The Japanese Clinical Practice Guidelines for Management of Sepsis and Septic Shock 2020 (J-SSCG 2020), a Japanese-specific set of clinical practice guidelines for sepsis and septic shock created as revised from J-SSCG 2016 jointly by the Japanese Society of Intensive Care Medicine and the Japanese Association for Acute Medicine, was first released in September 2020 and published in February 2021. An English-language version of these guidelines was created based on the contents of the original Japanese-language version. The purpose of this guideline is to assist medical staff in making appropriate decisions to improve the prognosis of patients undergoing treatment for sepsis and septic shock.

The exponential growth of public datasets in the era of Big Data demands new solutions for making these resources findable and reusable. Therefore, a scholarly recommender system for public datasets is an important tool in the field of information filtering. It will aid scholars in identifying prior and related literature to datasets, saving their time, as well as enhance the datasets reusability.

This paper describes an initial dataset and automatic natural language processing (NLP) method for extracting concepts related to precision oncology from biomedical research articles. We extract five concept types: Cancer, Mutation, Population, Treatment, Outcome. A corpus of 250 biomedical abstracts were annotated with these concepts following standard double-annotation procedures.

Clinical correlates of social cognition after an ischemic stroke: preliminary findings.

Dement Neuropsychol

January 2021

Laboratório Interdisciplinar de Investigação Médica, Faculdade de Medicina, Universidade Federal de Minas Gerais - Belo Horizonte, MG, Brazil.

Unlabelled: The co-occurrence of post-stroke behavioral disorders and cognitive impairment has been extensively investigated. However, studies usually do not include social cognition among the assessed cognitive domains.

Objective: To investigate the potential association between facial emotion recognition, a measure of social cognition, and behavioral and cognitive symptoms in the subacute phase of ischemic stroke.

We apply deep learning-based language models to the task of patient cohort retrieval (CR) with the aim to assess their efficacy. The task ofCR requires the extraction of relevant documents from the electronic health records (EHRs) on the basis of a given query. Given the recent advancements in the field of document retrieval, we map the task of CR to a document retrieval task and apply various deep neural models implemented for the general domain tasks.

Radiology reports have been widely used for extraction of various clinically significant information about patients' imaging studies. However, limited research has focused on standardizing the entities to a common radiology-specific vocabulary. Further, no study to date has attempted to leverage RadLex for standardization.

The rise of metabolic disorders in modern times is mainly attributed to the environment. However, heritable effects of environmental chemicals on mammalian offsprings' metabolic health are unclear. Inorganic arsenic (iAs) is the top chemical on the Agency for Toxic Substances and Disease Registry priority list of hazardous substances.

Objective: With a significant proportion of individuals with opioid use disorder not currently receiving treatment, it is critical to find novel ways to engage and retain patients in treatment. Our objective is to describe the feasibility and preliminary outcomes of a program that used emergency physicians to initiate a bridge treatment, followed by peer support services, behavioral counseling, and ongoing treatment and follow-up.

Methods: We developed a program called the Houston Emergency Opioid Engagement System (HEROES) that provides rapid access to board-certified emergency physicians for initiation of buprenorphine, plus at least 1 behavioral counseling session and 4 weekly peer support sessions over the course of 30 days.

A Natural Language Processing Tool to Extract Quantitative Smoking Status from Clinical Narratives.

Proc (IEEE Int Conf Healthc Inform)

March 2021

Health Outcomes and Biomedical Informatics College of Medicine, University of Florida Gainesville, USA.

This study presents a natural language processing (NLP) tool to extract quantitative smoking information (e.g., Pack-Year, Quit Year, Smoking Year, and Pack per Day) from clinical notes and standardized them into Pack-Year unit.

Objectives: Mitochondrial dysfunction plays a pivotal role in the pathogenesis of nonalcoholic fatty liver disease (NAFLD). We hypothesized that mitochondrial DNA (mtDNA) haplogroups affect the risk of NAFLD in Han Chinese patients and interact with PNPLA3 genotypes.

Design: NAFLD and control patients were recruited from a tertiary care centre.

Objective: The objective of this study was to determine whether crowding influences treatment times and disposition decisions for emergency department (ED) patients.

Methods: We conducted a retrospective cohort study at 2 hospitals from January 1, 2014, to July 1, 2014. Adult ED visits with dispositions of discharge, admission, or transfer were included.

Objective: The opioid epidemic continues to escalate, and out-of-hospital emergency medical services (EMS) play a vital role in acute overdose reversal, but could serve a broader role post-incident for follow-up, outreach, and referrals. Our objective is to identify the scope and prevalence of community-based, post-opioid overdose EMS programs across the United States.

Methods: We used a narrative review of prior studies in PubMed and Scopus for the last 20 years (1999-2020) to identify relevant medical literature and a web search to identify gray literature of EMS interventions involving opioids.

Article Synopsis
  • Radiology reports have important information about patients, often describing where things are in the body.
  • The study aims to automatically find these location-based words (called spatial expressions) in different types of radiology reports.
  • They created a new method using deep learning that is better at identifying these terms than older techniques, showing improvement in results.

Background Excess visceral adipose tissue (VAT) is a primary driver for the cardiometabolic complications of obesity; VAT-associated cardiovascular disease risk varies by race, but most studies have been done on Non-Hispanics. This study aimed to evaluate the clinical and metabolic correlates of VAT, its association with subclinical atherosclerosis, and the factors affecting this association in Mexican Americans. Methods and Results Participants (n=527) were drawn from the Cameron County Hispanic Cohort (CCHC), on whom a carotid ultrasound to assess carotid intima media thickness and a dual-energy X-ray absorptiometry scan to assess for VAT were obtained.

This study investigated the role of muscle damage in bone defect healing using skull and tibial double-defect and tibial fracture models in dystrophin/Utrophin double-knockout (dKO-Hom) mice. The skull and tibia bone defect and fracture healing was monitored using micro-CT, histology, immuohistochemistry and quantitative PCR. We found the skull defect healing is not impaired while the tibial defect healing was delayed at day 7 in the dKO-Hom group compared to wild-type (WT) group as revealed by micro-CT.

Background: Patients with hemorrhagic shock from trauma often require balanced blood product transfusion with red blood cells, plasma, and platelets. Resuscitation with whole blood resuscitation is becoming a common practice. We performed a systematic review and meta-analysis of studies comparing whole blood transfusion with balanced component therapy in patients suffering from traumatic hemorrhagic shock.

Background: The national incidence and characteristics of out-of-hospital cardiac arrest in the United States is unclear. We sought to describe the national characteristics of adult out-of-hospital cardiac arrest reported in the National Emergency Medical Services Information System (NEMSIS).

Methods: We used 2016 NEMSIS data, consisting of most emergency medical services (EMS) responses from 46 states and territories.

Article Synopsis
  • A national study analyzed data from over 6 million EMS responses, identifying that shock was present in 62,867 cases (about 1.02%).
  • The majority of these cases were medical shock (86.3%), with trauma shock primarily resulting from falls and motor vehicle crashes.
  • The study found that systolic blood pressure improved significantly for both medical and traumatic shock patients, with a quicker and greater increase in blood pressure noted in trauma cases.
