Although family caregivers are increasingly recognized for their essential role in helping vulnerable adults live in the community for as long as possible, their priorities and perspectives have not been well-integrated into quality assessments of home- and community-based services (HCBS). Our overall goal was to identify measurement gaps to guide monitoring and improve HCBS. Caregiver-specific measurement priorities were identified during a multi-level stakeholder engagement process that included 34 Veterans, 24 caregivers, and 39 facility leaders, clinicians, and staff across four VA healthcare systems.

Objective: To evaluate whether the Preventive Health Inventory (PHI)-a virtual care management intervention addressing hypertension and diabetes management implemented nationally in the Veterans Health Administration (VHA)-was delivered equitably among racial/ethnic groups and if existing inequities in hypertension and diabetes outcomes changed following PHI receipt.

Data Sources And Study Setting: We used data from the VHA Corporate Data Warehouse among Veterans enrolled in primary care nationally from February 28, 2021 to March 31, 2022.

Study Design: We used logistic regression to evaluate PHI receipt and hypertension and diabetes outcomes after PHI implementation among Veterans with hypertension and/or diabetes.

Objective: To develop, deploy, and evaluate a national, electronic health record (EHR)-based dashboard to support safe prescribing of biologic and targeted synthetic disease-modifying agents (b/tsDMARDs) in the United States Veterans Affairs Healthcare System (VA).

Data Sources And Study Setting: We extracted and displayed hepatitis B (HBV), hepatitis C (HCV), and tuberculosis (TB) screening data from the EHR for users of b/tsDMARDs using PowerBI (Microsoft) and deployed the dashboard to VA facilities across the United States in 2022; we observed facilities for 44 weeks post-deployment.

Study Design: We examined the association between dashboard engagement by healthcare personnel and the percentage of patients with all screenings complete (HBV, HCV, and TB) at the facility level using an interrupted time series.

Research regarding HIV, substance use disorders (SUD), and SARS-CoV-2 infections after COVID-19 vaccination is limited. In the Veterans Aging Cohort Study (VACS)-HIV cohort, we followed vaccinated persons with HIV (PWH) and without HIV (PWoH) from 12/2020 to 3/2022 and linked SARS-CoV-2 test results for laboratory-confirmed breakthrough infection through 9/2022. We examined associations of substance use (alcohol use disorder [AUD], other SUD, smoking status) and HIV status and severity with breakthrough infections, using Cox proportional hazards regression hazard ratios (HR).

Department of Medicine, Division of Geriatrics (ALB), University of California, San Francisco CA; Research Service, San Francisco VA Health Care System (ALB), San Francisco, CA; Department of Psychiatry and Behavioral Sciences (ALB), University of California, San Francisco CA.

Objective: There exist significant age disparities in mental health (MH) utilization, such that older adults, including older veterans, are much less likely to use MH services. In-home caregivers represent a novel, yet understudied, pathway to increase appropriate utilization. We sought to examine the association between receiving caregiving assistance and MH utilization and test moderation effects of cognitive status and depression severity in a sample of older veterans.

Reimagining Social and Emotional Supports in Schools for Immigrant Students: A Contextual and Structural Approach.

Division of Population Behavioral Health (R Ijadi-Maghsoodi, K Lee, and S Kataoka), Jane and Terry Semel Institute for Neuroscience and Human Behavior, University of California, Los Angeles, Calif; Department of Psychiatry and Biobehavioral Sciences (R Ijadi-Maghsoodi, JI Meza, and S Kataoka), University of California, Los Angeles, Calif.

Immigrant students and families experience disproportionate exposure to trauma, immigration-related stress, structural inequities, and poor access to mental health and social services which can lead to mental health inequities. Immigrant students and their families also have many strengths that can buffer potential negative mental health outcomes. Schools, which address social and emotional development in addition to academic achievement, are critical institutions that can play a unique role in enhancing the strengths and responding to the needs of immigrant students and families.

HSR Center for the Study of Healthcare Innovation, Implementation & Policy (CSHIIP), VA Greater Los Angeles Healthcare System, Los Angeles, CA 90073, United States.

Article Synopsis
  • Racial and ethnic disparities in diabetes care quality exist even in high-performing healthcare facilities, highlighting that overall high-quality ratings do not guarantee equitable care for all groups.
  • * The study focused on veterans receiving diabetes care from the Veterans Health Administration between March 2020 and February 2021, revealing specific inequities affecting American Indian or Alaska Native, Black, and Hispanic patients compared to White patients.
  • * Addressing these disparities is crucial; incorporating equity into quality measures can promote improvements in care for underrepresented groups in healthcare settings.

Article Synopsis
  • - The study investigates how well statistical methods are used in research examining racial and ethnic disparities in surgical care from 2021 to 2022, focusing on if they adequately adjust for variables that could influence outcomes.
  • - Out of 224 papers reviewed, a significant portion (63.2% of single-institution and 60.8% of multisite studies) adjusted for social determinants of health, though only a small fraction considered the clustering of patients within hospitals or the effects of different providers.
  • - The findings indicate that many studies fail to meet essential statistical criteria, suggesting that improving publication guidelines could enhance the accuracy of these studies' estimates of disparities in surgical access and outcomes.
