Article Synopsis
  • The study analyzed the timing of invasive coronary angiography in high-risk patients with non-ST-segment-elevation acute coronary syndrome, comparing early (within 12 hours) and standard (48-72 hours) strategies.
  • Among 2,092 patients, those with a higher GRACE risk score (>140) showed a decreased risk of death with early intervention, while those with a lower score (≤140) had an increased risk.
  • The findings suggest a significant relationship between the timing of treatment and GRACE score in affecting mortality, indicating a need for further clinical trials.

Pathogenic variants in the Wnt-pathway co-receptor low-density lipoprotein (LDL) receptor-related protein 5 (LRP5) cause high bone mass (LRP5-HBM) due to insensitivity to the endogenous antagonist of Wnt-signaling. Although indicating incessant progression of BMD and biomarkers reflecting bone formation, this has not been confirmed in individuals with LRP5-HBM. We investigated how the LRP5-HBM bone phenotype changes with age in adults and is associated with quantitative changes of bone turnover markers and bone-related microRNAs (miRNAs) in the circulation.

It is generally anticipated that particulate organic carbon (POC) for most part is degraded by attached microorganisms during the descent of "marine snow" aggregates toward the deep sea. There is, however, increasing evidence that fresh aggregates can reach great depth and sustain relatively high biological activity in the deep sea. Using a novel high-pressure setup, we tested the hypothesis that increasing levels of hydrostatic pressure inhibit POC degradation in aggregates rapidly sinking to the ocean interior.

Background Sodium-glucose cotransporter 2 inhibitors reduce hospitalizations for heart failure and cardiovascular death, although the underlying mechanisms have not been resolved. The SIMPLE trial (The Effects of Empagliflozin on Myocardial Flow Reserve in Patients With Type 2 Diabetes Mellitus) investigated the effects of empagliflozin on myocardial flow reserve (MFR) reflecting microvascular perfusion, in patients with type 2 diabetes mellitus at high cardiovascular disease risk. Methods and Results We randomized 90 patients to either empagliflozin 25 mg once daily or placebo for 13 weeks, as add-on to standard therapy.

Osteoporosis and cardiovascular (CV) disease share common risk factors and pathophysiology. Low bone mineral density (BMD) and fractures appear to increase the risk for multiple CV diseases. Equally, prevalent CV disease appears to predispose to bone loss and increase fracture rates.

Article Synopsis
  • Hadal trenches, the deepest ocean regions, show significant prokaryotic activity, and this study examined how prokaryotes and viruses vary in abundance in these areas compared to adjacent abyssal sites.
  • In the water column, both prokaryotic and viral abundance decreased with depth but stabilized around 4000 meters, while viruses became more prevalent relative to prokaryotes due to less organic material available.
  • Sediment samples from hadal trenches displayed variable prokaryotic and viral concentrations influenced by fluctuations in total organic carbon, indicating a more dynamic environment compared to the more stable abyssal sediments, suggesting a critical role for viruses in carbon recycling in these extreme habitats.

The paper studies the containment effects of public health measures to curb the spread of Covid-19 during the first wave of the pandemic in spring 2020 in Germany. To identify the effects of six compound sets of public health measures, we employ a spatial difference-in-differences approach. We find that contact restrictions, mandatory wearing of face masks and closure of schools substantially contributed to flattening the infection curve.

Article Synopsis
  • The study investigates cardiogenic shock (CS) by using machine learning to identify distinct patient phenotypes, highlighting variations in clinical profiles and in-hospital mortality rates.* -
  • Researchers analyzed data from 1959 CS patients across three international cohorts and identified three clusters: "non-congested," "cardiorenal," and "cardiometabolic," with the latter showing the highest mortality risk.* -
  • The findings suggest that understanding these phenotypes could enhance patient-specific treatment strategies and improve clinical trial enrollment for more effective management of CS.*

Background Overweight adults have low circulating concentrations of ANP (atrial natriuretic peptide) and proANP fragments. We tested the hypothesis that an intensive lifestyle intervention with an intended weight loss would increase plasma concentrations of a proANP fragment in overweight children. Methods and Results We measured MR-proANP (midregional proANP) concentrations in plasma from overweight children who participated in the OOIS (Odense Overweight Intervention Study).

Background Cardiorespiratory fitness may moderate the association between obesity and all-cause mortality (ie, the "fat-but-fit" hypothesis), but unaddressed sources of bias are a concern. Methods and Results Cardiorespiratory fitness was estimated as watts per kilogram from a submaximal bicycle test in 77 169 men and women from the UK Biobank cohort and combined with World Health Organization standard body mass index categories, yielding 9 unique fitness-fatness combinations. We also formed fitness-fatness combinations based on bioimpedance as a direct measure of body composition.

Immunoglobulin G4 (IgG4)-related disease is a systemic fibroinflammatory disorder that can affect almost any tissue. Isolated IgG4 pleural disease is a rare manifestation and, when present, is usually described in patients presenting with dyspnoea. We present a case of asymptomatic isolated IgG4 pleural effusion and highlight that IgG4-related disease should be remembered as a differential diagnosis in patients with pleural effusion and pleural thickening, even if asymptomatic and without any other organ involvement.
