linked more to SARS-CoV-2 infection compared to other conditions.
  • The research involved 25,910 participants from a French cohort who reported their symptoms during the pandemic and underwent serological testing to determine COVID-19 infection status.
  • Findings showed that those with confirmed COVID-19 (ECDC+/Sero+) had a significantly higher risk of persistent symptoms like loss of taste/smell and breathlessness, while some other symptoms, like abdominal pain, were less common in this group.

Article Synopsis
  • Twin pregnancies are increasing around the world, but they often lead to early births, which can cause health problems for babies.
  • The study aimed to find out if using a special device called the Arabin pessary could help improve the health of babies in twin pregnancies where the mother has a short cervix.
  • Researchers compared the effects of the pessary with regular care in 308 pregnancies to see if it reduced serious health issues or deaths for newborns.

Background: Prolonged pregnancies are a frequent indication for induction of labour. When the cervix is unfavourable, cervical ripening before oxytocin administration is recommended to increase the likelihood of vaginal delivery, but no particular method is currently recommended for cervical ripening of prolonged pregnancies. This trial evaluates whether the use of mechanical cervical ripening with a silicone double balloon catheter for induction of labour in prolonged pregnancies reduces the cesarean section rate for nonreassuring fetal status compared with pharmacological cervical ripening by a vaginal pessary for the slow release of dinoprostone (prostaglandin E2).

This publication is derived from and rooted in the authors' experience in designing the Health Support of a remote industrial site. Summarizing the main steps of this design is the purpose of the approach. As a first step devoted to "Evaluation" (Chapters 1-5) are displayed the fundamentals for designing a Project Health Plan, such as a realistic and operative definition of "patient stabilization" and the principles of tactical reasoning for Medevacs, specifying how pathophysiology and logistic constraints should be correlated.
