For decades, oncoplastic techniques have been an emerging, yet underutilized approach to breast cancer surgery. Recent developments in breast oncology support the potential for the omission of radiation therapy in a subset of patients eligible for breast-conserving surgery, which makes immediate reconstruction available to a much larger demographic. Although existing oncoplastic methods have offered durable and cosmetically acceptable results in select cases, there remains significant room for improvement.

Lesion characteristics using high-frequency low-tidal volume ventilation versus standard ventilation during ablation of paroxysmal atrial fibrillation.

J Cardiovasc Electrophysiol

October 2024

Cardiac Arrhythmia Service, Division of Cardiovascular Medicine, Brigham and Women's Hospital, Harvard Medical School, Boston, Massachusetts, USA.

Article Synopsis
  • High-frequency low-tidal-volume (HFLTV) ventilation is more effective than standard ventilation (SV) during radiofrequency catheter ablation (RFCA) for treating paroxysmal atrial fibrillation (PAF), as it enhances procedural efficiency and patient outcomes.
  • A study analyzed 70 patients who underwent pulmonary vein isolation with either HFLTV or SV, revealing that HFLTV led to shorter ablation durations, higher contact force, and better impedance reduction while maintaining the same ablation index.
  • Overall, HFLTV ventilation resulted in significant reductions in total procedural time, ablation time, and RF time compared to SV, indicating its advantages in improving ablation parameters.

Introduction: Retained shrapnel from gunshots is a common occurrence; however, retained shrapnel within the spinal canal is exceedingly uncommon. Guidelines for removal and treatment of these cases are a difficult topic, as surgical removal is not necessarily without consequence, and retention can lead to possible further injury or a secondary disease process of plumbism, which can be difficult to diagnose in this population.

Case Presentation: This case report provides a unique example of a young patient with retained shrapnel from a gunshot.

Refractory ischemic priapism is a difficult to treat clinical entity for which there are a host of shunt procedures identified, but no singular agreed upon technique for surgical therapy. Recent literature describes success using a penoscrotal decompression technique that uses a similar dissection of a penoscrotal penile prosthesis placement. We demonstrate that this technique is easily applicable in the private practice setting as it uses a familiar setup to most general urologists in our case report.

Background: Acute tandem occlusions (TOs) are challenging to treat. Although acute carotid stenting of the proximal lesion is well tolerated, there are certain situations when the practitioner may be wary of acute stenting (bleeding concerns).

Objective: The purpose of this study was to retrospectively study patients with tandem occlusions who had re-occlusion of the extracranial ICA and develop a Circle of Willis Score (COWS) to help predict which patients could forego acute stenting.

Article Synopsis
  • - The use of gonadotropin-releasing hormone (GnRH) agonists has become popular for temporarily suppressing ovarian function in pre-menopausal breast cancer patients undergoing treatment with aromatase inhibitors or tamoxifen, showing positive survival outcomes in studies involving nearly 15,000 women.
  • - Previous clinical trials have primarily measured testosterone in prostate cancer and estradiol in breast cancer to evaluate the effectiveness of GnRH agonists.
  • - The text emphasizes that estradiol is the preferred biomarker for monitoring ovarian function suppression with GnRH agonist therapy in pre-menopausal breast cancer patients and suggests that future research should focus on maintaining estradiol levels as a key endpoint in trials.

The Professional Doctorate in Medical Physics (DMP) was originally conceived as a solution to the shortage of medical physics residency training positions. While this shortage has now been largely satisfied through conventional residency training positions, the DMP has expanded to multiple institutions and grown into an educational pathway that provides specialized clinical training and extends well beyond the creation of additional training spots. As such, it is important to reevaluate the purpose and the value of the DMP.
