Noninvasive Expandable Endoprostheses From Reconstruction to Skeletal Maturity: A Surgical Technique and Lengthening Guide.

J Am Acad Orthop Surg

December 2021

From the Erlanger Orthopaedic Institute, University of Tennessee/Erlanger Orthopaedics (Voskuil), Division of Musculoskeletal Oncology, University of Tennessee College of Medicine-Chattanooga, Department of Orthopaedic Surgery (Voskuil), the Department of Orthopaedic Surgery, University of Tennessee College of Medicine Chattanooga (Miles), and the Division of Musculoskeletal Oncology, The James Cancer Hospital and Solove Research Institute, The Ohio State University Wexner Medical Center and Nationwide Children's Hospital (Scharschmidt and Alexander).

Limb salvage is the benchmark for pediatric extremity bone sarcomas. However, reconstructive strategies must account for any anticipated remaining growth potential and the resultant limb inequality. Expandable endoprostheses offer the theoretical advantage of immediate weight-bearing, predictable function, and reliable maintenance of leg-length equality.

Objective: We aimed to specifically compare the impact of a night-float system vs. a 24-hour call system on the number and types of cases performed by PGY-1 and PGY-2 general surgery residents to determine if both of these schedules could meet the ACGME first two-year 250 case minimum requirement, and if so, which schedule provided the best operative experience for PGY-1 and PGY-2 residents.

Design: This is a retrospective review of call schedules and operative case logs of PGY-1 and PGY-2 general surgery residents.

Article Synopsis
  • Higher body mass index (BMI) is linked to a higher risk of developing hip and knee osteoarthritis, leading to younger patients requiring total hip (THA) and knee arthroplasties (TKA).
  • A study analyzed data from the National Inpatient Sample (2002-2017) and found a significant increase in obesity rates among patients undergoing THA (from 7.0% to 22.7%) and TKA (from 10.7% to 30.4%), along with a decrease in the average age of patients for both procedures.
  • These findings highlight a concerning national trend where patients needing hip and knee surgeries are becoming both younger and more obese, corresponding with the rising obesity epidemic.

Spontaneous coronary artery dissection with cardiogenic shock in the third trimester, and a successful early-term delivery: a case report.

Eur Heart J Case Rep

March 2021

Department of Medicine, Centennial Heart at Parkridge, 2333 McCallie Avenue, Chattanooga, TN 37404, USA.

Background: Acute myocardial infarction in pregnancy is occasionally due to spontaneous coronary artery dissection (SCAD). Although uncommon, the majority of cases of pregnancy-associated SCAD (pSCAD) has critical presentations with more profound defects that portend high maternal and foetal mortality, and frequently necessitate preterm delivery. This is a case of pSCAD with ongoing ischaemia that required temporary mechanical circulatory support (MCS) and emergent revascularization, while the pregnancy was successfully continued to early-term.

Although the adverse effect of burnout on physicians has been widely documented, studies have shown an inconsistent relationship between burnout and the quality of patient care. We hypothesized that physician burnout will have an inverse relationship with the time spent at the bedside by physicians. In a cross-sectional study, we surveyed patients on their perception of the time spent by their physician on the day of the survey (4 categories: 0-5, 6-10, 11-15, >15 minutes).

Ankle fractures are an extremely common orthopaedic injury treated by surgeons on a routine basis. The deltoid ligament is torn in a large number of these fractures and is commonly seen with associated radiographic changes of medial clear space widening. The clinical relevance of addressing the injured deltoid ligament with acute surgical repair has been debated for decades.
