A life-threatening infection: Fournier's gangrene.

Fournier's gangrene is a life-threatening disorder in which infection of the perineum and scrotum spreads along fascial planes, causing soft tissue necrosis. If urgent surgery is delayed, the disease will soon result in septic shock, multiorgan failure, and death. In this study, we present 21 patients with Fournier's gangrene who were treated in period between 1994 and 2001.

A 35-year-old male diagnosed with Wegener's granulomatosis (WG) in 1989 recently came to our hospital with the complaint of left lumbar pain. He was found to have left nephrolithiasis and left diaphragmatic elevation. Fluoroscopic study and electromyographic examination revealed findings compatible with unilateral phrenic paralysis.

Pulmonary alveolar proteinosis (PAP) is the intra-alveolar accumulation of periodic-acid schiff (PAS) positive material. PAP is one of the underrecognized causes of pulmonary infiltrates in patients with hematologic malignancies. The intrauterine contraceptive device has been in use for many years, and migration of the device from the uterus to the pelvic cavity has been reported by many investigators. Although perforation of the uterus by an intrauterine contraceptive device is not uncommon, intravesical migration and secondary stone formation are rare complications. We report on a 27-year-old woman in whom an intrauterine contraceptive device (Copper-T) migrated from the uterus to the bladder and resulted in formation of a stone.

Effect of melatonin on cerebral edema in rats.

Objective: Melatonin (5-methoxy-N-acetyltrypamine), a chemical naturally produced in the pineal gland, has been suggested to be a free radical scavenger and an antioxidant. In the present study, the effect of melatonin on cold-induced brain edema was evaluated by determination of cerebral water content, blood-brain barrier permeability, and areas of infarct; the effects were also studied histopathologically.

Methods: Brain edema was produced in rats by creating a lesion via cortical freezing.

Atypical tuberculosis of the spine.

Spinal tuberculosis characteristically involves the paradiscal area of vertebral bodies with a narrowing of the disc space. In this study, we reported four atypical forms of Pott's disease, including one hundred and eighty-four patients treated between 1985 and 1998. Two cases presented with noncontiguous multilevel involvement, where one case had transverse process involvement alone and the other had involvement of the neural arch.

A left transverse facial cleft with a median cleft of the lower jaw.

Objective: A 13-day-old girl with a left transverse facial cleft and a median defect of the lower jaw is reported in detail. Left macrostomia was repaired using a W-plasty technique, and preauricular appendages were excised. The median defect of the mandible demonstrated by computed tomography scan on the first examination had disappeared at 1 year of age.

Objective: This controlled experimental study was designed to compare the effects of a well-known NSAID, tenoxicam, with mid-laser irradiation on the inflammatory component of adjuvant-induced arthritis (AIA). Four groups of animals, each consisting of 10 Wistar rats, were included in the study. The primary concern was not to investigate the antiinflammatory effects of tenoxicam, but to compare the previously proven effects of this drug with a physical therapy agent which might be considered to have fewer side effects and/or contraindications.

This study was designed to assess morphological changes in the rabbit tympanic membrane after Ciprofloxacin and Trimethoprim + Sulphamethoxazole had been instilled as a single dose into the middle ear cavities of 24 rabbits. At different times the rabbits were decapitated and the lesions which occurred on the tympanic membrane were studied histopathologically. Our results indicate that in the Ciprofloxacin group reactive inflammatory changes are reversible as in the Trimethoprim + Sulphamethoxazole group.
