Department of Critical Care Medicine, Zhongda Hospital, Southeast University, No.87, Dingjiaqiao, Gulou District, Nanjing, 210009, China.

Venoarterial extracorporeal membrane oxygenation (VA-ECMO) has been utilized to treat massive pulmonary embolism (PE) accompanied by cardiac arrest or refractory cardiogenic shock. Our team opted for a femoral-femoral approach for vascular cannulation, using drainage and return cannulas in the common femoral vein and artery, respectively. However, femoral venous cannulation can be limited or challenging due to the presence of thrombus in the inferior vena cava (IVC), making the insertion of the drainage cannula via the femoral vein difficult.

Article Synopsis
  • The study investigates sex differences in treatment approaches for pulmonary embolism (PE) among older adults, revealing that existing evidence on disparities is limited and often inconclusive due to small sample sizes and inadequate methods.
  • Analysis of data from a European PE registry and US Medicare beneficiaries shows no significant overall sex differences in the use of anticoagulation or advanced therapies, although fewer women received fibrinolytic therapy compared to men with intermediate-risk PE.
  • The authors suggest that future research should explore whether the observed sex disparities in treatment correlate with different clinical outcomes, especially regarding fibrinolytic therapy and advanced treatment utilization in older adults in the US.

Protein C deficiency is a hereditary disorder that increases the risk of thrombotic events but has unclear effects on rotational thromboelastometry (ROTEM) analysis. A man in his 60s with a history of protein C deficiency and multiple thrombotic events underwent inferior vena cava (IVC) filter removal, iliocaval thrombectomy and infrarenal IVC placement for IVC-related complete iliocaval and common femoral vein thromboses. A ROTEM analysis showed normal coagulation in NATEM and EXTEM and only a slight shortening in A10 and A20 in INTEM, which was unexpected given his diagnosis of protein C deficiency.

Injuries to the retrohepatic segment of the inferior vena cava require complex procedures, as exposure without prior vascular control can lead to uncontrollable and fatal bleeding. To achieve such control, the classic techniques of hepatic vascular exclusion and the implantation of an atriocaval shunt have been described, and more recently, endovascular strategies have been reported. However, there is no consensus in the literature regarding which of these strategies is associated with lower mortality.

Management of antithrombotic therapy in patients undergoing dental procedures.

J Thromb Haemost

October 2024

Department of Medicine, Thrombosis Research Group, Brigham and Women's Hospital, Harvard Medical School, Boston, Massachusetts, USA; Cardiovascular Medicine Division, Brigham and Women's Hospital, Harvard Medical School, Boston, Massachusetts, USA; Yale-New Haven Hospital/Yale Center for Outcomes Research and Evaluation (CORE), New Haven, Connecticut, USA. Electronic address:

A growing number of patients receiving antithrombotic therapy require dental procedures. Dental interventions in these patients can be challenging, as the risk of bleeding from the continuation of antithrombotic therapy needs to be weighed against the thromboembolic risk associated with drug interruption or de-escalation. Most minor dental procedures, including simple dental cleaning and filling, pose minimal bleeding risk, and antiplatelet or anticoagulation therapy can be continued without interruption.

Interactions between food and oral anticoagulants (OACs), particularly vitamin K antagonists such as warfarin, are widely recognized and may also be clinically relevant for direct OACs. Pharmacokinetic and pharmacodynamic interactions with food or herbs can lead to anticoagulation potentiation, increased risk of bleeding, or reduced drug efficacy, all compromising patient safety. We conducted a systematic search for randomized controlled trials (RCTs) on PubMed for assessments of interactions between OACs and various ingestants.

Central venous occlusion (CVO) or stenosis (CVS) is a common complication of long-term hemodialysis catheters. Endovascular intervention, primarily balloon angioplasty and occasionally stent placement, is the primary approach for managing CVS/CVO lesions. The presence of a filter within the inferior vena cava (IVC) lumen makes recanalization of the IVC more challenging.

Appropriateness guidelines for inferior vena cava filter utilization: A contemporary review.

Semin Vasc Surg

June 2024

Division of Vascular Surgery and Endovascular Therapy, University of Alabama at Birmingham, Birmingham, AL. Electronic address:

Concern regarding the exponential increase in optional utilization of inferior vena cava filters (IVCFs) in the early 2000s with a persistent low retrieval rate nationwide has resulted in increased scrutiny regarding clinical application of IVCFs. IVCFs are used in a variety of clinical scenarios, ranging from thromboembolic protection in patients with deep venous thrombosis and contraindication to anticoagulation to prophylactic deployment in multitrauma and critically ill patients. Evidence supporting IVCFs as mechanical thromboembolic protection in certain clinical scenarios has been established through evidenced-based guidelines.

Finding the Balance: Venous Thromboembolism Prophylaxis during Hematochezia in Crohn's Disease.

Case Rep Gastroenterol

July 2024

Emory at Grady, Clinical Informaticist, Grady Health System, Division of General Internal Medicine, Emory University School of Medicine, Atlanta, GA, USA.

Introduction: Despite heightened risk of venous thromboembolism (VTE) in hospitalized patients with inflammatory bowel disease (IBD), pharmacologic prophylaxis remains underutilized, particularly in those presenting with hematochezia. Although placement of retrievable inferior vena cava filters (rIVCF) may be considered in those with contraindications to anticoagulation and VTE risk, current recommendations from clinical guidelines are incongruent, leading to wide variation in practice.

Case Presentation: This report highlights a case of rIVCF used in the management of recurrent VTEs in a patient hospitalized for persistent gastrointestinal bleeding.
