Purpose: To evaluate the efficacy and safety of Ahmed glaucoma valve (AGV) implantation with subsequent trans-scleral diode cyclophotocoagulation (CPC) as the main intervention if IOP remained medically uncontrolled.
Patients And Methods: Charts of 108 consecutive eyes (90 patients) that underwent AGV implantation from 2003 to 2018 at a single clinical practice were retrospectively reviewed. The procedure was considered a failure if any of the following occurred: additional incisional glaucoma surgery, IOP >21 mmHg or < 20% reduction from baseline on 2 consecutive study visits after 3 months, IOP ≤ 5 mmHg on 2 consecutive study visits after 3 months, loss of light perception, or AGV removal.

Iatrogenic type A aortic dissection (IAAD) is a rare but life-threatening complication of percutaneous coronary intervention (PCI), often presenting significant therapeutic challenges. A looped guidewire between the right subclavian artery and brachiocephalic artery during coronary angiography (CAG) via right radial artery access can complicate catheter manipulation. This report discusses the case of a 58-year-old hypertensive female patient who developed aortic dissection during PCI, specifically due to wire manipulation at the brachiocephalic loop.

The Progressive Masked Voice Exercises (PMVE) with an innovative semioccluded ventilation mask fitted and adjustable positive end-expiratory pressure (PEEP) valve was evaluated. This study aimed to compare the effectiveness of the PMVE with the PEEP device and the Vocal Function Exercise (VFE) program on acoustic, auditory-perceptual, aerodynamic, and self-report measures. Twenty-five participants diagnosed with voice disorders met the criteria.

Specialist Perinatal Mental Health Midwife, Darent Valley Hospital, Kent DA2 8DA, UK.
Objective: In the United Kingdom (UK) and most countries worldwide, midwives are professionally required to undertake an initial perinatal mental health (PMH) risk assessment at every maternity contact. However, studies have found that midwives feel that they are not well-equipped to provide effective care for women with PMH needs. This study explores how the newly qualified midwives (NQMs) are prepared through pre-registration midwifery education and placements to have sufficient confidence in their , , , (KASH) for their post-registration PMH role.

Introduction: Patients who sustain tibial shaft fractures are at risk for delayed recognition of complications with inadequate postoperative follow-up. Area Deprivation Index (ADI) is a surrogate which may be used to analyze the effects of socioeconomic deprivation on postoperative follow-up rates. The purpose of this study was to determine if ADI is associated with incomplete postoperative follow-up after intramedullary (IM) nailing for fractures of the tibial diaphysis.

Attributes of successful relationships, personal and professional, closely echo those of a successful preceptorship. Applications, or "apps," are responsible for one third of today's relationships; they have been hugely successful in making personal relationships viable. This article explores the possibility of using existing algorithms of dating-style apps to match preceptors to preceptees.

Conduction disturbances (CDs) are common after transcatheter aortic valve replacement. Continuous improvements in preprocedural planification, implant techniques, and device design have markedly reduced periprocedural complications. However, CDs rate remains in the double-digit range.

In this paper, we explore the usage of decision aids, patient information leaflets (PILs), videos, social media and modern technology to empower patients and enable shared decision-making (SDM). It explores the role of enhanced consent processes in urology. A re-evaluation of the conventional consent process is required towards more patient-centred care and SDM, which prioritises patient education and understanding of their medical conditions and treatment pathways.

Therapeutics of neuroinflammatory disorders including multiple sclerosis is one of the fastest growing areas in neurology. However, pressures on higher specialty training in neurology together with an expanding curriculum have led to challenges in adequately preparing trainees for a subspecialist career. In this study we set out to understand current perceptions and barriers to training in neuroinflammatory disorders among neurology trainees in the UK.

Platelet-rich plasma (PRP) is an autologous blood product containing concentrated platelets, growth factors, and anti-inflammatory cytokines that promote healing and regeneration. Platelets release active components through a degranulation process, which is inhibited by certain nonsteroidal anti-inflammatory drugs (NSAIDs). Current deferral guidelines are not established, but NSAIDs are expected to have a time-dose relationship with platelet inhibition.

Lowering low-density lipoprotein cholesterol (LDL-C) plasma levels is crucial for the prevention of primary and secondary cardiovascular diseases (CVDs). Many patients struggle to obtain goal LDL-C levels, despite the availability of several lipid-lowering medications, because of limited efficaciousness and unfavorable side effects. Proprotein convertase subtilisin/kexin type 9 (PCSK9) targeting has drawn interest recently as a novel approach to further lower cardiovascular (CV) risk.

The Association of British Neurologists last published guidelines on disease-modifying treatment (DMT) in multiple sclerosis (MS) in 2015. Since then, additional DMTs have been licensed and approved for prescribing within the National Health Service for relapsing-remitting MS, early primary progressive MS and active secondary progressive MS. This updated guidance provides a consensus-based approach to using DMTs.

Introduction: Hepatic angioembolization is highly effective for hemorrhage control in hemodynamically stable patients with traumatic liver injuries and contrast extravasation. However, there is a paucity of data regarding the specific location of angioembolization within the hepatic arterial vasculature and its implications on patient outcomes.
Methods: A post-hoc analysis of a multicenter prospective observational study across 23 centers was performed.

Lower eyelid blepharoplasty has historically been approached from one of only two techniques, either the anterior subciliary incision or from the posterior transconjunctival approach. Both have been used with subtractive techniques of skin and fat resection in most cases and both have had issues with post operative lower lid retraction with scleral show, albeit less with the posterior approach. Lateral acces recontouring does not transgress either the anterior or posterior functional muscles of the lower lid and avoids damaging the innervation of the lower lid orbicularis.

The incidence of twin pregnancies has been rising, largely attributable to increasing use of artificial reproductive techniques. Ultrasound plays a critical role in establishing the chorionicity and amnionicity of multiple gestations, a key predictor of the expected risk and complications, along with guiding future clinical and imaging follow-up examinations and intervals. People carrying multiple gestations will typically undergo more ultrasound examinations (and occasionally fetal MRI) than those carrying singletons, at minimum including a first trimester dating scan, nuchal translucency scan at 11 to 14 weeks, an anatomy scan at 18 to 22 weeks, and other scans in the second and third trimesters for growth and surveillance.
